Biodegradation kinetics of high strength oily pet food wastewater in a membrane-coupled bioreactor (MBR).

Aerobic respirometric experiments were conducted at a temperature of 40 degrees C to determine biokinetic parameters describing growth and decay of aerobic biomass acclimatized in a membrane-coupled bioreactor (MBR). The kinetic parameters were determined with the volatile fatty acids (VFA) acetic acid and propionic acid as substrates as well as the soluble fraction of a high strength rendering wastewater as substrate. The oxygen uptake rate (OUR) and cumulative oxygen uptake (OU) recorded during the experiments were fitted to Monod kinetic model to obtain true yield (Y(t)), intrinsic maximum substrate removal (k), half-saturation constant (K(s)) and endogenous decay coefficient (k(d)). K(s) were determined to be 181, 271 and 806 mg COD l(-1) and k as 1.89, 1.08 and 0.85 mg COD mg VSS(-1)h(-1) for acetate, propionate and rendering wastewater, respectively. The continuous-flow MBR was operated under two HRT conditions, 10 and 5 d, attaining high BOD removal efficiencies of 99% and 87%, respectively. The observed yield in the MBR was 0.03 g VSS g(-1)COD.

In vivo study on the healing of bone defects treated with bone marrow stromal cells, platelet-rich plasma, and freeze-dried bone allografts, alone and in combination.

The repair of confined trabecular bone defects in rabbits treated by autologous bone marrow stromal cells (BMSC), platelet-rich plasma (PRP), freeze-dried bone allografts (FDBA) alone and in combination (BMSC + PRP; FDBA + BMSC; FDBA + PRP; FDBA + PRP + BMSC) was compared. A critical size defect was created in the distal part of the femurs of 48 adult rabbits. Histology and histomorphometry were used in the evaluation of healing at 2, 4, and 12 weeks after surgery. The healing rate (%) was calculated by measuring the residual bone defect area. Architecture of the newly formed bone was compared with that of bone at the same distal femur area of healthy rabbits. The defect healing rate was higher in PRP + BMSC, FDBA + PRP, FDBA + BMSC, and FDBA + PRP + BMSC treatments, while lower values were achieved with PRP treatment at all experimental times. The highest bone-healing rate at 2 weeks was achieved with FDBA + PRP + BMSC treatment, which resulted significantly different from PRP (p < 0.05) and BMSC (p < 0.05) treatments. At 4 weeks, the bone-healing rate increased except for PRP treatment. Finally, the bone-healing rate of FDBA + PRP, FDBA + BMSC, and FDBA + PRP + BMSC was significantly higher than that of PRP at 12 weeks (p < 0.05). At 12 weeks, significant differences still existed between PRP, BMSC, and FDBA groups and normal bone (p < 0.05). These results showed that the combination of FDBA, BMSC and PRP permitted an acceleration in bone healing and bone remodeling processes.

Knowledge, attitude and practice of natural family planning methods in a population with poor utilisation of modern contraceptives.

Sub-Saharan Africa has one of the highest fertility rates in the world, which is further promoted by the low utilisation of modern contraceptive methods. Yet, many communities claim to have traditional methods of family planning that pre-date the introduction of modern contraceptives, implying that contraception is a culturally acceptable norm. It was therefore postulated that the study population would have a high level of awareness and practice of natural methods of family planning. We aimed to obtain an insight into the extent and correctness of knowledge about natural family planning methods, and its practice as a guide to the general acceptance of contraception as a concept. Pre-tested structured questionnaires were administered to women of childbearing age in households properly numbered for primary healthcare activities. The level of awareness of natural family planning methods was significantly less than awareness for modern methods of contraception. The awareness rate for rhythm method, lactational amenorrhoea method and coitus interruptus was 50.7%, 42.1% and 36.1%, respectively. For all three national family planning methods, there is a steady decline between awareness, correct description of method and utilisation, a difference that was statistically significant in all cases. The sociodemographic factors of the responders had varying influence on utilisation of all three natural family planning methods studied. Rural dwellers practised the lactational amenorrhoea method significantly more often than urban dwellers. Significantly more Muslims than Christians with four children or more practised coitus interruptus or the rhythm method, while the use of lactational amenorrhoea method was significantly increased with the number of living children in both religious groups. There is a relatively low level of awareness of natural family planning methods in the study population, poor utilisation and wrong use of methods. Therefore, improving the correct level of information on natural family planning methods is likely to improve the use of both natural family planning and modern contraceptive methods.

Practice pattern of hemodialysis vascular access in Maiduguri, Nigeria.

BACKGROUND: Despite available evidence based recommendations and guidelines there are differences in the preferred type of vascular access in use in different localities and geographical regions. This one center study on the practice pattern of vascular access was carried out in Maiduguri, Nigeria covering a period of the first 5 years of renal care in the area. METHOD: We analysed the dialysis and hospital records of consecutive patients that received hemodialysis in the renal center during the period between January 2000 and December 2004. Data comprised patients' demographics, vascular access used, complications encountered during the treatment period. Clinical and laboratory features of the patients were also recorded and analysed. RESULTS: The study involved one hundred and seventy-nine patients (M:F=2:1) who suffered kidney failure whose cause was clinically determined to be mainly due to chronic glomerulonephritis, hypertension, obstructive nephropathy and interstitial nephritis. Majority presented as late referrals to the nephrologist. Overall 9% of the study population used AVFs while 91% used catheters inserted into the femoral or internal jugular veins. CONCLUSION: The use of catheters for vascular access was the practice in the majority of the patients and this is possibly related to the poor survival of the patients on hemodialysis. Socioeconomic circumstances of the patients in turn largely influenced the rate of AVF use.

Treatment of high-strength pet food wastewater using two-stage membrane bioreactors.

A two-stage membrane bioreactor was used to treat dissolved-air-flotation pretreated, high-strength pet food wastewater characterized by oil and grease concentrations of 50 000 to 82 000 mg/L and total chemical oxygen demand (COD) and five-day biochemical oxygen demand (BOD5) concentrations of 100 000 and 80 000 mg/L, respectively, to meet stringent surface discharge criteria (i.e., BOD5, total suspended solids [TSS], and ammonium-nitrogen [NH4(+)-N] of < 10 mg/L at an overall hydraulic retention time of 6.3 days). Organic contaminants were removed primarily in the first stage, followed by almost complete removal of ammonia in the second stage. Despite a rise in poorly biodegradable COD in the second stage, overall removal of TSS, BOD5, COD, and ammonia was 100, 99.9, 95.2, and 99.7%, respectively, thus readily achieving the required criteria. Consistent nitrite accumulation over a period of more than 100 days, even at dissolved oxygen concentrations of more than 2.5 mg/L, was remarkable. A residual alkalinity requirement for nitrification was quantified. Membrane performance was extensively studied in this work.

Conventional and thermophilic aerobic treatability of high strength oily pet food wastewater using membrane-coupled bioreactors.

Although thermophilic treatment systems have recently gained considerable interest, limited information exists on the comparative performances of membrane-coupled bioreactors (MBR) at thermophilic and conventional conditions. In this study aerobic MBRs operating at room temperature (20 degrees C) and at lower thermophilic range (45 degrees C) were investigated for the treatment of dissolved air flotation (DAF) pretreated pet food wastewater. The particular wastewater is characterized by oil and grease (O & G) concentrations as high as 6 g/L, COD of 51 g/L, BOD of 16 g/L and volatile fatty acid (VFA) of 8.3 g/L. The performances of the two systems in terms of COD, BOD and O & G removal at varying hydraulic retention time (HRT) are compared. COD removal efficiencies in the thermophilic MBR varied from 75% to 98% and remained constant at 94% in the conventional MBR. The O & G removal efficiencies were 66-86% and 98% in the thermophilic and conventional MBR, respectively. Interestingly, high concentrations of VFA were recorded, equivalent to 50-73% of total COD, in the thermophilic MBR effluent. The observed yield in the thermophilic MBR was 40% of that observed in the conventional MBR.

A novel two-stage MBR denitrification process for the treatment of high strength pet food wastewater.

A novel paradigm using pre-denitrification process is presented to optimize an existing system of two-stage MBRs treating high strength pet food wastewater. Successive reduction of organics in the 1st stage and almost complete nitrification in the 2nd stage generated effluent meeting stringent surface discharge criteria i.e. BOD5, TSS and NH4+ -N of < 10 mg/L at an overall HRT of 6.3 days. Pre-anoxic zone was created by a submerged coil in the path of influent to the 1st stage. Final effluent and the 1st stage mixed liquor were recirculated to the coil. With prevailing high denitrification rates, more than 94% of the recirculated nitrates were denitrified in less than 15 min of effective anoxic residence time. At a recycle ratio of 3:1, total nitrogen was reduced by 84%, aeration energy by 25% and the external alkalinity requirement by 65%, enhancing economical viability of the system.

Kinetic modeling of aerobic biodegradation of high oil and grease rendering wastewater.

Batch scale activated sludge kinetic studies were undertaken for the treatment of pet food wastewater characterized by oil and grease concentrations of up to 21,500 mg/L, COD and BOD concentrations of 75,000 and 60,000 mg/L, respectively as well as effluent from the batch dissolved air flotation (DAF) system. The conducted kinetics studies showed that Haldane Model fit the substrates and biomass data better than Monod model in DAF-pretreated wastewater, while the modified hydrolysis Monod model better fit the raw wastewater kinetic data. For the DAF pretreated batches, Haldane Model kinetic coefficients k, K(S), Y and Ki values of 1.28-5.35 g COD/g VSS-d, 17,833-23,477 mg/L, 0.13-0.41 mg VSS/mg COD and 48,168 mg/L, respectively were obtained reflecting the slow biodegradation rate. Modified hydrolysis Monod model kinetic constants for the raw wastewater i.e., k, K(S), Y, and K(H) varied from 1-1.3 g COD/g VSS-d, 5580-5600 mg COD/l, 0.08-0.85 mg VSS/mg COD, and 0.21-0.66 d(-1), respectively.

Predictive value of alarm features in a rapid access upper gastrointestinal cancer service.

AIMS: (i) To determine the value of individual alarm features for predicting cancer in subjects referred to a rapid access upper gastrointestinal cancer service; and (ii) to develop a clinical prediction model for cancer and to prospectively validate this model in a further patient cohort. METHODS: Patient demographics, referral indications, and subsequent diagnosis were recorded prospectively. Logistic regression analyses were employed to determine the predictive value of individual alarm features in an evaluation cohort of 1852 consecutive cases. The potential impact of applying a modified set of referral criteria was then examined in a validation cohort of 1785 patients. RESULTS: Evaluation cohort: mean age was 59 years; cancer prevalence 3.8%; and serious benign pathology 12.8%. Dysphagia (odds ratio (OR) 3.1), weight loss (OR 2.6), and age >55 years (OR 9.5) were found to be significant predictive factors for cancer but the value of other accepted alarm features was more limited. In particular, uncomplicated dyspepsia in those over 55 years was a negative predictive factor for cancer within this high risk cohort (OR 0.1). Validation cohort: the clinical prediction model would have selected 92% of cancer patients for fast track investigation while reducing the "two week rule" workload by 572 cases (31%). CONCLUSIONS: Fast track endoscopy in subjects fulfilling current criteria for suspected upper gastrointestinal malignancy results in a significant yield of cancer ( approximately 4%) and serious benign diseases such as peptic ulceration, strictures, and severe oesophagitis (13%). However, the predictive value of individual features for cancer varies widely. Uncomplicated dyspepsia in older subjects was a poor predictor of cancer. Application of narrower referral criteria for accessing fast track services may reduce pressures while retaining high sensitivity for cancer.

Treatability and kinetics studies of mesophilic aerobic biodegradation of high oil and grease pet food wastewater.

In this work, batch activated sludge studies were investigated for the treatment of raw pet food wastewater characterized by oil and grease concentrations of 50,000-66,000 mg/L, COD and BOD concentrations of 100,000 and 80,000 mg/L, respectively, as well as effluent from an existing anaerobic digester treating the aforementioned wastewater. A pre-treatment process, dissolved air flotation (DAF) achieved 97-99% reduction in O&G to about 400-800 mg/L, which is still atypically high for AS. The batch studies were conducted using a 4-L bioreactor at room temperature (21 degrees C) under different conditions. The experimental results showed for the DAF pretreated effluent, 92% COD removal efficiency can be achieved by using conventional activated sludge system at a 5 days contact time and applied initial soluble COD to biomass ratio of 1.17 mg COD/mg VSS. Similarly for the digester effluent at average oil and grease concentrations of 13,500 mg/L, activated sludge affected 63.7-76.2% soluble COD removal at 5 days. The results also showed that all kinetic data best conformed to the zero order biodegradation model with a low biomass specific maximum substrate utilization rate of 0.168 mg COD/mg VSS day reflecting the slow biodegradability of the wastewater even after 99% removal of oil and grease.

Temporal administration of entacapone with slow release L-dopa: pharmacokinetic profile and clinical outcome.

Entacapone is a specific, peripherally acting catechol- O-methyltransferase (COMT) inhibitor that prevents peripheral degradation of L-dopa, thus improving its bioavailability. Entacapone is known to have pharmacokinetics similar to standard L-dopa but not to that of controlled-release (CR) L-dopa. The aim was to determine whether delayed entacapone administration may prolong CR L-dopa half-life in comparison to the co-administration modality. We compared plasma L-dopa concentrations after co-administration of CR L-dopa and entacapone or after administration of CR and a delayed (30 and 90 minutes) entacapone dose in 10 parkinsonian patients. The area under the concentration-time curve and other pharmacokinetic parameters were not changed by the delayed administration of entacapone. Different temporal modalities of entacapone administration had similar effects on CR L-dopa pharmacokinetics and on L-dopa-induced clinical improvement.

The treatability study of high strength pet food wastewater: a continuous flow aerobic system performance evaluation.

This paper presents continuous flow two stage activated sludge treatability studies for the treatment of pet food wastewaters characterized by oil & grease concentration of 50,000-66,000 mg l(-1), total COD & BOD concentrations of 100,000 mg l(-1) and 80,000 mg l(-1), respectively. A pre-treatment system, dissolved air flotation (DAF) achieved 97-99% reduction in oil and grease down to about 400-800 mg l(-1). The pilot experimental results showed that for the DAF pretreated effluent, 97% soluble COD removal efficiency can be achieved by using two stage conventional activated sludge system at a 6-7 days HRT and applied initial soluble COD to biomass ratio of 1.17 mg COD mg VSS. Meanwhile, the system achieved 99.9% soluble BOD removal. Analysis of system kinetics indicated that this particular wastewater is very difficult to biodegrade, with an extremely high half velocity constant "Ks" of 5800 mg l(-1). The research showed the potential applications of activated sludge systems for the treatment of high oil and grease wastewater.

Delayed administration may improve entacapone effects in parkinsonian patients non-responding to the drug.

BACKGROUND: Entacapone is a COMT inhibitor used in Parkinson's disease (PD) patients, as an adjunctive therapy to L-dopa in order to prolong its bioavailability and thus its clinical effect. However, previous studies reported entacapone-induced L-dopa to have lower C(max) and delayed t(max) values, coupled with a delayed onset of the clinical effect, possibly suggesting an interference between the two drugs. The aim of our study was to evaluate whether a delayed entacapone administration in association with standard L-dopa/carbidopa, may in some subjects improve the entacapone effects on L-dopa AUC and thus on the clinical 'on time' duration. METHODS: Twenty-eight idiopathic advanced PD patients were blindly evaluated in three different test days, following administration of carbidopa/L-dopa or carbidopa/L-dopa plus co-administered entacapone or plus entacapone administered with 30 min of delay. RESULTS: The AUC, the 'on time' and UPDRS score of the whole group were improved by both modalities of entacapone administration. An ex post analysis showed that the delayed entacapone administration produced a significant improvement in a subgroup of 10 non-responding patients to the co-administration. CONCLUSION: We suggest that the delayed administration should be attempted in the subjects not improved by entacapone co-administration.

Cost of illness of inflammatory bowel disease in the UK: a single centre retrospective study.

BACKGROUND AND AIMS: The potentially high costs of care associated with inflammatory bowel disease (IBD) are recognised but we have little knowledge of the scale, profile, or determinants of these costs in the UK. This study aimed to describe costs of illness for a group of IBD patients and determine factors associated with increased healthcare costs. SETTING: A university hospital serving a target population of approximately 330 000. PATIENTS AND METHODS: A six month cohort of IBD patients receiving any form of secondary care was identified, comprising 307 cases of ulcerative (or indeterminate) colitis and 172 cases of Crohn's disease. Demographic and clinical data were abstracted from clinical records and individual resource use was itemised for all attributable costs (including extraintestinal manifestations). Item costs were derived from national and local sources. Cost data were expressed as mean six month costs per patient (with 95% confidence interval (CI)) obtained using non-parametric bootstrapping. Determinants of cost were analysed using generalised linear regression modelling. A postal survey of patients was undertaken to examine indirect costs, out of pocket expenses, and primary care visits. RESULTS: Inpatient services (medical and/or surgical) were required by 67 patients (14%) but accounted for 49% of total secondary care costs. Drug costs accounted for less than a quarter of total costs. Individual patient costs ranged from 73 to 33,254 UK pounds per six months. Mean (95% CI) six month costs per patient were 1256 UK pounds ( 988 pounds, 1721 pounds) for colitis and 1652 UK pounds (1221 pounds, 2239 pounds) for Crohn's disease. Hospitalisation, disease severity grade, and disease extent correlated positively with cost of illness but costs were independent of age or sex. Compared with quiescent cases of IBD, disease relapse was associated with a 2-3-fold increase in costs for non-hospitalised cases and a 20-fold increase in costs for hospitalised cases. Survey data suggested average six month costs were < 30 UK pounds per patient for primary care visits (both diseases) and median loss of earnings were 239 UK pounds for colitis and 299 UK pounds for Crohn's disease. CONCLUSIONS: This study represents the first detailed characterisation of the scale and determinants of costs of illness for IBD in a British hospital. Hospitalisation affected a minority of sufferers but accounted for half of the total direct costs falling on the healthcare system.

Pramipexole in comparison to l-dopa: a neuropsychological study.

Twenty right-handed patients affected by early/mild Parkinson's disease were evaluated in a randomised study using neuropsychological and clinical assessements during three treatment modalities: when in the off treatment condition, when on pramipexole, and when on l-dopa. In comparison to the off treatment condition, the DA-agonist pramipexole produced a significant impairment of short term verbal memory, attentional-executive functions and verbal fluency, while l-dopa did not. Moreover, pramipexole opposite to l-dopa, failed to improve FAS and Stroop tests. Present findings indicate that pramipexole may worsen cognitive functions although not exceeding normative values.

Decreased plasma and cerebrospinal fluid content of neuroactive steroids in Parkinson's disease.

The levels of the neuroactive steroids allopregnanolone (THP) and 5alpha-dihydroprogesterone (DHP) were quantified in the plasma of 11 (group 1) and in the liquor of 12 (group 2) Parkinson's disease (PD) patients using a gas-chromatographic/mass-spectrometric method. When compared with controls, both groups showed a significant decrease in DHP and THP concentrations. These decreases could be a useful marker of PD. Moreover, in view of the importance of GABA-ergic transmission to substantia nigra (SN) neurons and GABA-ergic modulation exerted by the two neuroactive steroids, our data indicate a global dysregulation of the SN GABA-ergic system in PD patients. Moreover, a lack of neuroprotective factors (i. e., GDNF, BDNF), promoted by DHP, may contribute to dopaminergic cell death.

Carcinoid tumour of the terminal ileum simulating Crohn disease.

Carcinoid tumours are often difficult to diagnose because of obscure or non-specific symptoms. Two cases of ileal carcinoid are reported in whom the diagnosis was delayed as the symptoms and small-bowel series were thought to be consistent with Crohn disease. This report emphasizes the difficulties of diagnosing carcinoid by conventional radiological methods. Ileal carcinoid should be considered in the differential diagnosis of Crohn disease, particularly in elderly patients presenting with chronic recurrent symptoms.
