[Percutaneous embolization with spirals in bleeding uterine cancer].

Percutaneous transcatheter embolization (PTE) in patients with life threatening bleeding carcinomas of the uterine cervix using stainless steel coils, proves to be an effective treating method. There is a low incidence of complications concerning the local cutis, nerves or arteries. PTE does not allow a causal therapy of the malignant disease, but in half of our patients (n = 13) embolization was the precondition for initial or further adequate therapeutic management.

In-vitro development of zygotes from superovulated prepubertal and mature gilts.

Ten prepubertal and 8 mature gilts were superovulated with PMSG and hCG, and inseminated with fresh boar semen. Zygotes were surgically recovered from oviducts 54-60 h after hCG. One and 2-cell zygotes were randomly allotted to Medium PL (modified BMOC-3 supplemented with 0.1 mM-EDTA and 1.5% BSA) or Medium G (Medium PL without pyruvate or lactate). Eggs were washed twice in medium, and placed in microdrops of medium overlaid with silicon oil for culture in an humidified 5% CO2, 5% O2, 90% N2 environment, then observed daily for 6 days. Development of eggs was dependent (P less than 0.001) on the interactive effects of age of gilt (prepubertal versus mature) and medium type (PL versus G) used in culture. A greater proportion of eggs cultured in Medium G developed further than did eggs in Medium PL (P less than 0.001). Additionally, a greater proportion of eggs from mature gilts developed further than did eggs from prepubertal gilts (P less than 0.02). We suggest that these results provide evidence that zygotes resulting from superovulation regimens of prepubertal gilts do not possess the same capacity for in-vitro development as do zygotes from pubertal gilts.

Frequent detection of HIV- and IgG-specific IgM and IgA antibodies in HIV-positive cord-blood sera: fine analysis by western blot.

Cord-blood sera of 36 babies born to HIV-positive mothers in Switzerland were tested for immunoglobulin (Ig) M or IgA by HIV Western blot. IgM was found in 28, and IgA in 19 of unabsorbed sera. Preabsorption with immobilized protein A or G was used to remove IgG, which allowed differentiation between HIV-specific and IgG-specific IgM or IgA. Protein G proved superior and showed that 30% of 23 sera had HIV-specific IgM, while 48% had HIV-specific IgA. HIV-specific IgM and/or IgA was found in 13 out of 21 cases (62%); four out of 21 (19%) had both. HIV-specific IgM reacted most frequently with pol or env proteins, while HIV-specific IgA reacted more frequently with gag than pol; no IgA were directed against env proteins. IgG-specific IgM and IgA, mostly at gag bands, were present in 83 and 38%, respectively. Thus, a large percentage of children born to HIV-positive mothers have HIV-specific IgA and/or IgM which can be distinguished from IgG-specific IgA or IgM, which is also present in the majority of such children. Future studies will have to show whether these antibodies are of diagnostic relevance.

Prevention of postoperative deep vein thrombosis by one daily injection of low molecular weight heparin and dihydroergotamine.

201 patients aged over 40 years undergoing abdominal surgery were divided randomly into two groups. Each patient received two subcutaneous injections daily: the first group received a morning injection of 1500 aPTT U of low molecular weight heparin combined with 0.5 mg dihydroergotamine (LMH/DHE) and an evening injection of placebo; the second group received morning and evening injections of 2500 IU standard heparin combined with 0.5 mg dihydroergotamine(H/DHE). 25 patients were withdrawn during the course of the trial, 13 in the LMH/DHE group and 12 in the H/DHE group. There was no significant difference between the two groups with regard to age, sex, body weight or history of thromboembolism. 125I-labelled fibrinogen test was routinely used to detect deep vein thrombosis (DVT), which was confirmed by phlebography. Ventilation-perfusion scanning was performed in patients in whom pulmonary embolism was suspected on clinical grounds. DVT occurred in 6 patients in the LMH/DHE group (6.9%) and in 7 patients in the H/DHE group (7.9%). Pulmonary embolism occurred in one patient in each group. The only noteworthy haemorrhagic incident was a haematoma of the abdominal wall in one patient (LMH/DHE). It was concluded that a single daily injection of 1500 aPTT U low molecular weight heparin combined with DHE is as effective and as well tolerated as two injections daily of 2500 IU standard heparin combined with DHE.

HTLV-1 in Switzerland: low prevalence of specific antibodies in HIV risk groups, high prevalence of cross-reactive antibodies in normal blood donors.

Sera from various Swiss population groups were tested for antibodies against the human T-cell leukemia virus type I (HTLV-I). Particle agglutination and ELISA were performed for screening; Western blot was done for confirmation. True-positive sera were found at a prevalence of 0.12% in a cohort of 846 individuals at risk for AIDS tested in 1984-1985. Prevalences of 0.35% were found among 575 HIV-I positives tested in 1987, and of 1.3% among 292 HIV-I positives of a different group tested in 1988. The 6 positives found in our study represent the first cases of HTLV-I infection, or HIV-I/HTLV-I double infection, diagnosed in Switzerland. In addition, high proportions of sera, regardless of whether they were from normal blood donors, HIV-positives, or individuals at risk for AIDS, had antibodies that reacted weakly with one or several proteins of the size of viral gag proteins. The prevalence of such antibodies in normal donors was in the range of 10 to 40%, depending on the strictness of interpretation. Competition Western blots performed with some of these sera showed that these antibodies reacted with HTLV-I, but not with HIV-I or cellular antigens, and had a lower affinity to HTLV-I proteins than the antibodies of human or goat antisera. The results indicate that these antibodies may be induced by agents immunologically related to, but different from, HTLV-I, which are highly prevalent in the Swiss population. Oligopeptide stretches with sequence homology to HTLV-I are known to exist in various normal body proteins, several infectious agents including common viruses and protozoa, but the results might also indicate the existence of additional human retroviruses. Screening of blood donors with sensitive tests for antibodies to HTLV-I might produce an unacceptably high rate of false-positive results, if stringent rules of interpretation analogous to those common in HIV screening are not used.

Prolactin in patients with major depressive disorder and in healthy subjects. I. Cross-sectional study of basal and post-TRH and postdexamethasone prolactin levels.

Prolactin (PRL) levels were investigated in patients with major depressive illness and in healthy subjects. Basal and postdexamethasone levels were measured in 27 patients, and levels after thyrotropin-releasing hormone (TRH) stimulation (delta PRL) were measured in 22 patients. Basal and delta PRL were also determined in 64 age- and sex-matched healthy subjects. Both basal and postdexamethasone PRL levels were normal in depressed patients, with the postdexamethasone levels in particular showing no correlation to postdexamethasone cortisol concentrations. One milligram oral dexamethasone did not influence 4:00 PM PRL levels in 15 healthy subjects. delta PRL was significantly elevated in both male and female patients. These increases were not correlated with severity of illness and are difficult to interpret owing to the complexity of the PRL regulatory system. No significant correlations were found between basal or post-TRH PRL and cortisol, thyroid-stimulating hormone (TSH), thyroid hormones, gonadotropins, or estradiol in the patients. However, surprisingly significant positive correlations between basal PRL and basal cortisol, T4 and reverse T3 occurred in healthy subjects. It is as yet unclear how this finding can be explained and what relevance it has. Women tended to have higher basal PRL concentrations than men, but the difference was not significant in either group. delta PRL was significantly higher in women than in men in both patients and controls. No significant influence of age was found.

Prolactin in patients with major depressive disorder and in healthy subjects. II. Longitudinal study of basal prolactin and post-TRH-stimulated prolactin levels.

Longitudinal investigations of basal prolactin (PRL) and prolactin concentrations following thyrotopin-releasing hormone (TRH) stimulation (delta PRL) were conducted in 17 patients with major depressive disorder and healthy subjects. The patients were being treated with either clomipramine or maprotiline. Both basal and delta PRL increased significantly after clinical response during treatment with both drugs. However, these increases in basal and delta PRL were independent of each other. Surprisingly, elevations of basal PRL were significantly greater in responders than in nonresponders, whereas those in delta PRL showed no corresponding significant difference. These results suggest that the two drugs stimulate basal and delta PRL by different mechanisms. The increases in basal prolactin levels found in responders may possibly be due to weaker inhibition of prolactin due to "down-regulated" beta adrenergic receptors and/or enhanced activity of supersensitive serotonergic receptors. Neither basal PRL nor delta PRL proved to be a predictor of therapy response. The intraindividual retest reliabilities of both basal and delta PRL in healthy subjects was so good that a single blood sample would seem to be sufficient for investigating most issues involving PRL in psychiatric patients.

Prolactin in patients with major depressive disorder and in healthy subjects. III. Investigation of basal and post-TRH prolactin during different forms of acute and chronic psychological stress.

Prolactin (PRL) levels after thyrotropin-releasing hormone (TRH) (delta PRL) were determined 4 times and basal prolactin levels 6 times in 10 healthy medical students before, during, and after a major examination in medicine. No significant differences in basal or delta PRL levels occurred during the examination period. Basal PRL was also measured in 14 doctors of medicine after delivering a paper at a clinical conference and a further 9 doctors both before and after delivering a paper. PRL was measured serially at 20-min intervals in 4 doctors on the day on which they presented the paper. No significant differences in PRL levels were found in any of the tests conducted during this kind of stress as compared with the corresponding values obtained under nonstressful conditions. Increases in PRL before delivering the paper were seen in 3 subjects, but such increases also occurred completely independently of stress. An 18-hr fast did not influence PRL secretion in 11 healthy volunteers. Both the information obtained from a review of the literature on the influence of stress on PRL secretion and our own results strongly suggest that contrary to common opinion, there is no evidence at all that psychological stress affects PRL secretion in man.

Bacteriophages of lactobacilli.

Lactobacilli are members of the bacterial flora of lactic starter cultures used to generate lactic acid fermentation in a number of animal or plant products used as human or animals foods. They can be affected by phage outbreaks, which can result in faulty and depreciated products. Two groups of phages specific of Lactobacillus casei have been thoroughly studied. 1. The first group is represented by phage PL-1. This phage behaves as lytic in its usual host L. casei ATCC 27092, but can lysogenize another strain, L. casei ATCC 334. Bacterial receptors of this phage are located in a cell-wall polysaccharide and rhamnose is the main component of the receptors. Ca2+ and adenosine triphosphate (ATP) are indispensable to ensure the injection of the phage DNA into the bacterial cell. The phage DNA is double-stranded, mostly linear, but with cohesive ends which enables it to be circularized. The vegetative growth of PL-1 proceeds according to the classical mode. Cell lysis is produced by an N-acetyl-muramidase at the end of vegetative growth. 2. The second group is represented by the temperate phage phi FSW of L. casei ATCC27139. It has been shown how virulent phages originate from this temperate phage in Japanese dairy plants. The lysogenic state of phi FSW can be altered either by point mutations or by the insertion of a mobile genetic element called ISL 1, which comes from the bacterial chromosome. This is the first transposable element that has been described in lactobacilli. Lysogeny appears to be widespread among lactobacilli since one study showed that 27% of 148 strains studied, representing 15 species, produced phage particles after induction by mitomycin C. Similarly, 23 out of 30 strains of Lactobacillus salivarius are lysogenic and produce, after induction by mitomycin C, temperate phages, killer particles, or defective phages. Temperate phages have also been found in 10 out of 105 strains of Lactobacillus bulgaricus or Lactobacillus lactis after induction by mitomycin C. Phages so far studied of the latter 2 and closely related lactobacilli, either temperate or isolated as lytic, may be divided into 4 unrelated groups called a, b, c and d. Most of these phages are found in group a and an unquestionable relationship has already been shown between lytic phages and temperate phages that belong to this group. Lytic phage LL-H of L. lactis LL 23, isolated in Finland, is one of the most representative of those of group a and has been extensively studied on the molecular level.

Influence of prostaglandins, omeprazole, and indomethacin on healing of experimental gastric ulcers in the rat.

We investigated whether the trophic actions of prostaglandins, omeprazole, and indomethacin on gastric mucosa lead to accelerated healing of gastric ulcers in the rat. Cryoulcers were produced in the corpus area and treated with 16,16-dimethyl prostaglandin E2 (5 or 100 micrograms/kg b.i.d., intragastrically), omeprazole (40 mumol/kg once daily, subcutaneously), indomethacin (2 mg/kg b.i.d., subcutaneously), or placebo. At the end of the treatment, plasma gastrin, cell labeling index (autoradiography with [3H]thymidine), and the size and depth of mucosal defects were measured. Compared with placebo, omeprazole accelerated ulcer healing as indicated by a smaller ulcer area [1.1 +/- 0.2 vs. 4.8 +/- 1.2 mm2 (mean +/- SEM)] and smaller ulcer depth (383 +/- 31 vs. 488 +/- 41 microns) after 10 days of treatment. Prostaglandins did not affect ulcer healing despite thickening of gastric corpus mucosa. Indomethacin delayed ulcer healing and reduced the labeling index. Omeprazole induced a marked hypergastrinemia (208 +/- 12 vs. 66 +/- 12 pmol/L on day 5, and 469 +/- 23 vs. 58 +/- 16 pmol/L on day 10). The results indicate that abolishment of acid secretion by omeprazole accelerates healing. Trophic actions and "cytoprotective" effects by prostaglandins are not relevant for ulcer healing in this model.

[Classification of Gonococcus: auxanologic and serologic aspects using monoclonal antibodies].

500 N. gonorrhoeae strains from the area of Lübeck/West Germany, collected in 1976-84, were auxologically and serologically classified by means of monoclonal antibodies from the Genetic Systems Corporation, Seattle, Washington (USA), against gonococcal outer membrane protein I. During the first years, auxological classification showed high proportions of AHU-strains (40%), which amounted to 20% over the total observation period. PPNG and DGI strains have characteristic type patterns. Our synopsis of serovars corresponds with the results obtained in a recent world-wide study. Combined auxological and serological classification shows two N. gonorrhoeae pools, 18% each, which cannot be further distinguished: AHU/A1 and prototrophic/B3. The advantages of the system have been proved in answering plain epidemiological questions. For further discrimination, an additional independent classification method might be useful.

Serial dexamethasone suppression tests in psychiatric illness: Part I. A study in schizophrenia and mania.

Weekly dexamethasone suppression tests (DSTs) were performed in 15 patients with schizophrenia (n = 12) and mania (n = 3) until clinical response. At initial evaluation, 53.4% of the patients were nonsuppressors, and 93.3% showed nonsuppression at least once during the treatment period. There was a tendency for DST results to normalize coincident with clinical improvement, although single peaks of DST nonsuppression occurred in several patients irrespective of clinical course. The tests did not prove useful as predictors of recovery or relapse. DST nonsuppression occurred significantly more often in severely ill patients than in moderately ill patients or in patients after recovery, emphasizing the effects of nonspecific stress factors and/or severity of illness on the DST. The cutoff point, established on the basis of DST results in 67 healthy controls, was lower than in other studies, and nonsuppression among healthy controls was associated with low dexamethasone serum levels.
