[Continuous plexus anesthesia to improve circulation in peripheral microvascular interventions].

Five patients were demonstrated: one finger replantation, three toe-to-finger transfers and one finger-finger transfer. An axillary plexus catheter was inserted for postoperative sympathetic blockade. This brought about spasmolysis, an increase in the circulation and an increase in the acral systolic blood pressure. Simultaneously an adequate analgesic effect was achieved through the administration of local anaesthesic. The muscle relaxation assisted the immobilisation of the extremity. The improved circulation, the analgesic effect and the suppression of the muscular activity through continuous plexus anaesthesia have a beneficial influence on healing in replanted or transferred digits.

[Restoration of an occipital cerebrospinal fluid fistula following roentgen injury].

After intensive radiation treatment following resection of a cerebellar tumor, necrosis of the occipital bone led to a chronic cerebrospinal fluid fistula. Because of eventual complications (constant loss of cerebrospinal fluid, meningitis, herniation of the brain) the chronic cerebrospinal fluid fistula needs to be treated as soon as possible. The treatment includes radical resection of the necrotic tissue, direct closure of the cerebrospinal leakage and stable soft tissue coverage. In the case presented closure was achieved with a free autologous fascia graft, cover with a myocutaneous trapezius island flap.

Cerebellar Neoplasms↗

[Restoration of hand grip ability by combined tissue transfer].

The coordination of such classical methods as skin grafts, bone grafts or pedicled flaps with microsurgical techniques opens new areas in the field of hand surgery. The combination resulted in useful function of three mutilated hands. A clear concept and good postoperative physiotherapy are important to achieve good results in these cases.

Periareolar abscess in the nonlactating breast.

Periareolar abscesses in nonlactating women are usually accompanied by a sinus tract that communicates with the nipple. This abnormal tract represents the source of the initial invasive infection and, if left unattended, may be the nidus for recurrence. The medical records of 32 patients with periareolar abscess treated at Temple University Hospital from January 1970 through January 1980 were reviewed. Follow-up data were obtained recently for 21 patients, including 1 patient with bilateral disease. Several patients also had an inverted nipple. Nineteen patients denied recurrence after our initial treatment and two required a secondary procedure for cure. Only one patient was dissatisfied with the postoperative appearance of the surgical site. None required partial or simple mastectomy for cure, as reported elsewhere. Excision of the sinus tract and, when present, correction of the inverted nipple as herein described are necessary to prevent reinfection. Simple abscess excision or incision and drainage is usually inadequate therapy.

[Hyperbaric oxygen therapy in experimental necrotising pancreatitis (author's transl)].

In the last years non operative and surgical efforts could not diminish the high lethality of severe acute haemorrhagic necrotising pancreatitis. While majority of patient die in consequence of pancreatitis-shock, hyperbaric oxygen therapy (HBO) can improve all hypoxic circulation situations. Therefore the value in treatment of experimental necrotising pancreatitis in pig is examined. 5 pigs were treated with, 5 without HBO and 5 served as control. With HBO liquid sequestration was diminished and total protein loss prevented significantly, but foremost survival time was significantly prolonged. In consequence demarcation of necrosis with connective tissue was possible, but nevertheless operative treatment remain necessary. Without HBO all pigs and with HBO only 2 pigs died in consequence of necrotising pancreatitis. It is demonstrated, that HBO as additive therapy can improve the prognosis of necrotising pancreatitis.

Acute Disease↗

[A "new joint" following articular panaritium].

This is a report of a 74 year old female patient with a septic arthritis of the DIP-joint of the left index finger. Treatment consisted of joint debridement and placement of one Gentamycin-PMMA-Bead in the residual defect. The bead was then left in the defect as a fulcrum for painfree motion in the new DIP-joint.

[Microsurgery in bone tumors. Recent treatment perspectives].

The trend in the treatment of bone tumors links to the preservation of function. This can be achieved by a better operative technique and interdisciplinary cooperation. The microsurgery offers here new ways. After radical resection of the tumor, the defect is bridged by a plate and a microvascular free fibula graft. Additional cancellous bone grafts produced a stable reconstruction of the resected area.

[Preservation and reconstruction of the thumb with the aid of microvascular surgery (author's transl)].

Report about two cases of severe injuries to the thumb. One severe crush injury was treated primarily; in the other case of complete amputation a late reconstruction was performed. Methods and modern concepts for these problems are discussed. The importance of long vein grafts in microsurgery is pointed out. The advantage of reconstructing all structures primarily is stressed. Good functional results are presented to support these concepts.

[Myelopathies: behavior of cerebrospinal fluid cells and proteins].

Data on cerebrospinal fluid (CSF) cells and proteins (total proteins and gamma globulins content) are reviewed in 287 patients with myelopathies. Clinical data on these patients were reported. Results obtained are distributed according to diagnoses and to time of disease (tables 1 and 2). Informations obtained through the study show that signalization of CSF system is related to the episode responsible by the disease, when the episode is singular. Repeated episodes or a progressive evolution are more apt to produce a maintained signalization of CSF cyto-protein dual. In this way data on infectious myelopathies, multiple sclerosis and neuromyelitis optica are compared to those found in post-vaccination, post-infection and post-intoxication myelopathies, as well as to those found in B-12 deficiency and myelopathies of vascular origin. Primary myelitis are evaluated in report to this comparison. It is shown that changes in the CSF cyto-protein dual are more related to those found in multiple sclerosis and neuromyelitis optica when chronic primary myelitis is considered. In acute and sub-acute primary myelitis they are related to those found in post-vaccination, post-infection and post-intoxication myelopathies.
