Plasma and buffy coat concentration of 8-methoxypsoralen in patients treated with extracorporeal photopheresis.

Recently, a new therapy involving an extracorporal activation of orally administered 8-methoxypsoralen (8-MOP), photosensitizing furocoumarin, is established for the treatment of different skin diseases, extracorporeal photopheresis (ECP). The pharmacokinetic profile of 8-MOP has been pursued as part of a clinical study which should assess the efficacy of ECP in patients with progressive systemic sclerosis and cutaneous T-cell lymphoma. The enormous intra-individual variations proofed for plasma as well as buffy coat concentration are unfavourable for oral 8-MOP therapy. Therefore, the introduction of liquid 8-MOP formulation that allows the direct administration of the drug in to the treatment bag of the ECP device is challenging.

Is there a beneficial effect of the calcium channel blocker diltiazem on cyclosporine A nephrotoxicity in rats?

To investigate whether or not there is a beneficial effect of diltiazem (D) on cyclosporine A (CsA) nephrotoxicity, renal function, CsA blood levels, and effects of CsA on biotransformation in the liver and on lipid peroxidation were characterized in rats. A single administration of D (60 mg/kg b.wt.) reduced urinary volume (UV), GFR and excretion of Na+ and K+, whereas a single dose of CsA (60 mg/kg b.wt.) alone had no respective effects. P-aminohippurate excretion was almost equal in all groups. Lower doses of D (and CsA) were without effects. After repeated CsA treatment a retardation in body weight gain was seen, with little effect of a co-administration with D hereon. In all tests, thymus mass was reduced by CsA, the weight of spleen, liver, adrenal glands, and kidney were not generally affected by any of the treatments. Furthermore, after repeated administration of CsA and/or D, urinary volume, GFR and Na+ excretion were reduced by CsA, too. Electrolyte concentrations in plasma showed no evident changes by any of the treatments for Na+ and Ca2+. After long time treatment, CsA and CsA + D quite similarly led to higher K+ but lower Mg2+ concentrations in plasma. Only with 7 days highest dosage treatment PAH excretion was reduced significantly by CsA and CsA + D treatment. Surprisingly, CsA levels measured in blood and in kidney tissue, showed lower values after co-administration with D compared to CsA treatment alone. This could be caused by higher activities of monooxygenase functions revealed after pretreatment with D alone. Reduced glutathione (GSH) contents in kidney were elevated in CsA and CsA + D treated groups. In general no significant differences were to be observed concerning lipid peroxidation and stimulated H2O2 formation. Altogether evident protective effects of diltiazem on CsA nephrotoxicity in rats could not be proven.

Progress in the management and treatment of infected pancreatic necrosis.

Infected pancreatic necrosis and sepsis are the leading causes of mortality in necrotizing pancreatitis. A review has been undertaken of the results of the past two decades relating to different surgical treatments of infected pancreatic necrosis. During the period 1978-85, the surgical treatment of necrotizing pancreatitis and its complications in our department consisted of the 'conventional' therapy (resection of the involved pancreatic tissue, or necrosectomy and drainage) in 61 patients, with a mortality rate of 36% (22 patients died). Since 1986, we have performed necrosectomy and other surgical interventions combined with continuous widespread lavage in 142 patients with infected pancreatic necrosis. The overall mortality decreased significantly to 6.3% (9 patients died). This result was achieved by means of aggressive surgical treatment, continuous, prolonged washing and suction drainage and supportive therapy, including immunonutrition, modifying the cytokine production and adequate antibiotic and antifungal medication. This surgical strategy provides the possibility for recovery in cases of necrotizing pancreatitis with septic complications.

Aminopeptidase B: a processing enzyme secreted and associated with the plasma membrane of rat pheochromocytoma (PC12) cells.

Aminopeptidase B (Ap-B) is a Zn2+-dependent exopeptidase which selectively removes Arg and/or Lys residues from the N terminus of several peptide substrates. Isolated and characterized from rat testes, this ubiquitous enzyme may participate in the final stages of precursor processing mechanisms. To test this hypothesis, we have investigated the secretion and subcellular localization of this enzyme in a rat cell line of pheochromocytoma (PC12 cells). By using a combination of biochemical and immunocytochemical methods, the following observations were made: (i) the level of aminopeptidase B detectable in the cell culture medium increased with time; (ii) 8-bromo-adenosine 3'-5'-cyclic monophosphate and the Ca2+ ionophore A23187 both stimulated enzyme liberation in the culture medium; (iii) brefeldin A, an inhibitor of vesicular transport from the endoplasmic reticulum to the Golgi apparatus, decreased enzyme secretion in a time-dependent manner; (iv) whereas nocodazole, a microtubule depolymerizing agent, inhibited enzyme secretion, cytochalasin D, a microfilament disruption agent, had no effect on released aminopeptidase B level; (v) immunofluorescence demonstrated the presence of aminopeptidase B in the Golgi apparatus; (vi) immunofluorescence, electron microscopy and tests of enzyme activity on intact cells showed an association of the peptidase with the external face of the plasma membrane. Together these data strongly argued in favour of the enzyme secretion by PC12 cells. It is concluded that aminopeptidase B may participate in processing events occurring either during its intracellular transport along the secretory pathway or at the plasma membrane level, or both.

[Aorto-bi-subclavian bypass in the treatment of left subclavian steal syndrome and stenosis of the innominate artery].

Authors report a case of a left side subclavian steal syndrome, caused by occlusion of the subclavian artery, combined with a high grade stenosis of the innominate artery. The rare combination of arteriosclerotic lesions of the supraaortic arteries have been treated by an aorto-bisubclavian bypass procedure performed from a median sternotomy. This is the first publication of this method in the Hungarian surgical literature.

Aminopeptidase B from the rat testis is a bifunctional enzyme structurally related to leukotriene-A4 hydrolase.

An aminopeptidase B (Ap-B) was previously purified to homogeneity from rat testis extracts and characterized. In the present work, by using oligonucleotides selected on the basis of partial amino acid microsequences of pure Ap-B and PCR techniques, the nucleotide sequence of a 2.2-kb cDNA was obtained. The deduced amino acid sequence corresponds to a 648-residue protein (72.3 kDa) containing the canonical "HEXXHX18E" signature, which allowed its classification as a member of the M1 family of metallopeptidases. It exhibits 33% identity and 48% similarity with leukotriene-A4 hydrolase, a relation further supported by the capacity of Ap-B to hydrolyze leukotriene A4. Both enzymes also were closely related to a partially sequenced protein from Dictyostelium discoideum, which might constitute the putative common ancestor of either aminopeptidase or epoxide hydrolase, or both. Ap-B and its mRNA were detected in the germ line and in the Sertoli and peritubular cells of the seminiferous tubules. Because the enzyme was found in the medium conditioned by spermatocytes and spermatids and in the acrosome during spermatozoa formation, together these observations suggested an involvement of this exometallopeptidase in the secretory pathway. It is concluded that this ubiquitous enzyme may be involved in multiple processing mechanisms.

[Cytokines in experimental acute pancreatitis].

The authors induced acute necrotizing pancreatitis in Wistar rat by intraductal injection of taurocholic acid (150 microliters or 200 microliters 6%). Plasma values of amylase, TNF, IL-6 levels and wet pancreas weight/body weight ratio have been determined. Histologic analysis of pancreas proved severe acute necrotizing pancreatitis with microabscess formation and beginning respiratory distress syndrome was observed in the lungs, TNF and IL-6 levels increased significantly after administration of 200 microliters 6% taurocholic acid. The authors emphasise the importance of cytokines in the development of acute necrotizing pancreatitis.

[Treatment of achalasia of the cardia using thoracoscopic esophago-myotomy].

We performed left thoracoscopic esophagomyotomy in four patients suffering from achalasia cardiac. In one patients the esophageal mucosa was opened during the myotomy. This was sutured through thoracoscopy and the per os feeding of the patient started on the 8th postoperative day, while in the other three patients this was done on the 2nd postoperative day. The mean emission of the patients was on the 6th postoperative day. Comparison of the status before and 6 weeks following the operation was done on the basis of results of different examinations i. e. x-ray, esophago-gastroscopy, manometry, pH-metry, isotope test. The favourable results of the operations were declared as all examinations revealed marked improvement and all patients have better swallow function and 6-9 kg increase in body weight.

Hormone replacement therapy and prevention of osteoporosis: risk assessment and practical advice.

A review of the Debrecen Regional Osteoporosis Program (DROP) in Hungary is given, with special reference to the detection of postmenopausal osteoporosis (PMOP) and its treatment by hormone replacement therapy (HRT). The new definition of osteoporosis by focusing on bone mineral density (BMD) measurements has the major advantage of practical usefulness. The algorithm of managing osteoporotic patients can be easily constructed from the result of bone densitometry as the primary diagnostic tool. The DROP serves a total population of 550,000, is equipped with a DXA bone densitometer, a bone metabolism laboratory and backed by a multidisciplinary team of clinicians from gynecology, radiology, rheumatology, internal medicine, and orthopedic surgery. In 1995 the total number of patients undergoing densitometry was 3170. In 2045 patients T scores of -1 or below were found. From this total number, 348 patients received HRT for 1 year or longer. The results of the treatment showed a positive response (i.e. no bone loss, or net gain) in 65%, while half of the 'non responders' proved in fact non compliant. The contradiction between risk assessment and early diagnosis is explained and replacing 'risk assessment' by 'selection criteria for bone densitometry' is proposed. 'Prevention of osteoporosis' is also to be replaced by 'prevention of complications', i.e. osteoporotic fractures. One of these measures is HRT. Its rational use in the prevention and treatment of osteoporosis and its relation to other treatment methods in the authors' own experience is discussed.

TGF-beta 1 and IL-6--new aspects in pancreas regeneration?

Plasma levels of interleukin-6 (IL-6) and transforming growth factor-beta (TGF-beta 1) were studied during cholecystokinin octapeptide (CCK-8)-induced regeneration after pancreas resection in rats. The weight of the pancreas and the DNA and protein contents increased significantly. The serum levels of TGF-beta 1 and IL-6 were increased significantly on days 7 and 14. There was no significant change in serum amylase levels. These findings indicate that cytokines such as TGF-beta 1 and IL-6 may play a role in the pathomechanism of pancreas regeneration.

The effects of glucocorticoids and a glucocorticoid antagonist (RU 38486) on experimental acute pancreatitis in rat.

The effects of glucocorticoids on acute pancreatitis are a matter of dispute. In animal experiments, dexamethasone and hydrocortisone significantly decreased the serum amylase activities 8 hours after the induction of pancreatitis. In the dexamethasone treated group, the serum IL-6 level was significantly decreased at 4 and 8 hours, while in the hydrocortisone treated group, all the IL-6 values were significantly diminished vs. the control group. As compared to the control, a glucocorticoid antagonist (RU 38486) did not influence the serum amylase activity, but significantly increased the serum IL-6 level. These results suggest that glucocorticoids may play a role in the control of pancreatitis caused by inhibition of cytokine production.

Plasma levels of TNF and IL-6 following induction of acute pancreatitis and pentoxifylline treatment in rats.

Activation of cytokine cascade is a decisive factor in determining the pathobiology of different inflammatory processes including acute pancreatitis. The purposes of this study were to determine the TNF and IL-6 levels after the induction of acute necrotizing pancreatitis, and to establish the effects of pentoxifylline on the cytokine production and the severity of pancreatitis. Acute necrotizing pancreatitis was induced by the retrograde injection of 200 microliters taurocholic acid into the pancreatic duct in male Wistar rats. TNF was titrated in a bioassay on cell line WEHI clone 164. IL-6 was measured via its proliferative action on the IL-6 dependent mouse hybridoma cell line B-9. Seven mg/kg pentoxifylline was administered intraperitoneally at the time of operation and/or 24 hours later. Rats were sacrificed, 48 or 72 hours after the operation. The TNF bioassay revealed high levels of TNF (36.6 +/- 6.0 U/ml) in the control group whereas levels decreased to zero in the pentoxifylline-treated group. The IL-6 bioassay likewise demonstrated high levels of IL-6 in the control group and markedly decreased levels in the pentoxifylline treated group (7083 +/- 2844 pg/ml, 6463 +/- 1307 pg/ml vs. 137.5 +/- 85.5 pg/ml, respectively, p < 0.05). The high mortality observed in the control group (43%) was sharply decreased by pentoxifylline administration to 11%. The data suggest that pentoxifylline is capable of modifying the cytokine production after 48 hours of induction of acute pancreatitis.

Intraoperative monitoring of the motor pathway using transtracheal stimulation of the cervical spine in dogs.

Although SEP monitoring of the spinal cord has been a well established method recently, not an ultimate, perfectly developed technique for monitoring of the motor system is known so far, particularly, because of the disturbing effect of narcotic drugs and relaxants on the motor evoked potentials. In this study the upper part of the spinal cord was stimulated in 14 anesthetized and relaxed dogs with a cathode attached to the intratracheal tube and an anode fixed to the cervical spinous processes. Single and serial stimuli were applied. Recordings were obtained from the exposed right femoral nerve and quadriceps muscle. Averaging was necessary when using serial stimulations. Responses were consequent and reproducible during regular anesthesia. The origin of the different responses in the spinal cord is discussed. The method seems to be appropriate for intraoperative monitoring of the thoracolumbar spine.

Laparoscopic deroofing of nonparasitic liver cysts. Comparison different methods.

Between 1982 and 1995 116 patients underwent operations because of NC of the liver, 18 of them were operated on by laparoscopic way. There were 94 male (mean age: 51.9 years) and 66 female (mean age: 54.6 years) patients. The mean size of the cysts were 57.4 mm in diameter (20-140 mm), In 94 cases they were solitary and in 11 they were multiple. In 31 cases we performed an enucleation, in 60 cases a fenestration and in one case a punction of the cysts. A liver resection had to be carried out in two cases. In 18 cases the fenestration of the cysts was performed by laparoscopy and in 10 cases cholecystectomy was simultaneously carried out. The most common complications were fever and wound suppuration. There was no mortality. Another 26 patients with NC have been treated with ultrasound guided puntion of the liver cysts. The cysts were solitary in twenty-three cases and multiple in three other cases. The number of the cysts were thirty-two. One case had to be operated on because of recurrence of the cyst. Mortality could not be observed. In the laparotomized group the average time of the hospitalisation was 14.3 days, and among the patients who were operated on by laparoscopic way 7.0 days. An intervention is indicated only in case of severe complaints or the growing of the lesion. Recently we prefer the ultrasound guided punction or the fenestration of the liver cysts by means of the laparoscopic way.

About the primary malignant tumors of small bowel.

Primary malignant tumors of small bowels are relatively rare among the GIA-tumors. There are no specific methods to find these tumors in an early stage. Only the specific radiological examination (double contrast method or CT-scan) or sometimes the specific jejunoscopy can prove the correct diagnosis before the operation. Usually surgeons have to operate because of the emergency situation caused by primary malignant tumors of small bowel. A total of 21 patients were operated on for malignant tumors of small bowels since 1st January, 1986 till 31st December, 1995 (during 10 years). 13 of the 21 were urgent cases. 3 perforations and 10 mechanical bowel obstructions were the indications of the acute surgical procedures. The preoperative diagnoses were correct in the eight elective operations. Types of operations: 8 jejunal-resection, 11 ileal-resection, 1 Whipple-operation, 1 ileal-resection with right hemicolectomy. Peri-operative mortality rate was zero. Histological reports: 13 adenocancers, 2 leiomyosarcomas, 5 malignant carcinoids, 1 anaplastic cancer. Lymphomas and tumors of papilla Vateri were excluded from this study. Most of the patients had abdominal symptoms earlier than 8 months before the operations. Earlier diagnosis may give a better chance to operate these tumors in earlier stages, and it can give a better chance for the patients for longer survival.

A tumor-selective somatostatin analog (TT-232) with strong in vitro and in vivo antitumor activity.

We report a series of new in vitro and in vivo data proving the selective antitumor activity of our somatostatin structural derivative, TT-232. In vitro, it inhibited the proliferation of 20 different human tumor cell lines in the range of 50-95% and induced a very strong apoptosis. In vivo TT-232 was effective on transplanted animal tumors (Colon 26, B16 melanoma, and S180 sarcoma) and on human tumor xenografts. Treatment of MDA-MB-231 human breast cancer xenografted in mice with low submaximal doses of TT-232 [0.25 and 0.5 mg/kg of body weight (b.w.)] caused an average 80% decrease in the tumor volume resulting in 30% tumor-free animals surviving for longer than 200 days. Treatment of prostate tumor (PC-3) xenografted animals with 20 mg/kg of b.w. of TT-232 for 3 weeks resulted in 60% decrease in tumor volume and 100% survival even after 60 days, while 80% of nontreated animals perished. We have demonstrated that TT-232 did not bind to the membrane preparation of rat pituitary and cortex and had no antisecretory activity. TT-232 was not toxic at a dose of 120 mg/kg of b.w. in mice. Long-term incubation (24 h) of tumor cells with TT-232 caused significant inhibition of tyrosine kinases in good correlation with the apoptosis-inducing effect. The level of p53 or KU86 did not change following TT-232 treatment, suggesting a p53-independent apoptotic effect. Preincubation of human breast cancer cells (MDA-MB-453) with TT-232 for 2 h decreased the growth factor receptor autophosphorylation. All of these data suggest that TT-232 is a promising and selective antitumor agent.
