[Non-Hodgkin's lymphoma, renal carcinoma, and meningioma. A clinical case].

Some second malignant neoplasms are increasingly being observed following NHL and a considerable amount of data has accumulated in the literature. The authors describe a case of 65-year old male who presented with submandibular adenopathy. Results of a biopsy of the mass surgically removed revealed low grade non-Hodgkin lymphoma. During the staging workup, a meningioma and a renal cell adenocarcinoma (RCC) were unexpectedly discovered and successively resected. The patient is currently alive with no evidence of metastatic diseases 12 months after diagnosis of non-Hodgkin's lymphoma (NHL), 10 months after meningioma resection and 8 months after RCC resection. The possibility of an underlying pathologic mechanism predisposing to multiple tumours should be considered. RCC and central nervous system (CNS) neoplasms are among second malignancies with higher incidences in non-Hodgkin lymphoma patients whereas with specific regard to meningioma, one of the most common benign intracranial tumours that sometimes shows biological aggressiveness and malignancy, we have currently no data in the literature. Increased risks for several malignancies occur late in the NHL follow-up period and are largely confined to patients receiving either radiation therapy or chemotherapy. On the other hand, increased risks for renal cancer have also been reported at less than one year after diagnosis of NHL and are present in all treatment subgroups (radiation therapy, chemotherapy, other-no treatment). Increased risks for CNS malignant neoplasms have also been reported at less than one year. The authors review the pathogenic significance of this case report neoplasms association in the light of the various explicative hypothesis of this concurrence. Possible immune mechanisms associated with these neoplasm are particularly pointed up.

[An epidemiological study of the health conditions of Milan traffic police with respect to pollution from vehicular traffic].

An investigation on the health effects of occupational exposure to motor vehicle exhaust and environmental pollutants was carried out on traffic wardens in Milan (Italy). Randomized samples of 292 traffic wardens (exposed group) and 60 hospital staff members (control group) underwent a physical examination and laboratory tests. No significant difference was observed between the exposed and control groups as regards general morbidity, apart from musculo-skeletal disorders in females. The mean blood lead level (PbB) among traffic wardens was 15.2 micrograms/dl compared with 11.7 in control (p less than 0.01). The carboxyhaemoglobin concentration (COHb) in traffic wardens at the beginning of the shift was 2.8% for smokers and 1.2% for non-smokers (3.0% and 0.9% respectively in controls). At the end of the shift COHb in the exposed group was 4.3% for smokers and 2.5% for non-smokers (p less than 0.01). PbB was significantly correlated (r = 0.17) with Median Nerve Motor Conduction Velocity (NCV) in the exposed but not in the control group. The same pattern was observed for the correlation of PbB and Systolic Blood Pressure (SBP) (r = -0.24). COHb was significantly correlated with HDL cholesterol (r = -0.20) in the exposed group only. It is questionable whether very low PbB levels can affect NCV and SBP directly or rather whether PbB, as well as COHb, should be regarded as tracers of exposure to those urban pollutants leading also to cardiovascular and nervous disorders.

[Restandardization of thyroid uptake indices in patients with euthyroidism using 131I].

The authors studied 454 patients in order to review the thyroid uptake pattern in São Paulo. They were submitted to clinical examination and hormone dosages (T3, T4 and TSH). Two hundred and fifty-four patients (55.9%) were excluded because they had hormonal dosages out of the normal range of substernal, diffuse and multinodular goiter, or were younger than 12. The 200 remaining patients (44.1%) were included in the euthyroidism group and the results were as follows: 2 hours thyroid uptake = 3 to 12%; 24 hour thyroid uptake = 8 to 32%.

Study of the negative inotropic action of somatostatin.

Somatostatin was evaluated as a chronotropic and inotropic agent in isolated, spontaneously beating guinea-pig atria. Somatostatin (3 x 10(-5)-1 x 10(-3) mM) caused a concentration-dependent negative inotropic effect and a small reduction of atrial rate. The negative inotropic effect of somatostatin was evaluated at different concentrations of extracellular calcium, on the positive inotropic response induced by calcium and on the positive chronotropic response induced by adrenaline. Small, nonsignificant changes in atria response to somatostatin were found both with the lowest (1.25 mM) and with the highest (5 mM) calcium concentration in the medium. Somatostatin (1 x 10(-4)-1 x 10(-3) mm) did not substantially modify the positive inotropic action of calcium; this substance (1 x 10(-3 mM) caused a parallel shift to the right of the adrenaline concentration-response curve. These results suggest that the negative inotropic and chronotropic actions of somatostatin do not appear to be due to an action on the voltage-sensitive calcium channels, but rather to an action on the calcium channels excited by the stimulation of beta-receptors, whose function is probably cooperative with that of the voltage-sensitive channels.

[Alternating chemotherapy and daily multiple-fraction radiotherapy in the treatment of advanced spinocellular carcinoma of the head and neck].

From January 1987 to May 1988 sixteen pts with advanced squamous cell carcinoma of the head and neck received a combined treatment based on an alternation of chemotherapy and MFD--radiotherapy. The chemotherapy regimen consisted of CDDP 20 mg/m2 and 5-FU 200 mg/m2 i.v. push, from day 1 to day 5 during weeks 1, 5 and 9. Radiotherapy was administered in two courses of 32 Gy each (total dose 64 Gy), during weeks 2-3 and 6-7. Each course was given in 2 fractions per day, five days per week. All sixteen patients could be evaluated for toxicity and fifteen for response. The following were observed: 7 CR, 6 PR, 1 SD and 1 PD. The overall response rate was 86.6%. Toxicity was high; 43.7% of the patients developed grade III-IV mucositis. These results suggest that CDDP and 5-FU alternating with MFD-radiotherapy is feasible, although new, less toxic scheduling must be determined.
