Cutaneous embolization of cardiac myxoma.

Atrial myxoma is the most common primary tumour of the heart. Skin manifestations in patients with a cardiac myxoma are frequent and may be due to cutaneous emboli, or may be specific findings as part of more complex syndromes. We present a 33-year-old-man with a history of episodes of pain in both legs and an ischaemic neurological event, who also had episodes of acral papular erythematous lesions on the legs and feet including the soles. The histological finding of dermal vessels occluded by a myxomatous material was the clue to the diagnosis of a cardiac myxoma. The diagnosis of this entity can be very difficult, because of the broad spectrum of clinical features; rarely the skin manifestations lead to the diagnosis of this tumour. The histological recognition of the myxomatous emboli is of vital importance for the diagnosis and treatment of this disease.

Linear discrete population models with two time scales in fast changing environments I: autonomous case.

In this work we consider a structured population with groups and subgroups of individuals. The intra-group dynamics is assumed to be fast in comparison with the inter-group dynamics. We study linear discrete models where the slow dynamics is represented by a single matrix and the fast dynamics is described by means of the first k terms of a converging sequence of different matrices. The number k can be interpreted as the ratio between the two time scales. The aim of this work is to extend aggregation techniques to the case of fast changing environments. The main idea of aggregation is to build up a new system, with lower dimension, that summarizes the information concerning the fast process. This "aggregated" system provides essential information on the original one. It is shown that the asymptotic behavior of the original system can be approximated by the asymptotic behavior of the aggregated system when the ratio between the two time scales is large enough. We present an example of an age structured population in a patchy environment. The migration process is assumed to be fast in comparison with the demographic process. Numerical simulations illustrate that the asymptotic growth rate and the stable age distribution of the population in the original and the aggregated systems are getting closer as the ratio k increases.

Occupational asthma caused by exposure to cyanoacrylate.

BACKGROUND: Exposure to acrylates may give rise to rhinitis and asthma in both industrial and domestic environments. The mechanisms underlying these respiratory conditions caused by acrylates remain largely unknown. METHODS: We studied two assembly operators exposed to cyanoacrylate glue who developed rhinitis and asthma symptoms. The causal relationship of these symptoms to cyanoacrylate glue exposure was investigated by serial peak expiratory flow (PEF) monitoring at work and off work. Moreover, inhalation testing was performed by asking the patients to mimic exposure at work with the cyanoacrylate glue in a 7-m3 challenge chamber. RESULTS: Serial PEF monitoring at work and away from work was consistent with occupational asthma in both patients. The methacholine inhalation test was negative in patient 1 (off work) and showed bronchial hyperresponsiveness in patient 2. After 20-min exposure to cyanoacrylate, the patients had late and progressive asthmatic reactions, respectively, and the methacholine test became positive in patient 1. Induced-sputum samples obtained 3 and 24 h after the cyanoacrylate challenge showed a marked increase in sputum eosinophils. CONCLUSION: Exposure to cyanoacrylate in these patients provoked not only variable airflow limitation and bronchial hyperresponsiveness, but also pronounced eosinophilia in sputum.

Full-thickness quadriceps tendon: An easy cruciate reconstruction graft.

Full-thickness quadriceps tendon strength has been found to be similar or higher than that of the patellar tendon. Current techniques spare the deep vastus intermedius to avoid piercing the suprapatellar pouch, which might result in loss of visualization. This approach mainly results in loss of graft thickness and surgical problems related to the dissection made through the tendon. We describe a technique where a full-thickness graft can be used, making tendon stripping easier. We also describe how to preserve visualization during surgery. A double anchorage of the tendinous part of the graft on the femoral side is used together with fixation of the bone end on the tibial side, allowing early motion and thereby avoiding suprapatellar pouch adhesions.

The Bayesian controversy in animal breeding.

Frequentist and Bayesian approaches to scientific inference in animal breeding are discussed. Routine methods in animal breeding (selection index, BLUP, ML, REML) are presented under the hypotheses of both schools of inference, and their properties are examined in both cases. The Bayesian approach is discussed in cases in which prior information is available, prior information is available under certain hypotheses, prior information is vague, and there is no prior information. Bayesian prediction of genetic values and genetic parameters are presented. Finally, the frequentist and Bayesian approaches are compared from a theoretical and a practical point of view. Some problems for which Bayesian methods can be particularly useful are discussed. Both Bayesian and frequentist schools of inference are established, and now neither of them has operational difficulties, with the exception of some complex cases. There is software available to analyze a large variety of problems from either point of view. The choice of one school or the other should be related to whether there are solutions in one school that the other does not offer, to how easily the problems are solved, and to how comfortable scientists feel with the way they convey their results.

Disruption and functional analysis of six ORFs on chromosome IV: YDL053c, YDL072c, YDL073w, YDL076c, YDL077c and YDL080c.

Six open reading frames (ORFs) from chromosome IV, YDL053c, YDL072c, YDL073w, YDL076c, YDL077c and YDL080c, were disrupted using the long flanking homology technique (LFH) to replace each target locus with the KanMX4 selection marker. We have also constructed plasmids containing replacement cassettes (pYORC) and the cognate clones (pYCG) for each ORF. Disruption of five of the ORFs-YDL053c, YDL072c, YDL073w, YDL076c and YDL080c (THI3)-resulted in no distinctive phenotype with respect to temperature or nutritional requirements. However, disruption of YDL077c (also known as VAM6) exhibited an slow growth phenotype in minimal media and also in rich media containing glycerol as a carbon source. The homozygous disruptant diploid corresponding to this gene also failed to sporulate.

Genetic analysis of growth curve parameters for male and female chickens resulting from selection on shape of growth curve.

The objectives of this research were to evaluate gender differences and selection on body weight as they affect growth curves of chickens. Marginal posterior densities of growth curve parameters were studied by Gibbs sampling on 10,671 male and female chickens originating from five lines. Line X-+ was selected on low body weight at 8 wk (BW8) and high body weight at 36 wk (BW36), line X+- on high BW8 and low BW36, X++ on high BW8 and BW36, X-- on low BW8, and BW36, and X00 was an unselected control line. Growth was modeled by a Gompertz function. Heritabilities and genetic correlations among parameters of the Gompertz curve were estimated. Marginal posterior densities were drawn for parameters of the growth curve and for sexual dimorphism at ages ranging from hatching to 1 yr. Lines selected for a higher BW8 had higher initial specific growth rates (L), higher maturation rates (K), and lower ages at inflection (T(I)). Lines selected for a higher BW36 had higher asymptotic body weights (A). Estimates of A, L, and T(I) were higher in males and K was higher in females. Difference between sexes for A was greater in the line selected for a lower BW8 and a higher BW36. Dimorphism for L and K was the lowest in lines X++ and X--, respectively. The greatest difference in T(I) was observed in the line selected for lower BW8 and BW36. Sexual dimorphism of body weight was lower at most ages in the lightest line. Before 15 wk, sexual dimorphism in X++ line was lower than in the line selected for higher BW8 and lower BW36. The increase in sexual dimorphism with body weight could be reduced by selecting animals on body weight at two ages instead of one, as is usually done in commercial lines.

Expression and secretion of Bacillus polymyxa neopullulanase in Saccharomyces cerevisiae.

We have isolated the gene encoding the neopullulanase enzyme from Bacillus polymyxa CECT 155. It consists of an open reading frame of 1545 bp that could code for a protein of 515 amino acids. This open reading frame was expressed in Bacillus subtilis and the corresponding transformants produced extracellular neopullulanase. The neopullulanase gene was also expressed in Saccharomyces cerevisiae placing it under the control of the yeast actin gene (ACT1) promoter. Clones containing the intact neopullulanase gene, including its own bacterial signal sequence, gave rise to the synthesis of active, but intracellular, enzyme by S. cerevisiae transformants. When sequences specifying the signal sequence and leader region of the yeast mating pheromone alpha-factor (MF alpha 1) were fused upstream of the gene encoding the neopullulanase enzyme, the enzyme was secreted by S. cerevisiae. The secreted protein presented the same biochemical properties and the same apparent molecular mass as the Bacillus polymyxa original enzyme. The predicted amino acid sequence of the neopullulanase protein contained sequence motifs conserved among amylolytic enzymes. Northern blot analysis indicated that the transcription of the neopullulanase gene in B. polymyxa was induced by the presence of the substrate, pullulan, in the culture, and was repressed by glucose.

Expression of LIP1 and LIP2 genes from Geotrichum species in Baker's yeast strains and their application to the bread-making process.

Lipolytic baker's yeast strains able to produce extracellular active lipase have been constructed by transformation with plasmids containing the LIP1 and LIP2 genes from Geotrichum sp. under the control of the Saccharomyces cerevisiae actin promoter (pACT1). Lipase productivity differed between both constructs, YEpACT-LIP1-t and YEpACT-LIP2-t, being higher for the strain bearing the LIP2 gene in all culture media tested. This result appeared not to be the consequence of a defect in the transcription of the LIP1 gene as revealed by Northern blot analysis. Replacing the signal sequence of LIP1 by that of LIP2 in the YEpACT-LIP1-t plasmid enhanced significantly the secretion of lipase 1, but the levels of lipase activity were still lower than those found for the YEpACT-LIP2-t transformant. Recombinant lipase 2 protein produced by baker's yeast exhibited biochemical properties similar to those of the natural enzyme. Fermented dough prepared with YEpACT-LIP2-t-carrying cells rendered a bread with a higher loaf volume and a more uniform crumb structure than that prepared with control yeast. These effects were stronger by the addition in the bread dough formulas of a preferment enriched in recombinant lipase 2.

Roles of bicarbonate, cAMP, and protein tyrosine phosphorylation on capacitation and the spontaneous acrosome reaction of hamster sperm.

Capacitation is a prerequisite for successful fertilization by mammalian spermatozoa. This process is generally observed in vitro in defined NaHCO3-buffered media and has been shown to be associated with changes in cAMP metabolism and protein tyrosine phosphorylation. In this study, we observed that when NaHCO3 was replaced by 4-(2-hydroxyethyl)1-piperazine ethanesulfonic acid (HEPES), hamster sperm capacitation, measured as the ability of the sperm to undergo a spontaneous acrosome reaction, did not take place. Addition of 25 mM NaHCO3 to NaHCO3-free medium in which spermatozoa had been preincubated for 3.5 h, increased the percentage of spontaneous acrosome reactions from 0% to 80% in the following 4 h. Addition of anion transport blockers such as 4,4'-diiso thiocyano-2, 2'-stilbenedisulfonate (DIDS) or 4-acetomido-4'-isothiocyanatostilbene-2,2'-disulfonic acid (SITS) to the NaHCO3-containing medium inhibited the acrosome reaction, with maximal inhibition at 600 microM, and with an EC50 of 100 microM. Increasing either extracellular or intracellular pH did not induce the acrosome reaction in NaHCO3-free medium. In contrast, addition of 500 microM dibutyryl cAMP (dbcAMP), alone or together with 100 microM 1-methyl-3-isobutylxanthine (IBMX), induced the acrosome reaction in spermatozoa incubated in NaHCO3-free medium. These compounds also partially reversed the inhibition of the acrosome reaction caused by the DIDS or SITS in complete medium. In contrast to these results, IBMX or dbcAMP did not induce acrosome reactions in cells incubated in Ca2+-free medium. When hamster sperm were incubated in the absence of NaHCO3 or in the presence of NaHCO3 and DIDS, cAMP concentrations were significantly lower than the values obtained from sperm incubated in complete medium. Protein tyrosine phosphorylation has also been shown to be highly correlated with the onset of capacitation in many species. During the first hour of capacitation, an increase in protein tyrosine phosphorylation was observed in complete medium. In the absence of NaHCO3, the increase in protein tyrosine phosphorylation was delayed for 45 min, and this delay was overcome by the addition of dbcAMP and IBMX. The induction of the acrosome reaction by calcium ionophore A23187 in NaHCO3-free medium was delayed 2 h, as compared with control medium. This delay was not observed in the presence of dbcAMP and IBMX. Taken together, these results suggest that a cAMP pathway may mediate the role of NaHCO3 in the capacitation of hamster spermatozoa and that protein tyrosine phosphorylation is necessary but not sufficient for complete capacitation.

Bayesian inference of genetic parameters and selection response for litter size components in pigs.

Three contemporary lines were formed from the progeny of 50 French Large White sows. In the first line, gilts were selected for ovulation rate at puberty. In the second line, they were selected for prenatal survival of the first two parities, corrected for ovulation rate. The control constituted the third line. Ovulation rate at puberty was analyzed using an animal model with a batch effect. Prenatal survival was analyzed with a repeatability animal model that included batch and parity effects. Flat priors were used to represent vague previous knowledge about parity and batch effects. Additive and residual effects were represented assuming that they were a priori normally distributed. Variance components were assumed to follow either uniform or inverted chi-square distributions, a priori. The use of different priors did not affect the results substantially. Heritabilities for ovulation rate ranged from 0.32 to 0.39, and from 0.11 to 0.16 for prenatal survival, depending on the prior used. The mean of the marginal posterior distribution of response to four generations of selection ranged from 0.38 to 0.40 ova per generation, and from 1.1 to 1.3% of the mean survival rate for average survival per generation.

Relationships of meat characteristics of two lines of rabbits selected for litter size and growth rate.

We measured meat characteristics of 46 commercial rabbit carcasses from two synthetic breeds selected for litter size or growth rate. Color measurements (CIELAB) were made on several muscle surfaces. Color measured on the muscular aponeurosis indicated a pale carcass at most of the points; Chroma (C*) and hue (H*) varied from 3.71 and 31.1, respectively, for longissimus at the 7th lumbar vertebra to 14.3 for C* of trapezius and 56.1 for H* of biceps femoris. Color of the longissimus cut surfaces differed substantially from the exterior surface color. The 1st lumbar vertebra cut also differed from the 7th lumbar vertebra cut. The pH of the biceps femoris was 5.80 and that of the longissimus was 5.75. Meat fat content of half a carcass was 5.28%. The first four principal components of an analysis with color and pH measurements of the longissimus and biceps femoris and meat fat content explained 68% of the variance. The entire set of variables was well summarized by the lightness (L*) and Chroma of the longissimus muscle exterior surface measured at the 4th lumbar vertebra, longissimus pH, and meat fat content. When the data were projected on the plane defined by the first two principal components, two separate groups of points appeared, corresponding to the animals of each breed.

Divergent selection for uterine capacity in rabbits.

To examine the genetic and phenotypic parameters of uterine capacity and its components in rabbits, we performed a divergent selection experiment for uterine capacity. Rabbit does were unilaterally ovariectomized, and a laparoscopy was performed at midgestation to count the number of corpora lutea and implanted embryos. Selection was performed on litter size in all parities. Genetic parameters were estimated by residual maximum likelihood. For most traits, phenotypic and genetic correlations were in the same range. In unilaterally ovariectomized rabbit does, litter size had a low genetic (.34) and phenotypic (-.08) correlation with ovulation rate, a high genetic (.71) and phenotypic (.60) correlation with the number of implanted embryos, and a high genetic correlation (.89) and moderate phenotypic correlation (.51) with fetal survival. Embryonal survival had a low phenotypic (-.26) or genetic (.12) correlation with fetal survival. A component of uterine capacity, fetal survival in one overcrowded uterine horn, had a low heritability (.05), and consequently limited possibilities for improvement by selection. Uterine capacity, estimated as litter size in one overcrowded uterine horn, had a moderately low heritability (.16). Divergent selection on uterine capacity was effective and a correlated response was found in implanted embryos. No correlated response was found in fetal survival and number of dead fetuses between implantation and birth.

Silent myocardial ischaemia and left ventricle hypertrophy in diabetic patients.

The purpose of this study was to evaluate the ability of three noninvasive techniques to detect silent myocardial ischaemia and analyse the factors associated with this condition, particularly left ventricular hypertrophy, in diabetic patients. An ECG stress test, a thallium-201 myocardial scintigraphy with dipyridamole intravenous infusion, ambulatory 48 h ECG monitoring and an echocardiographic study were performed in 92 diabetic patients without cardiac symptoms but with > or = 2 additional cardiovascular risk factors. At least one of these tests was positive in 28 patients (30.4%), suggesting silent myocardial ischaemia. Twenty-four of these patients had a coronary angiography which showed significant coronary stenosis in only 9 cases. An accurate echocardiographic tracing was obtained in 79 patients, particularly in 7 of the 9 with coronary stenosis. Left ventricular hypertrophy was detected in 34 patients, 6 of whom had coronary stenosis. In patients with left ventricular hypertrophy, the positive predictive values of myocardial scintigraphy and the ECG stress test were respectively 50% and 100%, as compared to only 33% and 11% in those without hypertrophy. In summary, coronary stenoses were found in < 10% of asymptomatic diabetic patients with > or = 2 cardiovascular risk factors, but more frequently in individuals with left ventricular hypertrophy. Thus, silent myocardial ischaemia should be searched for first in diabetic patients with hypertrophy, for which the stress test was the most accurate detection method in this study.
