[Vacuum phenomenon in the intervertebral disc].

Author observed two cases of vacuum phenomenon (pneumatic shadow) in the intervertebral disc. Out of a detailed description of the patients the diagnostic significance of the X-ray sign is discussed.

Experimental and clinical percutaneous angioscopy experience with dynamic angioplasty.

The authors have used ultrathin angioscopes with high optical resolution to assess the effects of dynamic angioplasty in vitro and in vivo. Experimentally, angioscopy was used to study the effects of the 5F "Kensey" catheter in "normal" porcine coronary arteries (NPCA) and postmortem human coronary arteries (PMHCA). In NPCA, the catheter keeps a coaxial position. Intimal flaps (IFs) were seen in 21/23 NPCAs. They occurred with all cam rotation speeds and were usually single and small (less than 25% of lumen). Perforations in patent arteries were rare (1/23). However, when the catheter was forced against the wall by passing through a narrowing of 5F diameter (made by a band ligature), perforations were more common at higher cam speeds. The epicardium remained intact in two thirds of perforations. Angioscopy visualized perforations in only 10% of cases (1/10), the common sign being that of large and multiple intimal flaps, which were often obstructive (5/10). In PMHCAs, angioscopy was more sensitive than angiography in detecting atheromatous lesions. The authors were able to give a better assessment of the effect of dynamic angioplasty on treated lesions, including the demonstration of intimal flaps that were not visible on angiography. In vivo, they have performed percutaneous angioscopy before and after dynamic angioplasty using 8 French Kensey catheters. Angioscopy revealed features that were not shown angiographically.

Plasma eicosanoids, platelet function and cold sensitivity.

As abnormal eicosanoid (prostaglandin) metabolism has been suggested as a factor in the aetiology of vasospastic diseases we have measured levels of stable eicosanoid metabolites using a radioimmunoassay in 30 normal subjects and 31 patients with Raynaud's phenomenon. There were 13 patients with primary Raynaud's, ten with Raynaud's secondary to scleroderma and eight men with vibration white finger (VWF) disease. We have also measured platelet aggregation to adenosine diphosphate (ADP), collagen and adrenaline in 19 normal subjects, 22 patients with primary Raynaud's, 12 with Raynaud's secondary to scleroderma and 14 men with VWF. When compared with our normal subjects, patients with VWF have an elevated thromboxane B2 level, with a normal 6-keto-prostaglandin F1 alpha (6-keto-PGF1 alpha) level. Their platelets are less sensitive to ADP and collagen. Patients with primary and secondary Raynaud's have elevated thromboxane B2 levels but this is much more marked in the secondary group. Patients with primary Raynaud's have a normal 6-keto-PGF1 alpha level but in patients with secondary Raynaud's the 6-keto-PGF1 alpha level is markedly raised. The platelets from both groups are more sensitive to ADP and collagen and this is more marked in the secondary group. Whether these phenomena are a cause or an effect of vasospasm remains unknown.

Risk to surgeons: a survey of accidental injuries during operations.

A survey was conducted with the participation of eight consultant general surgeons, two consultant urologists, four consultant orthopaedic surgeons and four surgical residents to find the incidence of accidental injuries to surgeons and their assistants during operations. Of the total of 2016 operations over a one year period there were 112 reported accidental injuries. These included 107 needle stick injuries, 4 knife cuts and 1 diathermy burn. We found that accidental injuries to surgeons during operations were inevitable.

Functional properties of neoplastic T cells in helper T cell prolymphocytic leukaemia with IgM hypergammaglobulinaemia.

A case of T cell prolymphocytic leukaemia of helper/inducer T cell phenotype with IgM hypergammaglobulinaemia in a 65-year-old Saudi man is presented. The neoplastic T cells in the patient had an OKT3+, OKT4+, OKT8-, OKT11+, OKCLL+, OKIa1+, OKDR+, Tac+ phenotype. The presence of OKIa1+, OKDR+, and Tac+ antigens indicates that the leukaemic T cells were in an activated state. The cells responded to phytohaemagglutinin, but showed a diminished response to concanavalin A. The reduced interleukin 2 receptor expression and interleukin 2 production were associated with defective concanavalin A-induced proliferation. There was suppression of mixed lymphocyte culture reaction between the patient and two healthy donors in response to concanavalin A. In vitro immunoglobulin production experiments demonstrate that the leukaemic T cells provided an enhanced helper cell function to produce IgM, and suppressor cell function to produce IgG immunoglobulins by pokeweed mitogen-induced normal B lymphocyte differentiation. In this study we also found that the patient's T cells proliferate in response to lipopolysaccharides, thus providing the first evidence that leukaemic (OKT4+) T cells from a prolymphocytic leukaemia patient can be stimulated by lipopolysaccharides.

Peripheral T cell lymphomas: an immunological study of seven unusual cases.

A multiparameter study of malignant lymph node cells and peripheral blood lymphocytes of seven patients with peripheral T cell lymphoma is presented. The results of monoclonal marker studies showed three cases of helper-suppressor T cell lymphoma (OKT4+, OKT8+), one case of suppressor T cell lymphoma (OKT8+), and three cases of helper T cell lymphoma (OKT4+). Immunophenotypic heterogeneity of neoplastic T cells with expression of pan-T antigens, OKT3+, and OKT11+ (erythrocyte rosetting+) was observed in most patients. Six of the seven cases tested showed Ia and DR antigens. No relationship was detected between patterns of reactivity with T cell reagents and histological types. When tested, the in-vitro malignant T cells of five patients proliferated in response to concanavalin A (Con A), but had poor response to phytohaemagglutinin. The interleukin 2 receptors showed maximum expression on Con A-activated T cells of five patients, and phytohaemagglutinin-activated T cells of one patient. The neoplastic T cells (OKT4+, OKT8+) of one patient studied had suppressor activity for IgG and IgA, and helper activity for IgM synthesis on pokeweed mitogen-induced normal B cell differentiations.

A monoclonal antibody and functional study of malignant T cells of a patient with suppressor-T-cell lymphoma.

We describe the immunologic characteristics of malignant T cells of a T-cell lymphoma patient. The neoplastic cells in lymph node expressed OKT3+, OKT11-, E-rosetting-, OKT4-, OKT8+, OKIa1+, OKDR+ suppressor-T-cell phenotype. Functionally, these malignant T cells did not respond to phytohemagglutinin, but produced a good response to concanavalin A. A defect in expression of interleukin 2 receptors was evident in phytohemagglutinin-activated T cells. In vitro immunoglobulin production experiments demonstrated that patient's malignant T cells possess helper function on normal B cells to produce IgM, and suppressor function to produce IgG.

Polyanionic inhibitors of calcium oxalate crystal agglomeration in urine.

The excretions and relative potencies of various macromolecular inhibitors of the crystallisation of calcium oxalate (CaOx) were measured in the urines of idiopathic calcium stone-formers and normal subjects. The stone-formers excreted significantly less polyanionic macromolecules in their urine than did the normals, the difference being attributable to the lower excretions of glycosaminoglycans (GAGS), ribonucleic acid (RNA) and Tamm-Horsfall mucoprotein (THM), all of which are precipitable with alcian blue. The relative potencies of the various inhibitors measured under 'whole urine equivalent' conditions using a batch crystallisation system, showed that the order of inhibitory activity towards CaOx crystal agglomeration was RNA greater than GAGS greater than THM greater than pyrophosphate (PPi). This paralleled the order of ability of these inhibitors to produce a high negative zeta potential on the surface of CaOx crystals.

A horseradish peroxidase study of vagal motoneurones with axons in cardiac and pulmonary branches of the cat and dog.

The location, within the brain stem, of vagal efferent neurones with axons in the cardiac and pulmonary branches of the cat, and the cardiac branch of the dog, have been defined. Horseradish peroxidase was applied to the cut central ends of these vagal branches and, after an appropriate time, sections of the brain stem were examined for labelled cell bodies. Labelled cell bodies were found ipsilaterally in the dorsal motor vagal nucleus, the nucleus ambiguus and in the reticular formation between these two nuclei. We suggest that the dual location for cell bodies of vagal axons may represent a separation of somata with myelinated and non-myelinated vagal axons to the nucleus ambiguus and the dorsal motor vagal nucleus respectively.
