[Myocutaneous flap as reliable defect coverage in high grade pelvic decubitus ulcers. Classification, therapeutic concept and presentation of personal patient sample of 16 years].

Infected pelvic pressure sores of Campbell stages IV-VII require soft tissue reconstruction, which means stable, multi-layered filling cover of the defect and reliable prophylaxis of relapse. Myocutaneous flaps meet these conditions well. Depending on the extent and the area of the sore, with predilection for the sacrum, the ischial tuberosity and the femoral trochanter, the gluteus maximus, biceps femoris and tensor fasciae latae muscles are most often used for myocutaneous flaps. Primary sutures, split skin grafts or local fasciocutaneous flaps are often sufficient treatment for smaller, superficial defects. Between 1981 and 1996, 133 patients (average age 50 years) with 212 pelvic pressure sores of all stages were treated in our clinic. After radical decubitus excision with pseudotumor technique and resection of the osseous prominences, one-stage reconstruction of solitary as well as multiple defects was performed with myocutaneous flaps in 135 cases. The postoperative general complication rate for all treatments was about 10-30%. With regard to the muscle flaps, one third healed without any problems, partial flap necrosis occurred in 6% and there was total loss of flap in 2% of all myocutaneous flaps. According to present knowledge, myocutaneous flaps seem to be the most reliable method for definitive covering of deep pelvic pressure sores, independent of the cause of the ulcer.

[Analysis of cost measures in treatment of severely burned patients].

An analysis of costs and profit and an evaluation of the different cost parameters was performed in our burn centre, which treated 50-80 patients per year between 1991 and 1995. The cost-profit relationship (costs 3.7-4.6 million DM/year, profit 3.8-5.0 million DM/year) is balanced. The main cost parameters are staff costs (1.3-1.9 million DM/year), costs for medical treatment (1.5-1.8 million DM/year), and costs for internal services (0.6-0.9 million DM/year).

[2-stage surgery of neoplastic left colonic obstruction remains the safest procedure].

From January 1989 to December 1996, 56 consecutive patients underwent emergency surgery for occlusive cancer of the left colon. Excepting 12 patients whose symptomatology and radiographic presentation required medial laparotomy, the elective procedure was initial colostomy. There were 11 men and 33 women, mean age 76 years (range 50-97). Two patients in poor general status (ASA III) died during the postoperative period. Among the 42 survivors, the second procedure was not performed because of poor general status or disease progression in 6. Mean delay to the second procedure for resection was 11.5 days; during the same hospitalization for 32 out of 36 patients. The second procedure was segmentary colectomy in 34 cases and limited to exploratory laparatomy because of inextricable lesions in 2. Among the 34 re-operations with segmentary colectomy, the ostomy was removed in 28 at the second procedure and a third procedure was required in 6 cases. All the anastomoses in this series were sutured manually. Mortality for re-operation was nil. Pathology results (Duke's classification) in the 36 reoperated patients was: stage B = 3, stage C = 19, stage D = 14. In this series, operative mortality only concerned those patients whose condition was incompatible with selective surgery for colostomy. This risk cannot be lowered by any, other surgical approach. For the 34 resection-anastomosis elective operations, no major complications or deaths were observed. These results led us to recommend two stage surgery as routine strategy since survival of all those patients capable of sustaining an elective colostomy in an emergency setting can be assured.

[Treatment of congenital dilatation of the bile ducts. Apropos of 3 cases].

Congenital dilatation of the bile duct is an uncommon lesion requiring surgical treatment. Complete excision of the diseased bile duct prevents development of carcinoma of the bile duct. We report three cases of congenital dilatation of the biliary three, each illustrating a particular aspect of the therapeutic strategy. One patient had an unusual anomaly of the pancreaticobiliary junction that required pancreaticoduodenectomy. Another patient developed carcinoma of the bile duct 15 years after an internal derivation of a choledochal cyst. The last patient had dilatations of the common and intrahepatic bile ducts associated with congenital hepatic fibrosis.

[False radiologic pneumoperitoneum. The value of emergency abdominal computed tomography].

We report 2 cases of false pneumoperitoneum. The diagnosis was suspected on an upright chest or abdominal plain films. The absence of clinical signs and symptoms of peritoneal irritation suggested a wrong radiological diagnosis. Emergency CT scan definitely eliminated a pneumoperitoneum and suggested the right diagnosis in both cases. This exam may play the decisive role in preventing needless emergency laparotomy.

Hepatitis C plasma viral load is associated with HCV genotype but not with HIV coinfection.

The influence of human immunodeficiency virus (HIV) coinfection and hepatitis C virus (HCV) genotype distribution on HCV viral load and alanine amino transferase (ALT) levels in chronically infected patients remains unclear. In the present study, serum samples from a group of haemophiliac patients were investigated retrospectively. HCV geno- and subtyping was carried out using the Inno line probe assay (Inno LIPA, Innogenetics, Zwijnaarde, Belgium) in 87 patients positive by HCV RT PCR. Of these patients, 31 (35.6%) were HIV coinfected. HCV RNA was quantified with the HCV Monitor kit (Roche, Basel, Switzerland) in 43 patients (22 HIV-negatives, 21 HIV-positives). The most prevalent genotypes were 1 (n = 52) and 3a (n = 16) followed by genotype 2 (n = 9) and 4 (n = 3). Mixed infections were detected in 7 patients. Of genotype 1 positive samples, 24 and 23 were classified as subtype a and b, respectively. Five samples could not be subtyped. Although higher mean values of ALT were observed in genotype 1 infected patients, there was no statistically significant association between HCV genotype or subtype and liver enzymes (P > 0.05). On the other hand, statistically significant higher HCV RNA titres were observed in haemophiliacs infected with HCV genotype 1 in comparison to those infected with other genotypes (P < 0.01). No relationship was found between the presence of HIV coinfection and viral load of HCV RNA. There was no evidence that HCV infection had a more severe outcome in HIV-positive patients who had been infected with HIV and HCV more than ten years ago, even in those with very low CD4+ cell counts. No clear association between high ALT levels and large amounts of viral RNA was observed. In conclusion, a large viral load is associated with HCV genotype 1 infection; HIV coinfection has no clear effect on the intensity of HCV replication. An ongoing prospective study will evaluate the respective role of viral load, genotype, HIV coinfection and ALT level in the response to interferon therapy.

Measurement of amniochorionic membrane thickness using high-frequency ultrasound.

Premature rupture of the membranes (PROM) accounts for approximately 30 per cent of all preterm deliveries. PROM is thought to be mainly due to a decrease in membrane integrity. The aim of our investigation was to determine, post-partum after 28 normal deliveries, the thickness of the amniochorionic membrane using a 20 MHz high-frequency ultrasound. The data obtained were compared with histological sections for measurement accuracy using a linear regression analysis method. The membrane thickness of the total study group was 0.83 +/- 0.11 mm (0.72-1.08 mm). Based on a statistical comparison with the histological sections, the high-frequency ultrasound examination was shown to be highly reliable, with a correlation coefficient of r = 0.96 (P < 0.0001). High-frequency ultrasonographic examinations of membrane thickness are an objective and reliable method and may be a gain to prenatal diagnostics once this method can be used in vivo.

True aneurysms of the palmar arch of the hand: a report of two cases.

We report on two cases of true aneurysms of the palmar arch, both of which were post-traumatic. One was due to repetitive blunt trauma and the other to a glass cut wound. Both aneurysms were detected clinically as pulsating masses without functional disorders. The presence of elastic fibres and smooth muscle in the aneurysm wall proved it to be a true aneurysm. Both aneurysms were excised and replaced by a vein graft from the foot.

Technetium-99m human immunoglobulin (HIG): a new substance for scintigraphic detection of bone and joint infections.

Technetium (99m-Tc)-labelled, polyclonal human immunoglobulin (HIG) has been described as a new agent to detect local infection and inflammation. In this study, we tested 99m-Tc HIG in 55 patients with suspected chronic (n = 42) and acute (n = 13) skeletal infection. Diagnosis was proven operatively (n = 44) and clinically (n = 11), including microbiological culture tests (n = 46). A gamma camera scan was performed 4 and 24 hours after I.v. injection of 500 MBq 99m-Tc-HIG. 99m-Tc-HIG scanning achieved a sensitivity of 91% and a specificity of 93%. We found one false negative and five false positive scintigraphic results in 55 patients. No clinical or biochemical side effects were encountered after 99m-Tc-HIG injection. We recommend this technique especially for localisation of low-grade, chronic osteomyelitis. The mechanisms and kinetics of 99m-Tc-HIG, however, are worth investigating more extensively.

[Use of endoscopy in plastic surgery].

Now that endoscopic techniques have been established in visceral and trauma surgery under the concept of minimal invasive surgery, plastic surgery has also begun to accept these techniques since minimal invasive surgery is very important in this field. In contrast to abdominal and thoracic surgery, plastic surgery cannot be done in preformed cavities. Therefore, it was necessary to develop new techniques and instruments. We present the most important operations in the field of plastic surgery that can so far be carried out endoscopically. The development phase is only at the beginning, and new indications are being added daily since the technical equipment can also only be gradually adapted to the needs that arise.

[Model for a computer-assisted quality assurance system in burn intensive care].

The prerequisites for an integrated quality management system for hospitals were developed as a result of considerations regarding the necessity of an appropriate quality-assurance system for the intensive care sector. The basis for a dynamic quality-assurance system on the documentational level is a comprehensive data base containing patient files unlimited in terms of time and data technology. The necessary hardware and software structure will be provided by consistent application of a client-server architecture. Permanent surveillance of existing quality objectives within the scope of an economic close-loop system requires a system with flexible query mechanisms. Informational data access from all hierarchical level within the hospital organization is facilitated by the data warehouse concept, a data base system with subject-oriented, integrated, time-variable, and persistent data.

Systemic liberation of interleukin-1 beta and interleukin-1 receptor antagonist in the perioperative phase of liver transplantation.

We measured systemic serum levels of interleukin-1 receptor antagonist (IL-1ra), interleukin-1 beta (IL-1 beta), tumor necrosis factor alpha (TNF-alpha), and interleukin-6 (IL-6) during the preoperative, anhepatic, and postreperfusional phases up to the 7th postoperative day in 60 patients undergoing orthotopic liver transplantation (LTx). In contrast to IL-1 beta, IL-1ra, TNF-alpha, and IL-6 showed a significant elevation in relation to the early phase after reperfusion, while TNF-alpha displayed a high grade of scatter. In addition, IL-1ra levels were significantly elevated during the anhepatic phase. Maximum serum levels were found at 15 min after reperfusion, 120 min after reperfusion, and on the 1st postoperative day, respectively. Serum levels decreased considerably at 24 h and 7 days after reperfusion. The comparative monitoring of systemic cytokine and cytokine antagonist levels, in particular the liberation of IL-1ra and IL-6 may provide useful parameters for the development of new liver preservation theories for LTx.

[Treatment of mentosternal contractures by flap-plasty].

Mentosternal contractures still represent a surgical challenge due to their exposed location. They require early operative treatment for both functional as well as aesthetic reasons. Careful clinical examination of scar location and traction forces both in the resting and functional, moving state including proper evaluation of the surrounding soft tissue is mandatory for exact preoperative planning of the reconstructive surgical procedure required. In general, the technically most feasible operation is favoured, if functional and aesthetic results are good and the postoperative risk of recurrent mentosternal contractures is low. Between 1987 and 1994, 21 patients with cervical, mentosternal and mentothoracic contractures underwent operative procedures at our clinic. Eight patients underwent a surgical reconstruction with local flaps and 13 patients received a free microvascularized flap.

[Budget analysis in a burn center for severely burned patients].

The treatment of patients with severe burn injuries is expensive and belongs in the hands of specialized burn centres. In our burn centre 257 patients were treated from 1991 to June 1995 (between 50 and 60 patients per year except for 1993 with 77 patients). Patients were evaluated according to the ABSI score (mean 7) and had mortality of around 15%. The costs range between 3.7 and 4.8 million DM per year, the profit lies in the same range. The costs are divided into staff costs (1.4-1.8 million DM per year), costs for medical treatment (1.5-1.9 million DM per year) and costs for internal services (600,000-900,000 DM per year) such as laboratory examinations (247,000-386,000 DM per year) and blood bank (100,000-215,000 DM per year), the main factors in this category. The different fields of costs and profit are compared in this analysis. Possible ways of reducing costs are pointed out, and the question of effectiveness of therapy is discussed. Conclusions from this analysis are limited by the individuality of each case with different clinical courses, the great number of influencing factors and the comparably small numbers of patients.

Lack of correlation between different hepatitis C virus screening and confirmatory assays.

Numerous 2nd and 3rd generation screening and confirmatory assays for the detection of anti-HCV antibodies have been introduced on the international market. The aim of the present study was to compare the performance of five different commercially available screening assays and four 'confirmatory' assays in a panel of serum samples that had tested positive or borderline with a 2nd generation EIA (Abbott HCV EIA 2nd generation). Considerable discrepancies were observed between the different screening assays and confirmatory tests. The antigens from the putative 'core' region of HCV were recognized most frequently by the confirmatory assays. By considering the reactivity to either NS5 (RIBA III and Inno-LIA) or E2/NS1 antigens (Inno-LIA Ab III) no sample could be identified as anti-HCV positive that would otherwise have been regarded as borderline or negative according to its banding pattern with core, NS3 and NS4 proteins. All 24 HCV-RT-PCR positive samples were anti-HCV reactive by the screening EIAs but only 18 and 21 samples were confirmed anti-HCV positive with the RIBA II and III, respectively. A clear association was observed between HCV-RNAemia in serum samples and index values (O.D. sample/O.D. cut-off) of the screening EIAs as well as with the number of reactive proteins in the confirmatory assays. In conclusion, the results of current screening and confirmatory assays are highly divergent. The additional diagnostic significance of the relatively expensive and labour-intensive immunoblots appears to be very limited. For the serological diagnosis of HCV infection and for blood donor screening, confirmatory assays should only be used if there is a borderline result by HCV EIA. The determination of infectivity by qualitative PCR and the follow-up of patients undergoing IFN therapy by HCV-RNA quantification appears to be much more useful.

Accessory scrotum or perineal collision-hamartoma. A case report to illustrate a misnomer.

A case of a congenital perineal tumor in a male infant is described, which consisted of separated fat and smooth muscle tissue in nodular arrangement. According to its clinical presentation, this lesion would be grouped into the category of so called "accessory scrotum," as part of the entity of ectopic scrotum. Based on our case and on a review of the literature, the possibility is raised that this labelling may be inappropriate on grounds of etiology and pathomorphology. It is suggested to consider similar lesions as the result of a perineal hamartomatous fat tissue overgrowth with transposed scrotal skin, which may or may not show additional hamartomatous proliferation of smooth muscle bundles, and to label them "perineal lipoma/lipoblastoma with ectopic scrotal skin" or "perineal collision-hamartoma" in the case of a prominent isolated smooth muscle component.

Dissecting processing and apoptotic activity of a cysteine protease by mutant analysis.

We have compared the behavior of wild-type mouse NEDD-2, a neural precursor cell-expressed, developmentally down-regulated cysteine protease gene, to various mutant forms of the gene in both apoptotic activity in neuronal cells and proteolytic cleavage in the Semliki Forest virus and rabbit reticulocyte protein expression systems. Our results confirm that NEDD-2 processing and apoptotic activity are linked phenomena. They identify aspartate residues as likely targets for autocatalytic cleavage. They establish that cleavage events only occur at specific sites. Finally, they pinpoint differential effects of individual mutations on the overall proteolytic cleavage patterns, raising interesting questions related to the mechanisms of subunit assembly.
