[Hypoxic training -- effective method of raising skin vitality in cases of local plastic operations].

In spite of the complications after expander use the advantages of local plastics predetermine constant return to the method. Systemic use of hypoxic training produces stimulating effect on the skin microcirculation and can be used widely in clinical surgical practice in cases of local plastic operations to prevent possible complications reducing consequences of blood supply disturbance and tissue oxygen provision. The method of hypoxic adaptation let us receive as non-medicamentous mean to prepare for shorter period of time more vital flap.

[Clinical and morphological parallels in combined deformations of the nose].

Analysis of clinical characteristics of 56 patients with combined deformations of the nose and morphological (histological and histochemical) changes in the nasal septal cartilage in 20 patients showed that common nasal deformations are complicated with age, impeding nasal respiration. The intensity of dystrophic changes in chondrocytes and intercellular cartilaginous matrix depends on the duration of posttraumatic or congenital deformation but not on its clinical severity. This necessitates earlier interventions, before the development of pronounced deformation of the whole nose and deterioration of nasal respiration.

[Use of fibrin glue for covering microsurgical autotransplants with free split skin].

An experimental study on 90 Wistar rats has been carried out. A split skin transplant about 2.25 cm(2) in size was taken from the back of the animal with scalpel and fixed on the neck's muscles with: 1) 8 sutures; 2) pressure dressing of "Pelot" type; 3) 0.2 ml of "Tissukol" fibrin glue. There were no complications. Complete skin retention was achieved in all the cases. Histological study demonstrated acceleration of reparative processes and decreased inflammation in autotransplant and muscular bed due to improvement of revascularization, a decrease of fibrous transformation of the skin graft and muscular tissue when fibrin glue (to a greater extent) and pressure dressing (to a lesser extent) were used for fixation of skin graft. But it is preferable to use fibrin glue due to acceleration of complete skin retention compared with pressure dressing of "Pelot" type.

[An experimental substantiation of nitric-oxide containing gas flow in the treatment of eye traumas].

The hypothesis on the biostimulating effect of exogenous nitric oxide (NO) was made use of to develop a new method to stimulate the healing of wounds through treating them by a NO saturated gas flow. The above gas flow is generated by air-plasma unit "Plazon". The experimental and clinical studies confirmed that the NO-therapy is a highly effective treatment method for different lesions of the skin and soft tissues. We tried to use the above method in ophthalmology. A comprehensive experimental study was carried out to assess the impact of the NO-containing gas flow on the eyeball structures. An optimal mode was designed, which does not exert any influence on the intraocular pressure, Ph of the lachrymal fluid, antioxidative activity and on the proteinase-inhibitor balance in tears; no morphological changes occurred in the ocular tissue structures. The mentioned morphological and biochemical studies confirmed that the application of the NO-containing gas flow speeds up the healing, process of both an experimental cornea erosion and penetrating corneal wounds. Optimal modes of NO-therapy were defined for both types of lesions.

[Ozone-ultrasonic therapy in the treatment of purulent wounds].

Based on planimetric, bacteriologic and histologic study high efficiency of local ozonotherapy of wound in combination with low-frequency ultrasound was demonstrated experimentally on rat model of infected purulent skin wound. This method was used in 45 patients with purulent wounds of soft tissues (postoperative, posttraumatic, burn, sore spot) that led to fast cleaning of wound surface, decrease of bacterial contamination and granulations. It permitted to eliminate inflammation and to create optimum conditions for wound closure.

[Use of gas flow with nitrogen oxide (NO-therapy) in combined treatment of purulent wounds].

NO-therapy was used in 40 patients with purulent wounds of soft tissues. Bacteriological, cytological and morphological examinations demonstrated acceleration of wound process compared with control group (40 patients without NO-therapy). Laser doppler flowmetry demonstrated significant improvement of microcirculation in wound zone during NO-therapy. This method shortens by 3-5 days the time of preparation of wound surface for surgical closure.

[Use of a new biological factor--exogenous nitric oxide--during surgical treatment of periodontitis].

In order to improve the efficiency of surgical treatment of periodontitis, the operative wound was exposed to nitrogen oxide (NO) during osteogingivoplasty and on days 3, 5, and 6 after it. Clinical and laboratory data making use of the cytomorphometrical method indicate that NO exposure decreased and maybe prevented secondary injury to the operation wound in up to 46.6% patients, which was confirmed by decrease and even normalization of destruction index and inflammatory destructive index on days 7 and 14, which was not observed in the reference group patients not exposed to NO.

[Holmium laser in chondrology: potentials and prospects].

To study the mechanism of simulation of a cartilage, to define the optimum parameters and laser radiation regimens, the authors made a complex of in vitro experiments on the cartilages of the nasal septum in man and domestic animals and in vivo experiments on those of the concha in rabbits and pigs. Holmium laser radiation is shown to be the most suitable tool for formation of a cartilage. Evidence is provided for the possibility of irreversibly altering the shape of a cartilage during laser radiation. A procedure has been developed for non-invasive correction of the septum of the nose for its curvature. Restored or improved nasal respiration was observed in more than two thirds of the patients. Laser surgery is not of age-limited application, it is noninvasive, can be performed in the outpatient settings and requires no drug treatment in the postoperative treatment.

[New approaches to the treatment of patients with idiopathic mitral valve prolapse].

AIM: To assess efficiency of magnerot, magnesium orotate, in patients with idiopathic mitral prolapse (IMP). MATERIAL AND METHODS: 84 patients with IMP were randomized to the study group (43 patients) and control group (41 patients). Patients of the study group received magnerot tablets (Germany) containing 500 mg of magnesium orotate (daily dose 3000 mg) for 6 months. The examination performed before the treatment and 6 months after it included: modified clinical and phenotypic records, echocardiography, 24-h ECG and AP monitoring, spectral analysis of cardiac rhythm variability, evaluation of quality of life according to Visual Analog Scale and Disability Scale and of treatment results according to Clinical Global Impression scales, measurements of magnesium in the hair by plasmic nuclear emission spectrometry, histological and histochemical skin tests. RESULTS: IMP patients appeared to suffer from magnesium deficiency which is responsible for many symptoms in mitral prolapse. 6-month therapy with magnerot completely or partially reduced the symptoms in more than half the patients. Positive changes were registered primarily in clinicofunctional manifestations. Morphological changes in the skin correlating with the disease severity alleviated. CONCLUSION: Good objective and subjective response to magnerot 6-month therapy (3000 mg/day) is demonstrated.

[Reestablishing of nasal respiration with Holmium laser in distortion of nasal septum in cartilaginous part].

To study mechanisms of cartilage modelling, estimation of optimal parameters and regimens of laser radiation, the authors made in vitro and human experiments on nasal septum cartilages and in vivo experiments on cartilages of the floor of the auricle in rabbits and pig. Ho-laser radiation is thought the most appropriate tool for cartilage shaping. A method of non-invasive correction of the nasal septum cartilage in its distortion has been designed. 2/3 of the patients recovered or improved nasal respiration. Laser operation has no age restrictions, is non-invasive, can be made outpatiently, is not followed by pharmacological treatment.

[The tissue reaction to acrylic plastics modified by supercritical extraction with carbon dioxide].

The process of extraction of admixtures from acryl plastic widely used in dentistry by means of supercritical carbon dioxide (sc-CO2) was studied and effects of extraction conditions on biocompatibility and toxicity of resultant materials assessed, sc-CO2 effectively purified the specimens from toxic compounds (monomers and low-molecular oligomers, methylmethacrylate, dichloroethane) and notably improved the biocompatability of polymer implants. Tissue reaction to ethacryl and protacryl depends on the degree of implant polymerization and duration of extraction of toxic substances from polymer.

[Stimulation of regenerative processes and correction of the functional activity of the liver in its partial resection and toxic lesions].

The effects of hepatotropic growth factors (HGFs) and phospholipid drugs on the recovery of functions and the regeneration of the rat liver were studied in CC14-induced toxic damage and after partial hepatectomy (PHE). HGFs isolated from the cytoplasmic cells of the regenerating liver, as well as from the liver of the animals given prodigiozan and from the media taken after culturing the explants of the regenerating liver were found to stimulate DNA synthesis and hepatocytic proliferation following PHE and in the cirrhotic liver. Prodigiozan was shown to induce the formation of HGFs not only in the rat liver following PHE, but in the liver of intact animals. It was established that the covalently binding complex of albumin and bilirubin stimulated the synthesis of proteins and DNA in the regenerating liver, but non-covalently binding complex inhibited these processes. When CC14 was administered to the animals, the two complexes enhanced the reparative synthesis of DNA, without changing the level of replicating synthesis, the non-covalently binding complex completely eliminating the single-strand breaks in DNA. Phospholipid agents containing soybean and sunflower phosphatidylcholines increased the synthesis of RNA and albumin, which were decreased due to exposure to CC14 and had the property of stimulating the synthesis of total DNA and considerably enhancing that of mitochondrial DNA.

[Gapkol--a new osteoplastic material].

Hapcol, a new osteoplastic material, representing a biological composition on the basis of a natural biopolymer collagen with biologically compatible osteotropic mineral hydroxyapatite has been developed and allowed for clinical use. It is intended for use in dentistry in surgical treatment of periodontal diseases, deformations of the abutment tissues of the face, filling of bone defects after cystectomy, surgery for bone plasty, etc. Hapcol is characterized by antiinflammatory action, stimulates repair osteogenesis and collagenogenesis. Joint stock company Polistom starts commercial manufacture of the new material.
