Structural and ultrastructural patterns of the human preovulatory endometrium following clomiphene citrate treatment.

To investigate ultrastructural modifications in the endometrium of women receiving clomiphene citrate (CC) treatment, we performed endometrial biopsies in the preovulatory phase in 17 patients with anovulatory infertility and 16 normo-ovulating women with tubal or male sterility. Histology, scanning and transmission electron microscopy (SEM and TEM) revealed midproliferative phase aspects in 11.7% of the treated group and 6% of the controls, late proliferative aspects in 76.4% and 82%, and postovulation days 1-3 characteristics in 11.7% and 12% respectively. No significant differences were observed between the patients and controls; our data indicate that CC taken in a dose not exceeding 100 mg/day from day 3 of the cycle can induce endometrial maturation that is almost identical with that of a spontaneous preovulatory phase. This type of endometrium should therefore allow implantation of a fertilized egg and maintain unmodified the complex biohumoral relations with the embryo.

Marked deciliation and insufficient secretory modification of endometrial surface during treatment with a new progestogen-estrogen combination.

Endometrial modifications induced by SHB 209 AE, a new progestogen-estrogen combination, was investigated using scanning and transmission electron microscopy, in order to demonstrate specific variations of the endometrial surface and if these contribute to the contraceptive effects of the preparation. Endometrial biopsies were performed in 27 volunteers who had taken SHB 209 AE during the preceding 3 months. Endometrial biopsies of 15 normomenstruating volunteers were used as controls. Each fragment was subdivided for scanning and transmission electron microscopy examination. The ciliated cells were always significantly fewer in the various phases of the treatment cycle than in the control endometria although, as in the controls, their number and ratio with nonciliated cells was greater mid-way through the cycle. Ciliogenesis was always notably delayed compared to the normal endometria during the different treatment cycle phases. Secretory modifications appeared prematurely but remained incomplete through the treatment cycle. These modifications caused total and characteristic alterations of the mucosal architecture and cellular maturation that comprised the implantation phase.

Cytochemical study on human spermatozoa metabolism during in vitro capacitation.

In eutherian mammalian spermatozoa the capacitation is coupled to a specific type of metabolism, that is glycolysis or oxidative respiration. A cytochemical study was carried out on cytochrome oxidase and lactate dehydrogenase in human spermatozoa collected at different times during in vitro capacitation. Human spermatozoa were incubated in Biggers, Whitten and Wittingham's medium supplemented with 15% heat-inactivated human serum. Both histoenzymological reactions based on oxidative polymerization of diaminobenzidine (cytochrome oxidase) or on tetrazolium salts reduction (lactate dehydrogenase) can be quantitated and have been evaluated by microdensitometric method (Vickers M85). The results suggest that human spermatozoa depend almost quite on the anaerobic glycolysis during in vitro capacitation.

[Famotidine versus placebo in prevention of the recurrence of duodenal ulcer disease. A multicenter study in Germany, Austria and Italy].

The aim of this study was to gain experience concerning efficacy and safety of famotidine, the new H2-receptor antagonist, for the maintenance of duodenal ulcer disease. 344 patients whose acute duodenal ulceration had recently been healed under famotidine or ranitidine were recruited for a year maintenance treatment with 20 mg bedtime dose of famotidine or placebo. 167 patients were treated with famotidine over 6 and 52 over 12 months. The corresponding numbers in the placebo control were 177 and 21. A life table method of analyses showed that the ulcer relapse rate was consistently and significantly (p less than 0.01) lower on famotidine than on placebo after 6 months (26% [43/167] versus 55% [98/177]). Of the 52 patients treated with 20 mg famotidine at night for further 6 months 7 (14%) developed an ulcer relapse. Of the 21 patients treated for further 6 months with placebo 5 (24%) showed an acute ulcer crater at endoscopy. Famotidine was well tolerated in the longterm therapy. The results confirm the efficacy and safety of famotidine in the prevention of duodenal ulcer relapse for at least 6 months.

Application of the Goldman-Hodgkin-Katz current equation to membrane current-voltage data.

A new approach for the analysis of current-voltage (IV) data is presented and applied to a variety of published data collected from various systems. Our analysis of published results shows that our method of analysis can account for the observed IV data. The calculated permeability coefficients are in reasonable agreement with those calculated from ion fluxes. In those cases where two ions are assumed to carry the current, the ratios of the calculated permeability coefficients are in agreement with those ratios determined from the Goldman-Hodgkin-Katz voltage equation. In most cases, the entire IV curve can be accounted for by using our method of analysis. In several examples, only a portion of the IV curve is in agreement with the predictions. We attribute the failure to account for the IV data to reflect the failure of one or more of the assumptions used by the GHK current equation. In other cases, assuming that an additional ion carries the current, the treatment can account for the IV data. However, the identity of the extra ion cannot be established from these published data without additional studies (e.g., ionic replacement studies).

[The environmental characteristics of patients' rooms and their interactions with the lifting of patients].

The Law (Presidential Decree) of 14/1/97 requested the regions of Italy to define and develop procedures for the accreditation of their health facilities. Since accreditation promotes the quality of the system, the support of ergonomics can be fundamental. Ergonomics, whose quality objective is harmony of the environment-man-object relationship, is in a position to make a major contribution to the complete application of the procedures for quality certification (ISO regulation 9000) and of the regulations regarding mandatory safety regulations (Law 626/94). The present paper analyzes the tasks of health workers vis-a-vis patients, highlighting the improvement or worsening effect of environmental features which not only have a direct effect on movements and general activity but also have an indirect effect on psychological and behavioural conditions. The first requirement that should be taken into consideration in environments that are also used by patients is that everything that can facilitate or make their walking and access to facilities self-sufficient will certainly make the healthcare workers' tasks easier, which can then be limited to lifting required for treatment and washing. The ward, especially, is an area that is extremely important from the point of view of design because it is where the patient does practically everything and where the staff, too, are mainly engaged in hygiene/treatment tasks, involving both patients and their relatives. The bed must be able to be easily used by potentially disabled patients who have difficulty in walking and moving. The beds should therefore be equipped with or have close by handles and rails that assist the patient to lie down, get up and move from the bed; the bed should also have a manual or electrical device to adjust the height (from 40 to 70 cm) and have four wheels (that can each be blocked) to make it easier to move. The supporting legs should occupy as little space as possible and should not interfere with wheelchairs or other walking aids or with the movements of the staff. The head and foot-boards should have parts that can be used as grips. The space between both sides of the beds should be at least 90 cm and the space between headboard and wall 120 cm; bedside tables should be no deeper than 30 cm and be fitted with wheels. On the question of space distribution design, another important requirement is flexibility. Adequate flexibility of space distribution together with adaptable technological systems mean that the spaces and equipment of the ward can be easily adapted to the great variety of needs.

[Ergonomics in planning and reconstruction].

The state of well being of people is not represented only by any illness, but also by the satisfaction of all environmental components around him. For this reason, the hospitals demand new and more project attentions and in particular they demand specific attentions for all the environmental qualities (light, colours, indoor air qualities, and temperature), the equipment, the furniture and the privacy. This new attention to the requirements of people, during the phase of planning, needs the ergonomic studies. This kind of discipline, in fact, has got the objective of good relationships between men and context where they live. The study of ergonomic qualities needs different competence of multiple areas of acquaintance, that collaborate to analyse the requirements of sanitary structure, and in particular they analyse needs of different categories of people that work or stay in this place (the patients, visitors and sanitary staff). Moreover ergonomic studies concurs to analyse the various factors of complexity, of the buildings, that are very important for the project of hospital. From the scientific literature, we say that some physical environmental characteristics (light, colour, noise,) can influence the psychological conditions of people, and in particular they can generate three different categories of reactions: behavioural, neuropsychological, neuroendocrinal. For this reason the study of all the environmental factors, simple and composed, is a primary necessity in the hospital planning. In this way we can guarantee to the patient, a better sanitary service and a better comfort, and we can guarantee to all sanitary staff adequate conditions of security and functionality.

Serum Ca-125 concentrations in endometriosis.

Serum Ca-125 levels were measured in 154 patients undergoing laparoscopy or laparotomy for infertility and/or pelvic pain. Pelvic endometriosis at various stages was observed in 102 of the patients, and 52 had a visually normal pelvis and served as a control group. We found a significantly higher mean serum Ca-125 concentration in the patients with stages III and IV endometriosis compared with the controls (P less than 0.001) whereas in the patients with stages I and II disease the mean antigen level was comparable to that of the control group. Considering its low sensitivity (14.7%) the usefulness of serum Ca-125 measurements as an initial diagnostic test for endometriosis is scanty. On the contrary, because of its elevated specificity (100%), this test may be useful in indicating early surgical exploration of the pelvis in cases of infertility and/or dysmenorrhea which are associated with high values of Ca-125.

Poor reproductive prognosis in severe teratozoospermia with a predominant sperm anomaly.

A retrospective examination of the semen analyses performed in our clinic from January 1979 to September 1986 revealed that approximately 15% of the patients were affected by severe teratozoospermia (greater than 80% abnormal forms and greater than 5 x 10(6) sperm/ml). In approximately 8% of these cases, a single predominant anomaly (same defect in greater than 50% of the sperm) was reported and confirmed by subsequent analyses (n = 37). The types of monomorphic teratozoospermia encountered in this study included round head (n = 6), amorphous head (n = 16), small head (n = 6), tapering head (n = 2), pin head (n = 1) and midpiece anomaly (n = 6). The clinical data suggest that familial genetic factors are probably involved in round head-monomorphic teratozoospermia, whereas testicular factors may be associated with amorphous head-monomorphic teratozoospermia. No matter what type of monomorphic teratozoospermia, the data in vivo (no pregnancies recorded in the follow-up period ranging from 2-8 years) and in vitro (negative SPAs) suggest a poor prognosis for the couples affected by this syndrome.
