[Quantification of cardiac function with multislice spiral CT using retrospective EKG-gating: comparison with MRI].

PURPOSE: To quantify left ventricular function derived from retrospectively ECG-gated multislice spiral CT (MSCT) data sets in comparison to MRI. MATERIALS AND METHODS: In 16 patients (14 males, 2 females, mean age 56.8 +/- 11.5 years), retrospectively ECG-gated MSCT angiography of the coronary arteries and breath-hold steady state free precession cine MRI were performed. From MSCT data-sets, 20 axial image series were reconstructed every 5 % of the RR interval. Multiplanar images were reformatted in the short axis orientation from axial images. End-systolic and end-diastolic images were selected. From these images end-systolic volume (ESV), end-diastolic volume (EDV) and stroke volume (SV) as well as the ejection fraction (EF) and myocardial mass (MM) were determined using the Simpson's method and compared with MRI. Furthermore, image quality was assessed for both imaging modalities using a four point grading scale. RESULTS: All parameters were found to have an excellent correlation between MSCT and MRI data (Pearson's correlation coefficient 0.95 - 0.99), without clinically relevant differences between both modalities. On average, the difference between both methods was 0.5 ml for ESV, 0.8 ml for EDV, 1.3 ml for SV, 0.9 % for EF and 2.3 g for MM. Image quality was slightly better for MRI (1.5 +/- 0.65) than for MSCT (1.64 +/- 0.74). CONCLUSION: Retrospectively ECG-gated MSCT angiography can not only visualize the coronary arteries but also enables precise quantification of the left ventricular function from the same MSCT data set.

[MR-guided coil embolisation of renal arteries in an animal model].

PURPOSE: To test the feasibility of MR-guided coil embolization with passive visualization in an animal model. MATERIALS AND METHODS: All experiments were performed on a 1.5 T interventional MR-scanner (Gyroscan ACS-NT, Philips, Netherlands). A gradient echo sequence was combined with radial k-space acquisition and sliding window reconstruction technique to calculate the raw data with a specially designed back projector, yielding a frame rate of 20 images per second. The images were directly displayed on LCD-screens next to the MR scanner. A multipurpose catheter with dysprosium markers was placed into the renal arteries of two pigs under MR guidance, and a nitinol coil and platinum coil each were deployed. The position of the coils was verified with magnetic resonance angiography and conventional angiography. RESULTS: MR-guided catheterization of renal arteries with subsequent coil embolization was successfully carried out but the configuration of the coils could not be visualized on the MR images. CONCLUSION: MR-guided coil embolization of renal arteries is feasible. Exact guidance of coil deployment is not attainable with passive visualization.

[Interstitial T1-weighted MR lymph fistulography with Gadomer-17 in an experimental animal model].

PURPOSE: To visualize and localize fistulas of the thoracic duct with interstitial T 1 -weighted MR lymphography using Gadomer-17. MATERIALS AND METHODS: In 10 domestic pigs, leaks of the thoracic duct were created surgically or interventional-radiologically. The lymphatic leakage was located within the abdominal portion of the thoracic duct in 5 pigs, within the thoracic portion of the thoracic duct in 3 pigs, and in both, abdominal and thoracic portions of the thoracic duct, in 2 pigs. Subsequently, 10 micro mol/kg KG Gadomer-17 (1.5-1.8 ml) was administered interstitially in both hind legs of the animals. MR lymphography was performed with a 1.5 T MR unit using two different 3D gradient echo sequences before and 10 - 90 minutes after administration of contrast material. RESULTS: Leaks within the abdominal portion of the thoracic duct were directly visible as opacified fistulas. Indirect signs of active lymphatic fistulas were increasing extravasations of contrast material and free abdominal fluid. The 3D gradient echo sequence with the highest planar resolution (TR = 8,7 - 8,8 ms, TE = 4,2 - 4,3 ms, FA = 40 degrees, matrix size = 327 x 512) was best suited for distinct delineation of the lymphatic system and detailed demonstration of the thoracic duct fistulas. Intrathoracic leaks could not be demonstrated by MR lymphography due to reduced lymphatic flow or extravasated contrast medium at the abdominal puncture site. CONCLUSION: Interstitial MR lymphography with Gadomer-17 allows sensitive detection and localization of abdominally located leaks of the thoracic duct.

Aneurysmal bone cyst: value of MR imaging and conventional radiography.

Our objective was to investigate the accuracy and the diagnostic value of different imaging features of primary aneurysmal bone cysts (ABC) in conventional radiographs and MRI. Conventional radiographs and MR images of 34 patients with a suspected aneurysmal bone cyst were reevaluated by six independent radiologists in a blinded fashion. Morphological features, MR signal characteristics, and contrast enhancement patterns were assessed. Diagnoses were correlated with histology. Sensitivity and specificity of the different imaging findings for each imaging technique were calculated. In 24 patients ABC was histologically proven. In 10 cases diagnoses other than ABC were established. Conventional radiography and MRI each demonstrated a sensitivity of 76.4 and 77.8% with a specificity of 55.0 and 66.7%, respectively. With combined use of both imaging modalities sensitivity and specificity increased to 82.6 and 70%, respectively. The ABC was significantly more often seen as circumscribed lesion with bone expansion, lobulation, septa, and pathological cortical bone reaction than those cases with different diagnoses. Septal contrast enhancement proved to be a useful MR imaging finding in suspected ABC. The combined use of conventional radiographs and MRI revealed the highest sensitivity in the diagnosis of ABC in relation to histology. Magnetic resonance imaging provides improved diagnostic specificity and valuable information on soft tissue changes.

[Percutaneous radiofrequency ablation of liver malignancies: first experience with a 200-W radiofrequency generator].

PURPOSE: To evaluate the effectiveness of a percutaneous radiofrequency ablation device, consisting of a LeVeen probe and a newly developed 200-W radiofrequency generator. MATERIAL AND METHOD: In 20 patients (9 male, 11 female), 30 primary and secondary liver malignancies were treated by CT-guided percutaneous radiofrequency ablation. The patients were separated in a potentially curable (n = 10) and a palliative (n,= 10) treatment group. The treatment was performed using an umbrella-shaped LeVeen probe with a diameter of 3.5 cm or 4 cm and a 200-W generator (Radiotherapeutics, Mountain View, CA, USA). All interventions were performed under general anesthesia by a standardized treatment protocol. The size of the lesions treated ranged from 2.2 x 2 x 2 cm to 10.8 x 10.1 x 10.5 cm. RESULTS: In all patients, the correct placement of the probe and radiofrequency ablation was technically successful. In two patients more than one treatment session was necessary to achieve the intended treatment result. In all patients of the potentially curable group a complete tumor necrosis could be achieved, whereas in the palliative treatment group complete tumor necrosis was only achievable in one patient. The achieved necrosis size ranged from 2.4 x 2.8 x 2.5 cm to 9.2 x 8 x 10.5 cm. In one patient a colon fistula was observed 4 weeks after treatment. No further complications were observed. The follow-up period ranges from 1 to 15 months (mean 4.5 months). CONCLUSION: By use of a powerful generator in combination with a LeVeen probe percutaneous treatment of even large liver tumors is possible. Radiofrequency ablation is a safe procedure for the minimal-invasive therapy of non resectable liver neoplasms.

[Selective CT mesentericography in the diagnostics of obscure overt intestinal bleeding: preliminary results].

AIM: To evaluate intra-arterial CT mesentericography (CTM) in the diagnostics of severe obscure overt intestinal bleeding in comparison with conventional mesentericography (MG) and surgery. METHODS: In 8 patients (23 - 82 years, mean 59 years), CTM was performed via the catheter left in the superior mesenteric artery after MG to detect the source of bleeding. Early and late-phase spiral CT scans were acquired after administration of contrast medium. Active bleeding was considered to be present if extravasation of contrast medium into the bowel was found. The results of MG and CTM were compared with the results of surgery. RESULTS: With MG active bleeding was found in one patient, with CTM in five patients. In three patients, both MG and CTM were negative. Six patients underwent surgery. Five cases of bleeding detected with CTM were confirmed by surgery. In one case, bleeding found with CTM was not confirmed by surgery. One patient underwent surgery although all imaging procedures were negative. The source of bleeding remained unknown. Surgically, the site of bleeding was located in the jejunum in 3 patients (jejunitis, jejunal ulcers, carcinoid), one patient had a diverticulum in the ascending colon. The colonic bleeding site was correctly localized with CTM, whereas the small bowel bleeding could only roughly be assigned to the proximal or distal jejunum or jejunoileal transition area. CONCLUSION: Preliminary results indicate that selective CTM is superior to MG in the evaluation of severe obscure overt intestinal bleeding.

Real-time MR Guidance for inferior vena cava filter placement in an animal model.

It was the aim of this study to examine the feasibility of real-time magnetic resonance (MR) imaging for MR-guided placement of inferior vena cava (IVC) filters, which were placed in five pigs via a femoral approach. The introducer sheath and dilator were marked with Dysprosium rings. The procedures were performed under MR guidance with use of a 1.5-T ACS-NT imager. Radial filling of k-space in conjunction with the sliding window reconstruction technique achieved real-time MR imaging with a frame rate of 20 images/sec. Simultaneous real-time visualization of the vascular anatomy and interventional instruments was achieved under real-time conditions and allowed correct placement of IVC filters in all five cases as confirmed by radiographic angiography.

[Differentiation of serous and purulent fluids in vitro and in vivo by means of diffusion-weighted MRI].

AIM: The aim of this study was to test the feasibility for differentiation of serous fluid collections and abscess fluid with diffusion-weighted imaging and to compare three different diffusion sequences. MATERIAL AND METHODS: Thirteen puncture samples (6 serous, 7 purulent) and 19 patients with either serous fluid collections (n = 12) or abscesses (n = 7) were examined with diffusion-weighted spin-echo (SE), stimulated-echo (STE), and fatsuppressed SE (SE-SPIR) sequences. For in vitro studies 4 different b-factors (0, 87, 355, 798 s/mm2 in SE and SE-SPIR and 0, 51, 204, 460 s/mm2 in STE) were chosen and the apparent diffusion coefficient (ADC) was calculated. For in vivo measurements identical sequences with two b-factors (0.598 s/mm2 in SE and SE-SPIR and 0.360 s/mm2 in STE) were applied and the normalized signal attenuation was calculated. A navigator-echo technique and peripheral pulse triggering was used for motion artifact reduction. RESULTS: The in vitro study yielded an ADC of serous fluid, which was close to that of free water, whereas for purulent fluid a significantly lower ADC was calculated. During in vivo examinations, serous fluids showed a strong signal attenuation (down to 22-32% of basic value) compared to a minor signal attenuation in purulent fluids (down to 86-94% of basic value) (p < 0.05). CONCLUSIONS: In summary, with all three investigated diffusion schemes serous and purulent fluids can be clearly differentiated.

Implantation of a new access device for hemodialysis (Dialock): initial experience in 5 patients.

PURPOSE: A new hemodialysis access port system was implanted. METHODS: The Dialock consists of a port-like double-valve, implanted subcutaneously below the clavicle, which is attached to two catheters, placed in the right atrium via the jugular vein. The device has been implanted in 5 patients (4 female, 1 male). RESULTS: In all 5 patients the implantation of the catheters and the port was technically successful. Total average duration of dialysis was 3.6 months. Two patients developed a port pocket hematoma 10-14 days post implantation, one of them required surgical revision. One port was explanted due to septicemia, whereas a port infection was not confirmed. One patient showed a thrombotic occlusion of both catheter tips 8 days after implantation, fixed by catheter exchange. Another patient presented with slight migration of the port catheters, which was managed by refixation of the port within the pocket. Beside these complications, the devices were working well. CONCLUSION: The Dialock system offers an interesting alternative to external catheters for hemodialysis. With respect to the complications it deserves further studies to determine its future role in the field of vascular access.

Nephron-sparing percutaneous ablation of a 5 cm renal cell carcinoma by superselective embolization and percutaneous RF-ablation.

PURPOSE: To report on the nephron-sparing, percutaneous ablation of a large renal cell carcinoma by combined superselective embolization and percutaneous radiofrequency ablation. MATERIALS AND METHODS: A 5 cm renal cell carcinoma of a 43-year-old drug abusing male with serologically proven HIV, hepatitis B and C infection, who refused surgery, was superselectively embolized using microspheres (size: 500 - 700 microm) and a platinum coil under local anesthesia. Percutaneous radiofrequency ablation using a 7 F LeVeen probe (size of expanded probe tip: 40 mm) and a 200 Watt generator was performed one day after transcatheter embolization under general anesthesia. RESULTS: The combined treatment resulted in complete destruction of the tumor without relevant damage of the surrounding healthy renal tissue. The patient was discharged 24 hours after RF ablation. No complications like urinary leaks or fistulas were observed and follow up CT one day and 4 weeks after the radiofrequency intervention revealed no signs of residual tumor growth. CONCLUSION: The combined transcatheter embolization and percutaneous radiofrequency ablation of renal cell carcinoma has proved technically feasible, effective, and safe in this patient. It may be offered as an alternative treatment to partial or radical nephrectomy under certain circumstances.

[Interstitial T1-weighted MR lymphography with use of the dendritic contrast agent Gadomer-17 in pigs].

PURPOSE: To investigate the enhancement of regional lymph nodes, lymph vessels and the thoracic duct after subcutaneous administration of the dendritic contrast agent Gadomer-17. MATERIALS AND METHODS: Interstitial T(1)-weighted MR lymphography was performed in 15 domestic pigs in a 1.5 T MR scanner using the body coil. The contrast agent was injected subcutaneously at a dose of 2.5, 10 and 20 micromol gadolinium/kg/bw (1.4 - 2.3 ml) in the hind legs of 5 pigs, respectively. T(1)-weighted coronal 3D-gradient-echo images were obtained 1 to 150 minutes and 24 hours after contrast material injection. RESULTS: Inguinal lymph nodes, iliac lymph nodes and lymph vessels showed peak enhancement within 10 minutes after subcutaneous administration of the contrast agent. The enhancement of the lymphatic system decreased slowly until 150 minutes post injection. The time-response study, which was performed for inguinal and iliac lymph nodes, revealed the best lymphographic effect for Gadomer-17 at a dose of 10 micromol gadolinium/kg/bw. At this dose the paraaortic lymph nodes and the thoracic duct were also best visualized. CONCLUSION: Interstitial administration of the evaluated dendritic contrast agent Gadomer-17 is highly efficient for the performance of T(1)-weighted MR lymphography in regional lymph nodes, lymph vessels and the thoracic duct.

Interstitial T1-weighted MR lymphography: lipophilic perfluorinated gadolinium chelates in pigs.

PURPOSE: To investigate the enhancement of the regional lymph nodes, lymphatic vessels, and thoracic duct after interstitial administration of lymphotropic perfluorinated gadolinium chelates at magnetic resonance (MR) imaging. MATERIALS AND METHODS: Two perfluorinated gadolinium chelates, gadofluoramide and gadofluorine 8, were injected subcutaneously into the hind legs of 10 pigs, respectively. Both contrast media were studied at doses of 10 and 25 micromol per kilogram of body weight. T1-weighted three-dimensional gradient-echo and maximum intensity projection images were obtained at 1.5 T between 1 and 210 minutes and 24 hours after injection. The contrast agents were qualitatively compared regarding enhancement and depiction of the regional lymph nodes, lymphatic vessels, and thoracic duct. RESULTS: The inguinal and iliac lymph nodes and lymphatic vasculature enhanced substantially within 10 minutes after subcutaneous administration of both lymphotropic contrast agents. Gadofluorine 8 showed a lymphographic effect superior to that of gadofluoramide. The paraaortic lymph nodes and thoracic duct were best visualized 10--50 minutes after injection of 25 micromol/kg of gadofluorine 8. Lymphatic system enhancement diminished after 2 hours, and the liver and bowel tract enhanced within 24 hours. CONCLUSION: Interstitial administration of perfluorinated gadolinium chelates offers great potential for T1-weighted MR lymphography with positive enhancement of the lymph nodes and lymphatic vasculature.

Diagnosis of pulmonary arterial hypertension and pulmonary embolism with magnetic resonance angiography.

BACKGROUND: Pulmonary magnetic resonance angiography (PMRA) has been proven to be accurate for the diagnosis of suspected acute or chronic pulmonary embolism (PE). Only limited data exist on the reliability of PMRA for the diagnosis of acute and chronic pulmonary artery hypertension (PAH). The aim of this study was to determine the accuracy of PMRA in the differentiation between patients suffering from PAH of varying etiologies. METHODS: Fifty patients (21 women; mean [+/- SD] age, 52 +/- 16 years) were examined with gadolinium-enhanced PMRA for the evaluation of pulmonary artery (PA) disease. The diagnosis of PAH (ie, systolic PA pressure of > 35 mm Hg) was determined by Doppler echocardiography. The criteria for the diagnosis of chronic PAH by PMRA were dilated central PAs (diameter > 28 mm) and abnormal proximal-to-distal tapering of the PAs. The diagnostic criterion for acute and chronic PE was the presence of an intravascular filling defect. RESULTS: Chronic PAH was present in 18 patients, which was correctly identified by PMRA in 16 patients (sensitivity, 89%). All patients without PAH had normal findings on PMRA (specificity, 100%). Only 1 of 18 patients with normal findings on PMRA showed moderate chronic PAH (negative predictive value, 94%). PAH due to acute/subacute pulmonary thromboembolism (15 patients) was identified in all patients (sensitivity, 100%). Acute PAH was differentiated from chronic PAH in all cases by the detection of intravascular filling defects and the lack of abnormal proximal-to-distal tapering of PAs. CONCLUSIONS: PMRA is a promising noninvasive imaging modality for the identification of patients with acute or chronic PAH. This technique should be considered a sensitive and highly specific screening tool for suspected chronic PAH.

[Radial k-scanning for real-time MR imaging of central and peripheral pulmonary vasculature].

PURPOSE: To depict the central and peripheral pulmonary vessel anatomy with real-time radial MR scanning without respiratory control. METHODS: Three healthy volunteers and one patient with pulmonary embolism proven by spiral CT angiography were studied with a 1.5 T MR imaging system. First, a breath-hold, contrast-enhanced MR angiography was performed for comparison of accuracy. Gradient echo images (TR 16 ms, TE 4 ms, flip angle 18 degrees) were obtained applying a combination of radial scanning and the sliding window reconstruction technique. A dedicated back-projector allowed data reconstruction in real-time with a frame rate of 20 images per second. Scanning was performed during free breathing. RESULTS: The described technique depicted the central and peripheral portions of the normal pulmonary anatomy with comparable image quality as the 3D contrast-enhanced angiography. Visualization of the pulmonary emboli as demonstrated by spiral CT was possible. CONCLUSION: Real-time radial scanning allows promising image acquisition of the pulmonary vasculature without respiratory control. Further technical improvements and clinical trials are required to evaluate its role in the diagnosis of vascular pathologies.

[Interventional magnetic resonance imaging--non-invasive imaging for interventions].

As a prerequisite for MR-guidance of interventional procedures, instruments have to be well depicted in the MR image without obscuring or distorting the underlying anatomy. For non-vascular interventions the imaging speed has to be in the range of seconds while control of vascular interventions requires real time imaging speed. The imaging contrast has to be maintained as well as a high spatial resolution. Furthermore, sufficient patient access has to be provided by the MR scanner. Neither an ideal magnet nor the optimal single sequence are available to fulfill the above-mentioned criteria. The type of sequence--gradient echo versus spin echo--together with changing of the echo time and phase encoding direction will ensure an appropriate size of the artifact and thereby of the appearance of the instrument in the MR image. The feasibility of non-vascular MR-guided interventions has been proved at field strengths ranging from 0.064 T to 1.5 T. Bone biopsies, soft tissue biopsies, drainages, and control of interstitial thermo- and cryotherapy have been reported. For vascular interventions, different real time MR strategies are currently under investigation. The development of dedicated catheters and guide wires has enabled MR-guided dilatations, stenting, placement of vena cava filters, and TIPS procedures. Considering the fast progress being made in this field, there can be no question that interventional MRI will become a well-accepted clinical tool offering potential advantages such as excellent soft tissue contrast, multiplanar imaging, flow measurements, high resolution imaging of vessel walls, and lack of ionizing radiation.

[MRI of osteomyelitis: sensitivity and specificity of STIR sequences in comparison with contrast-enhaned T1 spin echo sequences].

PURPOSE: To evaluate the need for additional MR sequences including administration of Gd-DTPA after inconspicuous Short-Tau Inversion-Recovery (STIR) sequence to exclude the diagnosis of osteomyelitis. MATERIAL AND METHODS: 112 MR examinations of 79 patients acquired for the detection of possible osteomyelitis were analyzed retrospectively. All examinations were performed at 0.5 T including STIR, T1-weighted spin echo sequences (T1 SE) before and after application of Gd-DTPA. Additionally, 93 T2-weighted spin echo sequences were available. The examinations were analyzed by two experienced radiologists. First, the STIR sequences were studied, followed by the T1 SE images before and after administration of contrast material. Finally, the T2-weighted images were evaluated. Diagnoses were confirmed by operation (22), biopsy (10), and follow-up (80). RESULTS: In 53 cases osteomyelitis was diagnosed, while the remaining 59 cases suffered from another disease. The sensitivity of the STIR sequence was 100% while the specificity for osteomyelitis was 49.2%. The specificity increased to 79.7% by including T1 SE images into the analysis and reached 83.1% after considering the contrast enhanced images. T2-weighted images yielded no additional information. CONCLUSION: The combination of STIR and T1SE images shows a high sensitivity and specificity for osteomyelitis, thus obviating the need for any additional examinations.
