[Clinical and psychological analysis of migraine population].

The study presents a clinical analysis of a large population of migraine patients (n = 320). Computed database used in the study included over 100 clinical parameters and was based on analysis of primary medical records. As a result of statistic processing of the migraine archive, a general review of clinical picture and migraine course in patients from Moscow and Moscow region community and analysis of the peculiarities of migraine heredity, concomitant symptoms, provoking factors, co-morbid diseases, state in attack-free period, psychological characteristics and quality of life are provided. Factors determining course severity and quality of life in migraine are discussed.

[Clinical and neurophysiological analysis of pyramidal syndrome in right and left hemisphere stroke].

The study aimed at investigation of clinical and neurophysiological features of pyramidal syndrome in patients with vascular damage of right- and left hemisphere in stroke. Forty patients with consequences of acute brain circulation damage were studied: 21 of them with right hemi-paresis (RHP) and 19--with left hemi-paresis (LHP). Cliniconeurological analysis, transcranial magnetic stimulation (TMS), evoked abdominal reflexes, movement motor potential method and somatosensory evoked potentials were used for the examination. Pronounced paresis, tone disturbance, Babinsky symptom, other extensor pathological feet signs, Zhukovsky hand reflex, pseudobulbar syndrome, gait disorder were determined more frequently in patients with LHP. Patients with RHP were characterized by more expressed distal hand paresis and pathological bending feet signs. Compared to RHP patients, those with LHP displayed significantly longer central conduction in TMS, increase of latent periods of components N20, P25, N30 of somatosensory evoked potentials and motor potential area reduction. Functional features of brain hemispheres may to play a certain role in pyramid syndrome formation, right hemisphere damage being characterized by more expressed pyramid disturbances. The data obtained allow to differentiate a great brain hemispheres role in descending motor control as to inhibiting and activating pyramid influences and hemisphere interactions.

[Bendotiamine efficacy in alcoholic polyneuropathy therapy].

Benfogamma efficacy in alcoholic polyneuropathy therapy with pain syndrome and other sensor disorders has been studied. Fourteen males with stage II-III chronic alcoholism (mean age 41.2 +/- 9 years, mean alcoholism duration 20.6 +/- 6 years, mean alcoholic polyneuropathy therapy duration 6.8 +/- 4.9 years) have been examined, 93% of the cases having positive family history of alcoholism. Clinical neurophysiological examination was conducted at the beginning and at the end of 6-week therapy, 450 mg/day (2 weeks) and 300 mg/day (4 weeks). During the treatment the regress of algic, other sensor and movement disorders, as well as some neuropathy symptoms has been observed. The evidence of positive dynamics at peripheral and segmental nerve system level was supported by neurophysiological data.

[Migraine: dynamics of psychophysiologic pattern].

85 patients with migraine without aura and 20 healthy individuals were examined between the attacks, before the attack, during and after it (1-2 days). The examination included clinical-neurological analysis; psychological Spilberger's and Beck's tests, scale of alexithymia and complex algesic questionnaire; recording of the conditional negative wave; determination of nociceptive flexor reflex; recording of trigeminal evoked potentials; spectral analysis of the heart's rhythms; polysomnographic study. Psychophysiologic pattern characteristic for all the patients was established. Psychological and neurophysiological parameters changed synchronously in the periods before the attack, during and after it. Before the migraineous attack a specific pathologic integration increased and presented maximally (the activity of antinociceptive system weakens anxiety increased, tension of catecholaminergic systems grew, hyperactivity in trigeninal sphere elevated, tolerance to the pain decreased). A clear tendency to the reversibility of these changes was observed after an attack. A dynamics revealed in the psychophysiological pattern could serve as a predictor of a migraineous attack and open some perspectives for prophylaxis of the algesic attack.

[Trigeminal evoked potentials in migraine].

Trigeminal evoked potentials (TEP) were studied in 19 patients with migraine without aura. Significant shortening of latent periods of early and late TEP components was observed during stimulation on the side of headache as compared with the same indices during stimulation on nonalgesic side. Hyperreactivity of trigeminal vascular system in patients with migraine without aura during the period between the attacks is discussed. Asymmetry of laten TEP periods may determine the preferential side of headache, on the one hand, and may reflect the changes in trigeminal system following the repeated lateralized fits of migraine, on the other hand. Administration of 1 g of aspirin resulted in increase (normalization) of the latentions of TEP components. After efferalgan administration TEP changes were not registered. The suggestion is made that inhibitory influence of aspirin on the truncal trigeminal system is one of the central mechanisms of its action.

[Diabetic polyneuropathy treatment by milgamma-100 preparation].

Efficiency of Milgamma-100 preparation (100 mg of benfothiamine + 100 mg of pyridoxin) was studied in treatment of diabetic polyneuropathy in 14 patients with diabetes mellitus type II (1 dragee 3 times a day, within 6 weeks). After the course of treatment the intensity of pains was decreased according to visual analogous scale on the average from 8.2 to 2.3 scores, the indices of vibratory sensitivity improved significantly as well as the data of cardiovascular tests characterizing parasympathetic control of heart rhythm. Meanwhile latent periods of the evoked sympathetic potentials on arms and legs which were initially lengthened became significantly shorter. A clear-cut tendency was also found to increasing conduction rate for excitation through the motor nerves. The treatment resulted in the improvement of the condition in 93% of the cases. The conclusion was made about efficiency and safety of Milgamma-100 preparation application in therapy of patients with diabetic polyneuropathy.

[Reflex sympathetic dystrophy].

35 patients with syndrome of reflex sympathetic dystrophy (RSD) were examined clinically by means of psychological tests, method of evoked skin sympathetic potentials (ESSP) and nociceptive flexor reflex R III. RSD syndrome had central origin (after strokes) in 17 patients (group I) while in 18 patients it arose after some peripheral damages (group II). RSD syndrome was characterised in both groups by triad of symptoms: acute pain, autonomic disorders and osteoporosis. Decreased life quality, high anxiety and depression were quite characteristic for the patients with RSD. Both significant prolongation of latent period and decrease of amplitudes of ESSP were observed on the damaged limb in both groups of patients. These damages were evidently connected with disorders of central-peripheral autonomic interactions in paretic limbs in group I, in group II they were conditioned by damages in peripheral sympathetic sudomotor fibers. Significant increase of both thresholds of pain perception and flexor reflex was found in patients with RSD while the value of the coefficient of their correlation was decreased. Such disorder might be caused by initial insufficiency of antinociceptive functions and that permitted to suggest the hypothesis about the role of this factor in pathogenesis of RSD syndrome and in development of chronic pain its leading symptom. The conclusion was drawn that development of RSD syndrome was not associated with location, severity and character of the damage, but it was conditioned mainly by the state of functions of cerebral antinociceptive systems as well as by emotional and personal peculiarities of the patients.

[Instenon in the treatment of acute disorders of the cerebral circulation].

Effectiveness of Instenon (Alkaloid Pharm. Co.), the preparation, containing hexobendin, hydroxytheophyllin, etamivan was investigated in treatment of patients with acute cerebrovascular disorders. Instenon was applied in acute phase of ischemic stroke both in form of ampules and dragee as well as for intravenous injections too. Marked positive influence of Instenon was observed on motor, sensory, autonomic and psychic functions. Instenon in form of ampules had advantage compared to dragee form in terms of faster positive alterations onset. Application of Instenon in form of ampules was more effective in patients with disorders of consciousness while usage of drug in the form of dragee was more effective in patients with cerebrovascular disorders in vertebro-basilar system. The drug hadn't significant influence on systemic arterial pressure either after single injection or long-termed treatment; the mild hypotensive effect was observed after therapeutical session. The drug was well tolerated by patients in both forms and hadn't any side effects. There were no negative consequences of Instenon interaction with other preparations. The conclusion was made about possibility of application of Instenon for the treatment of ischemic stroke.

[Caffetin in the treatment of neck pain and lumbago-sciatica].

The effectiveness of caffetin, combined analgetic drug, was estimated in treatment of acute cervicalgia and lumbar ischialgia (1 tablet 3 times daily during 10 days). Both clinical and psychological parameters were analysed. Caffetin had pronounced analgetic effect which was observed on the 2-3d day of drug application. Meanwhile there was improvement of clinical and psychological parameters, namely: relaxation of the muscles, decrease of either tension symptoms or of the levels of anxiety and depression as well as improvement of quality of life. The patients well tolerated caffetin administration without any side effects. There were no negative consequences of interaction of caffetin with other drugs (vascular, psychotropic). The conclusion was drawn about safety and effectiveness of caffetin for alleviation of acute pains in patients with myofascial, myotonic and radicular syndromes of different origin.

[The treatment of a complex regional pain syndrome].

35 patients with complex regional pain syndrome (CRPS) were treated by hyperbaric oxygenation (HBO) and caffetin preparation. A significant decrease of pain was observed in all the patients after the treatment course. Intensity of pain was diminished according to visual analogous scale. Meanwhile considerable regression of autonomic disorders and weakening of anxious and depressive manifestations was noted too. The tendency to normalization of evoked skin potentials was also found. Some elevation of the threshold of nociceptive reflex was conditioned by displacement toward general increase of antinociception after the treatment. Effect of HBO therapy persisted during 6 months in 87% of the patients. Efficiency of caffetin was restricted by the time of its administration. The conclusion was made about possibility of successful treatment of CRPS patients by both methods.

[The treatment of tension headache with sirdalud].

For the first time sirdalud was used in the treatment of 32 patients with chronic form of tension-type headache. The preparation was applied during 14 days (4 or 6 mg daily). Positive effect was observed in 75% of patients (a decrease of both headache and tension of pericranial muscles). However, sirdalud was so effective neither in patients with pronounced depression nor in ones without pericranial muscular tension. The drug had a number of psychotropic effects: it decreased anxious-depressive manifestations, normalized night sleep, significantly increased the thresholds of both ache and nociceptive flexor reflex.

[The effect of aspirin on the contingent negative wave in healthy subjects].

Alterations in parameters of slow brain potentials (contingent negative variation, CNV) were examined in healthy individuals under influence of aspirin according to double blind method. The early and late waves of CNV were analysed. The significant decrease of early wave of CNV was obtained after aspirin administration. Late wave of CNV increased after aspirin and placebo intake. The alterations described were explained in terms of central mechanisms of aspirin action including noradrenergic and dopaminergic systems involvement.

[Idiopathic orthostatic hypotension (the Bradbury-Eggleston syndrome)].

The case of idiopathic orthostatic hypotension (Bradbury-Eggleston syndrome) which gradually arose in 70-years-old women was described. The clinical pattern of syndrome included pronounced orthostatic hypotension, fixed constant rare pulse (46-56 per minutes), independent of the changes in body position together with hypohidrosis and constipation. Autonomic nervous system investigation carried out with the help of the cardiovascular tests, spectral analysis of heart rhythm as well as sympathetic skin potential revealed expressed disturbances in autonomic functions of cardiovascular and sudomotor systems. Peculiarity of the case was the development of disease on the background of drug-compensated hypothyreosis which was not however the cause of progressive autonomic disorders in patient.
