Confirmation of the multifactorial threshold model for congenital structural talipes equinovarus.

The first degree relatives of 174 children with congenital isolated structural talipes equinovarus were examined by an orthopaedic surgeon. The affected rate of first degree relatives was 27 times higher than the birth prevalence of Hungarian newborns. The heritability in the parents and sibs was 0.82 +/- 0.15. The multifactorial threshold model was proved by the so-called GAMT programme on the basis of the identity of the h2 values in the different generations of first degree relatives. A relatively lower familial clustering was found in gypsies who had a higher prevalence.

[The effect of operations on soft tissues in the development of tarsal bones in patients with clubfoot (author's transl)].

Authors investigated the process of ossification of tarsal bones in patients with club foot previously treated with redression in plaster and soft tissue release. A control group of 150 children between 1 and 12 years of age without club foot deformity was used. Based on analysis of two side X-ray pictures it is stated that 17.8 per cent of cases with club foot reveal a general backwardness and other variables of ossification. No obvious damaging effect of the operation was observed. In their opinion by means of soft tissue release the noxious effect of forced manipulation can be prevented. By ceasing the malposition in time the ossification can be accelerated.

[Transient arthritis of the hip in childhood and Perthes disease (author's transl)].

The possibilities of the relationship between transient arthritis of the hip and Perthes disease have been analysed here. They pointed to the fact, that the intra-articular pressure is increased by the overproduction of the synovial fluid as a result of inflammation, and a continuously high pressure can lead to necrosis of the femoral of inflammation of the femoral capital epiphysis. Experiences are reported here, when an intertrochanteric osteotomy at the early stage of Perthes disease was performed with simultaneous fenestration of the joint capsule for decompression, and a great amount of synovial fluid was discharged under high pressure. They conclude therefore, that transient synovitis can not be regarded only as a harmless condition, but it can precede Perthes disease.

[Results of soft tissue operations in the management of congenital club foot (author's transl)].

A review is given on the experiences of 152 cases of operatively treated club foot. Exact X-ray assessment is thought to be very important, as even after excellent morphological and functional end results residual X-ray changes have been observed which potentially may lead to the redevelopment of abnormalities. Clinical and radiological assessment together can only be informative of the result of management. Analysis of the "own" and "referred" patients showed better clinical and radiological results in the first group, which has been attributed to the early operation, regular follow-up and care.

Genetics of adolescent idiopathic scoliosis.

A genetic family study was undertaken by photofluorography of the first, second, and third degree relatives of 116 index patients with adolescent idiopathic scoliosis (AIS). The index patients were ascertained in the course of an epidemiological screening. The pattern of familial clusters and the recurrence risk related to the number of affected relatives and to the severity of the disorder in the index patients support the theory of polygenic inheritance, a multifactorial-threshold aetiological model. The recurrence risk table for first degree relative, prepared by computerised data processing and analysis, may contribute to the early diagnosis and prevention of the disorder.

[Congenital knee luxation].

The authors discuss the frequency, etiology, symptoms and classification as well as treatment of congenital dislocation of the knee. 8 own cases are reported in the article. The authors suggest that congenital dislocation of the may be well influenced by non-operative methods. Etappe redressement by means of plaster does not interfere with the functional treatment of the hip dislocation usually treated with the Pavlik braces. Associated foot deformities may be easily corrected at the same time. The so called "earliest treatment" is most important in all the three deformities (knee-hip dislocation, club foot), the authors emphasise.

[Congenital dislocations of the knee joint (author's transl)].

The authors discuss the frequency, etiology, clinical symptomatology, classification, and treatment of congenital dislocations of the knee joint. They present 8 cases of their own material. The congenital knee joint dislocation can be treated conservatively with good results. Treatment with plaster casts does not interfere with treatment of simultaneous hip dysplasias in Pavlik bandages. Even foot deformities can be treated at the same time. They emphasise the need for earliest treatment which seems equally important for all three deformities: knee joint dislocations, hip dysplasias, and club feet.

[Early surgery of the soft tissues in congenital clubfoot].

The role and position of early complete operative procedures on the soft tissues in the treatment of the congenital club foot is discussed in the article. The authors report on the operative indications in principles and in their own practice. Closed and operative treatment are not contradictory but complementary terms. The aim of the operative treatment is to bring all residual contractures following conservative treatment to an end. It must be done by relatively minimal intervention on the soft tissues and as early as possible. Thus the proper development of the foot and the prevention of bone deformation can be attained.
