Radical hysterectomy with lymphadenectomy for treatment of early stage cervical cancer: clinical experience of 278 cases.

This study includes 278 cases of stage I and II cervical cancer subjected to radical hysterectomy and lymphadenectomy. The clinical experience of 278 cases is reviewed. Of these cases, 215 were in stage I, and 63 were in stage II. Intraoperative complications occurred at a rate of 9.7% and involved injuries to the great vessels, lower urinary tract, nerves, and rectum. Operative mortality was found to be 0.3%. Postoperative complications were observed in 20.1% of patients. Fistulae were observed in 8 (2.8%) patients. The overall survival rate was 83.8%. The 5-year survival rates for stages IA, IB, IIA, and IIB were 100.0%, 87.9%, 71.0%, and 64.0%, respectively. Pelvic lymph node metastases varied from 0% for stage IA to 40.0% for stage IIB and paraaortic involvement varied from 0% for stage IA to 50.0% for stage IIB. Radical surgery seems to be the treatment of choice for patients with early invasive cervical cancer.

Correlation between pathological risk factors and pelvic lymph node metastases in stage I squamous carcinoma of the cervix: a multivariate analysis of 194 cases.

This study includes 194 patients with stage I cervical cancer subjected to surgical therapy. Of 14 pathological factors analyzed, microscopic parametrial involvement (P = 0.001), depth of invasion (P = 0.001), and lymphovascular space invasion (P = 0.029) were found to be the most significant factors for positive pelvic lymph nodes. Combination of significant factors permitted categorization of patients into risk groups with pelvic lymph node metastases ranging from 0.0% to 90.9%.

Expression of epidermal growth factor in human tissues. Immunohistochemical and biochemical analysis.

The expression of epidermal growth factor (EGF) was examined on various human tissues by radioimmunoassay, immunohistochemistry and Northern blot analysis. Immunoreactive EGF was found in most of the human tissues by radioimmunoassay at various levels. Large quantities of EGF were detected in the kidney and thyroid gland. Immunohistochemically, EGF immunoreactivity was detected mainly in the epithelial cells of the lung, stomach, duodenum, pancreas, kidney, pituitary gland, thyroid gland, mammary gland, ovary, uterus and placenta. Weakly EGF-positive cells were also found in the adrenal gland. The results of EGF-immunostaining were not always consistent with the data from radioimmunoassay. We consider that the amount of EGF measured by radioimmunoassay reflects the density of EGF-positive cells in the tissues and the concentration of EGF in individual EGF-positive cells. Furthermore, EGF mRNA was expressed in the salivary gland, thyroid gland, mammary gland and kidney. It is thus evident that EGF is produced by a variety of human tissues. The kidney expressed exceptionally high levels of EGF mRNA which was about one-tenth of the expression in mouse submandibular gland, suggesting that most of EGF in the urine is produced and secreted by the epithelial cells of renal tubules.

Immunohistochemical detection of beta-human chorionic gonadotropin in urothelial carcinoma.

Histologic sections from the specimens of 60 patients with urothelial carcinoma were stained immunohistochemically to search for beta-human chorionic gonadotropin (B-HCG) production. There were 6 doubtful, 5 weak and 1 strong positive B-HCG stainings among 65 sections from 60 patients. De novo acquisition of B-HCG production capability of tumour cells seemed to be predictive for early haematogenous dissemination of the disease, but the data obtained in this particular study were insufficient to suggest B-HCG as a routine prognostic marker.

Relation between the birthweight and cord, maternal serum and amniotic fluid growth hormone levels.

Maternal and cord serum and amniotic fluid growth hormone (GH) levels were measured in 55 term pregnancies in order to evaluate the role of GH in prenatal fetal somatic growth. The study group consisted of patients who had small, large, and average for gestational age infants. The difference in mean maternal and cord serum and amniotic fluid GH levels between the three weight groups were found to be insignificant. It is concluded that GH does not influence fetal somatic growth.

Pregnancy and its complications after cardiac valve replacement.

The outcome of 64 pregnancies in 40 women who conceived after cardiac valve replacement were reviewed. Fetal wastage was 53.2% (25/47) in coumarin administered pregnancies, 36.4% (4/11) using heparin and 16.7% (1/6) without anticoagulants. Two fetal malformations were noted in coumarin administered pregnancies. There were two maternal deaths. Maternal morbidity due to antenatal bleeding, atrial fibrillation, thromboembolic episodes and cardiac failure were found to be 20.0% (8/40), 17.5% (7/40), 10.0% (4/40) and 10.0% (4/40), respectively.

Subsequent testicular histology: fertility and fecundity of rats subjected to unilateral incarcerated inguinal hernia during the prepubertal period.

An experimental study was designed to evaluate subsequent ipsilateral and contralateral testicular histologies, fertility, and fecundity of rats subjected to unilateral incarcerated inguinal hernia during the prepubertal period. Three study groups each containing 12 rats (28-day-old) were established. In group 1 the rats were only anesthetized. In group 2 the rats had undergone a sham operation. Left-sided incarcerated inguinal hernias were created in rats constituting group 3. After the rats had reached 4 months of age, each male rat was housed with two known fertile female rats for 3 weeks. Male rats were then castrated and the testes were evaluated histologically. Female rats were followed for 30 additional days, and the number of females impregnanted and the number of rats given birth to by each female rat were recorded. Left-sided incarcerated inguinal hernia has caused a lessened testicular/body weight ratio in the left testes, and histological deteriorations in both left and right testes that were expressed through diminished mean seminiferous tubular diameter and mean testicular biopsy scores. The fertility rate was not affected, but the number of rats produced by each male rat was lesser after having the incarcerated inguinal hernia. Unilateral incarcerated inguinal hernia during prepubertal period resulted in deteriorated ipsilateral and contralateral testicular histology and diminished the exocrine function of testes in rats.

Serum ferritin: a tumor marker for renal cell carcinoma.

Serum ferritin levels in 32 patients with renal cell carcinoma were evaluated preoperatively and postoperatively. Serum ferritin concentration was significantly higher in renal cell carcinoma patients compared to controls (259.10 versus 61.30 ng./ml., p less than 0.001). Furthermore, there was a steady and statistically significant increase in serum ferritin levels with advancing disease stage, as well as a significant decrease in serum ferritin levels after nephrectomy for stages 1 and 2 disease. The intracellular content of ferritin as estimated by polyclonal antibody was dramatically increased in renal cancer tissue compared to normal parenchyma. Although serum ferritin regulation is complex and only partly understood, the present study suggests that serum ferritin may be a useful tumor marker for renal cell carcinoma.

Transferrin receptor activity as a marker in transitional cell carcinoma of the bladder.

An immunohistochemical staining method was used to study transferrin receptors (TFR) in bladder tissue from 10 patients with benign conditions and 33 with bladder cancer. All patients with bladder cancer were followed up by cystoscopy at intervals of 3 months (mean follow-up period 11 months). The relationship between TFR activity and recurrence rates was investigated. It was not possible to demonstrate TFR in normal bladder mucosa except for the proliferating cells of the basal layer. TFR activity in malignant tissue correlated well with the histological grade and pathological stage of the tumour. Patients with low grade superficial tumours showing TFR activity had a higher recurrence rate than those with no TFR activity. It was concluded that TFR activity in low grade superficial bladder tumours is a useful marker for predicting the recurrence rate.

Borderline epithelial ovarian tumours.

Twenty-four patients with borderline epithelial ovarian tumours treated in the Department of Obstetrics and Gynaecology of Hacettepe University during the last 12 years were evaluated with regard to histopathology, therapeutic modalities employed and outcome. No mortality was encountered among the 23 patients with Stage 1 disease, regardless of the surgical mode of treatment or adjuvant therapy. The related literature was reviewed briefly to help enlighten the controversial issue of borderline ovarian tumours.

Complications and bilaterality of mature ovarian teratomas (clinicopathological evaluation of 286 cases).

Bilaterality, complications and associated pathology were reevaluated in 286 cases of mature teratoma of the ovary, treated in the last 25 years. Torsion was the most frequent complication, occurring in 22 (7.7%) patients. Rupture, infection and secondary malignant change were uncommon complications, occurring in 11 (3.8%), 7 (2.4%) and 1 (0.3%) patients, respectively. Both ovaries were evaluated histologically in 226 patients, 31 (13.7%) of whom were shown to have bilateral tumours, including 2 patients with tumour in the normally appearing ovary, contralateral to the gross tumour.

Cardiac rhabdomyoma in a newborn two-dimensional echocardiographic diagnosis.

Primary cardiac tumors are quite rare in the newborn period. Prior to surgery, cardiac catheterization and angiocardiography have been performed to confirm the two-dimensional echocardiographic findings. In this report a 2-day-old baby with the clinical impression of severe cyanotic congenital heart disease diagnosed by two-dimensional echocardiography as multiple rhabdomyoma and confirmed by surgery is presented. It is emphasized that two-dimensional echocardiography is a very useful technique for the diagnosis of primary cardiac tumors. By means of this method severely symptomatic newborns may be taken directly to surgery without prior cardiac catheterization, thus minimizing complications.

The role of lipid peroxidation in the genesis of vasospasm secondary to subarachnoid hemorrhage.

The aim of this study is to find out the possible trigger role of lipid peroxidation in vasospasm. Haemoglobin-free washed erythrocyte membranes (erythrocyte ghost) corresponding to 2.5 mg membrane protein was mixed with 0.5 umol NADPH 3 umol ADP, 4 umol ferrous sulphate and injected into the cisterna magna of cats to stimulate lipid peroxidation in vivo (Group 1). The second group was injected lml/kg whole blood and the control group 1 ml/kg saline. Angiographic studies revealed significant vasospasm in five cats in group 1. Severe vasospasm was seen in six cats in group 2. A significant increase in lipid peroxidation was observed in groups 1 and 2, compared to the control group. These results may suggest that free radical products may play an important role in the complex genesis of vasospasm in subarachnoid haemorrhage.

Complications of radical hysterectomy in women with early stage cervical cancer: clinical analysis of 270 cases.

During a 25 year period, 270 cases of stage IB and IIA cervical cancer were subjected to radical hysterectomy and lymphadenectomy. The morbidity rate was 27.6% and mortality rate was 0.3%. While injuries to the great vessels were the most common intra-operative complications, the most frequent postoperative complications were in the urinary system. The leading causes of morbidity were bladder dysfunction (16.2%), urinary infection (5.9%), lymphocyst (6.4%) and wound infections (3.5%). While the overall complications seem high, the frequency of serious morbidity has diminished and radical hysterectomy now represents one of the accepted methods of treatment of cervical carcinoma, with good 5-year survival rates.

Comparison of clinical and surgical-pathologic staging in patients with endometrial carcinoma.

In that accurate staging is essential to proper management of patients with endometrium cancer, preoperative clinical staging was compared with surgical-pathological staging in 160 patients with endometrium cancer. One hundred fifty-two patients had clinical stage I, and eight had clinical stage II disease. Overall, the clinical stage was changed in 26.9% of patients. The incidence of inaccurate staging was 25% for clinical stage I and 62.5% for clinical stage II disease. Endocervical curettage was found to have a 12.5% false-positive rate and an 8.6% false-negative rate. The extent of malignant disease for endometrium cancer (clinical stages I-II) cannot be adequately and accurately assessed with clinical staging because there is a high rate of discordance between clinical and surgical-pathologic staging.

Predictors of recurrent disease after negative second-look laparotomy for epithelial ovarian cancer.

Twenty-four patients with epithelial ovarian cancer who had negative second-look laparotomy (SLL) were assessed in regard to recurrent disease. Four patients showed recurrence 11-36 months after negative SLL. All four patients had advanced stage and grade III tumors, with two having residual tumors greater than 1 cm in diameter after initial laparotomy. We believe that patients with advanced stage and high-grade tumors should receive some kind of treatment after negative SLL.

Expression of several growth factors and their receptor genes in human colon carcinomas.

We have examined the expression of mRNAs for epidermal growth factor (EGF), transforming growth factor-alpha (TGF-alpha), EGF receptor (EGFR), PDGF-A chain (PDGFA), PDGF-B chain (PDGFB) and PDGF receptor (PDGFR) genes in seven human colorectal carcinoma cell lines and 18 human colorectal carcinomas. In surgically resected specimens of the 18 colorectal tumors, TGF-alpha, EGFR, PDGFA, PDGFB and PDGFR mRNAs were detected at various levels in 15 (83%), 9 (50%), 18 (100%), 8 (44%) and 12 (67%), respectively. They were also detected in normal tissues. Interestingly, EGF mRNA was detected in only five (28%) of the tumors, but not in normal mucosa. Expression of EGF was also confirmed immunohistochemically in tumor cells. Of the five tumors expressing EGF, four expressed EGFR mRNA and showed a tendency to invade veins and lymphatics. All the colorectal carcinoma cell lines expressed TGF-alpha mRNA, and five cell lines expressed EGFR mRNA simultaneously. Production of TGF-alpha protein by DLD-1 and CoLo320DM cells was confirmed by TGF-alpha specific monoclonal antibody binding assay. The spontaneous 3H-thymidine uptake by DLD-1 was suppressed by an anti-TGF-alpha monoclonal antibody. PDGFA and PDGFB mRNA were also expressed in four cell lines, but PDGFR and EGF mRNA was not detected. These results suggest that human colorectal carcinomas express multi-loops of growth factors and that TGF-alpha produced by tumor cells functions as an autocrine growth factor in human colonic carcinoma.

Effects of prophylactic chemotherapy for postmolar trophoblastic disease in patients with complete hydatidiform mole.

An analysis of 233 patients with complete hydatidiform mole admitted to Hacettepe University Hospital between 1964 and 1988 has been carried out. Methotrexate was administered prophylactically to 19 of 120 low-risk and to 52 of 113 high-risk patients. The difference in the incidence of postmolar gestational trophoblastic disease between prophylactically untreated and treated groups of either low-risk (13.9% versus 5.3%, P greater than 0.01) or high-risk (26.2% versus 25.0%, P greater than 0.01) patients was found to be statistically insignificant. Drug toxicity and mortality rates were 16.9% and 2.8%, respectively. It is concluded that prophylactic chemotherapy is not highly effective in the prophylaxis of postmolar gestational trophoblastic disease. Strict follow-up through sensitive betahuman chorionic gonadotropin assays should be the standard management of postmolar patients.
