Hepatic lymphangiomatosis. Report of a case and review of the literature.

Hepatic lymphangioma is an extremely rare benign tumor, which usually occurs as part of a multiorgan lymphangiomatous involvement. We describe an adult patient with a huge hepatomegaly caused by a diffuse lymphangiomatous infiltration of the liver, which has never been reported before. Extrahepatic localizations could not be demonstrated. On ultrasonography and computed abdominal tomography, the liver lesions were not distinguishable from necrotic metastases. However, the endoscopic and histologic picture as well as the benign clinical evolution during a 2-yr follow-up allowed us to diagnose this rare benign disorder.

The sonographic aspects of haemobilia. Clinical and experimental study.

The sonographic aspect of the gallbladder in seven cases of haemobilia is analysed. The changes include a diffuse echogenic gallbladder content during the initial stage. Later, irregular shaped inhomogeneous non shadowing masses are seen in the dependent parts of the lumen. This variable aspect was studied in an experimental model by percutaneous injection of blood in the gallbladder of Guinea pigs. The hyperechoic gallbladder content, seen early after injection is a transitory phenomenon which seems related to red cell aggregation before coagulation occurs. During the later stage the intraluminal masses were shown by histology to represent bloodclots. In a patient the observation of such a rapid evolution from diffuse hyper-reflectivity to less reflective masses is strongly suggestive of haemobilia.

Radiographic evaluation of regional pulmonary dimensions and volumes: effect of age.

On chest radiographs performed at spirometrically controlled residual volume (RV), functional residual capacity (FRC) and total lung capacity (TLC), several distances were measured in 8 young and 7 elderly male subjects. Regional volumes above and below the minor fissure were calculated using Barnhard's method for thoracic gas volume determination. When the volumes above and below the fissure were expressed as a percentage of their volume at TLC, and related to overall lung volumes (in percent of TLC), graphs similar to the ones obtained in scintigraphic studies were obtained. This illustrates that the present radiographic method may provide information on regional pulmonary volumes. Regional TLC of the upper zone was larger in elderly subjects than in young ones, while the opposite was true for regional TLC of the lower zone, indicating that maximal regional expansion changes with age. For the upper zone, the changes in diameters between RV and TLC were largest in antero-posterior direction and smallest in cranio-caudal direction. For the lower zone, changes were largest in cranio-caudal direction. This suggests an anisotropic expansion of the upper and lower zone. This anisotropy was more pronounced in the young than in the elderly subjects.

Ultrasonic appearance of the filled and contracted normal gallbladder.

The echographic appearance of the wall of normal well-filled and contracted gallbladders was studied prospectively in 25 persons. Using gray-scale equipment, the wall of the filled gallbladder appeared as a single reflecting structure, whereas during contraction, three different components could be distinguished. The exact recognition of this particular pattern is important in the differential diagnosis of real wall thickening and incomplete contraction.

Applications of specific vascular opacification in CT of the upper abdomen.

Selective visualization of the arterial and venous structures by CT slices during the early vascular phase after a bolus injection has proven to be a great value in resolving the problems of false images, created by incorporation of these vascular structures in the silhouettes of abdominal organs. This constitutes an important advance in the study and differentiation of pseudopancreas pathology, pseudolacunar images in the hapatic parenchyma and in the study of the porta hepatis.

Computed tomography of the normal and pathologic muscular system.

A standard program of computed tomographic (CT) scanning of the human skeletal muscular system was developed. Scans were obtained on five levels: the neck, the shoulder girdle, the pelvic girdle, the thigh, and the lower leg. The normal size and density of several muscles were determined. CT scans were also obtained in patients with various neuromuscular diseases. Several types of lesions were found, and their morphologic descriptions and quantitative data are presented. It is concluded that CT opens new possibilities for the evaluation of muscular atrophy and hypertrophy.

[Study of the liver by computerised tomography. Assessment of one years experience involving 1,480 investigations (author's transl)].

During a period of one year, and in two departments with identical equipment, 1,480 underwent examination of the liver by computerised tomography (C. T.). A critical analysis of the method is made, based upon the diagnostic results obtained as well as the errors and gaps noted. After describing the normal liver, the authors give the "C. T. appearances" of the main hepatic conditions, based upon correlation with macroscopic pathological findings.
