A study to assess the colour stability of acetal resins subjected to thermocycling.

The purpose of this study was to compare colour stability of pink and white acetal resin with one conventional poly (methylmethacrylate) resin. Five specimen disks 50 +/- 0.1 mm in diameter and approximately 1.0 mm thick were prepared from each material. The discs were hydrated for a period of 30 days in distilled water and were then desiccated to a constant weight. Total of fifteen specimens were thermocycled for 100 and 300 hours (4000-12000 cycle) between water baths of 5-55 degrees C. Changes from baseline colour readings were recorded at each interval. Data were analysed with descriptive statistic, repeated measured analysis of variance and single factor analysis of variance. The colour changes for all materials at 100 hours were in the categories of slight, based upon the National Bureau of Standard System (NBS) for expressing colour difference. At 300 hours, the colour changes of materials were significantly different (p = 0.0001). All the materials tested showed clinically acceptable discolouration values.

The attitude of complete denture wearers towards denture adhesives in Istanbul.

The purpose of using denture adhesives is to subjectively benefit denture-wearers with improved fit and comfort of their dentures and to improve their chewing ability and confidence. The purpose of this study was to investigate the approach and thought of complete denture wearers to denture adhesives and to see why they had tried denture adhesive, and their reasons for its current use or non-use through a questionnaire. One hundred and fifteen patients attending the Marmara University, Dental School for new denture treatment were surveyed. In the survey, there were 62 females (54%) and 53 males (46%). Nearly half of these patients (53%) were in the 61-70-year age group with a mean age of 64 years. One hundred and six (92%) patients had never tried denture adhesive. Most patients (73%) who did not use denture adhesives managed their dentures well, but a significant number 101 (87%) did not know that denture adhesives existed. Nine (8%) had tried denture adhesives, and none of the patients were currently using them. Those who ceased using denture adhesive complained mainly that it did not improve the fit and chewing ability significantly. The outcome of this survey demonstrated that the knowledge of the patients is not enough on denture adhesives in Istanbul as it is still generally believed by the practitioners that the prescription of a denture adhesive is a sign of failure of the dentist.

Congenital eventration of the diaphragm in children: 25 years' experience in three pediatric surgery centers.

Eventration of the diaphragm is generally defined as an abnormal elevation of all or a portion of an attenuated but otherwise intact diaphragmatic leaf. Previous studies have indicated that eventration is a relatively rare condition, which can be symptomatic and requires surgery. We aimed to evaluate our patients with diaphragmatic eventration, and to discuss their characteristics in the light of the relevant literature. We retrospectively analyzed age, sex, incidence, location of the eventration, symptoms, associated anomalies, surgical technique, complications and survival in our patients. Between 1974 and 1999, 33 patients were treated in the Pediatric Surgery Departments of Ege University, SSK Children's Hospital and Behçet Uz Children's Hospital, 18 of them boys and 15 girls. The ages of our patients ranged from three days to 12 years. All of the patients had at least one of the respiratory symptoms such as cough, respiratory distress and fever; 3 newborns were admitted with severe respiratory distress while children belonging to higher age groups had symptoms of acute or recurrent pulmonary infections and failure to thrive. The eventration was right-sided in 22, 11 eventrations were left-sided. Diagnosis was performed with the help of a number of radiological studies such as fluoroscopic investigation, contrast study of the upper gastrointestinal system, direct X-ray graphies of the thorax, CT scan and ultrasonography, as necessary. Surgery was performed via thoracotomy in 20 patients and the 12 other patients underwent laparotomy for plication. One patient underwent thoracoabdominal plication. Two patients died because of cardio-respiratory complications in the early postoperative period and the rest of them survived to annual follow-ups. In conclusion, diaphragmatic eventration is an important condition which can eventually be mortal. Early diagnosis is necessary and plication is the treatment of choice.

Oral hygiene habits, denture cleanliness, presence of yeasts and stomatitis in elderly people.

The purpose of this study was to determine oral hygiene habits, denture cleanliness, presence of yeasts and denture stomatitis in elderly people. Seventy complete denture wearers were investigated clinically and mycologically. Subjects were evaluated according to, presence of denture stomatitis, presence of yeasts, denture cleanliness, frequency of denture brushing and denture cleaning methods. Swabs were taken from the palate investigated mycologically in order to identify the yeast colonies. No statistical relationship was found between denture stomatitis and frequency of denture brushing and denture cleaning methods. However, there was a statistically significant relationship between denture stomatitis, yeasts' presence and denture cleanliness.

Dental caries prevalence, tooth brushing and periodontal status in 150 young people in Istanbul: a pilot study.

OBJECTIVE: To evaluate periodontal conditions, prevalence and determination of dental caries and oral health behaviours in a group of young people in Istanbul. DESIGN: Subjects were orally examined and questioned. SETTING: A high school in Istanbul. SUBJECTS: One hundred of fifty students. RESULTS: This study showed that 28 per cent of teenagers had adequate oral hygiene. These findings suggest that 72 per cent of the subjects needed oral hygiene education and required restorative dental care. CONCLUSIONS: The present study showed that dental treatment need is very high in the sample group. Strong co-operation should be set up between families, schools, communities, regulators and legislators in order to develop and implement comprehensive oral health promotion programmes.

Hydatid cyst of the pancreas: a case report with 5 years' follow-up.

Primary hydatid disease of the pancreas is rare. The authors report a 3-year-old girl who had a cystic mass in the head of the pancreas with no other viscera involved. At laparatomy the entire endocyst was totally extracted from the surrounding pancreatic parenchyma. The postoperative course was uneventful. She is currently disease-free with a follow-up of 5 years. Only four other pediatric cases have been reported in the English literature.

The use of a stent-resin-bonded splint combination to stabilize a free gingival autograft.

This article describes the use of a resin-bonded retainer in combination with an acrylic resin stent to stabilize a free gingival graft. This retainer also prevents migration of muscle attachment after surgery to deepen a section of the vestibule where the oral mucosa closely approached the gingival margin. The acrylic resin stent is attached to the resin-bonded retainer by means of an integrally cast metal framework. After surgery, the retainer is attached to the occlusal two thirds of the buccal surfaces of maxillary molar teeth by using the acid-etched resin bonding technique. The stent effectively retains the graft, is simple to fit, and does not cause any permanent damage to the enamel. It has none of the disadvantages of conventional removable acrylic resin stents.

Effects of recasting on the amount of corrosion products released from two Ni-Cr base metal alloys.

The corrosion products released from two recast Ni-Cr base metal alloys Wirolloy and Wiron 99 were investigated. Cast samples were placed in Meyer's modified Fusayama solution for 2 months and in 0.1M Lactic acid 0.1M NaCl solution for 7 days. The release of Ni, Cr and Mo ions from both alloys was measured by using a flame model Atomic Absorption Spectrophotometer. A Scanning Electron Microscope was used to evaluate the surface morphology of the samples before and after corrosion tests. Release of Ni and Cr from Wirolloy samples immersed in 0.1M Lactic acid 0.1M NaCl solution were much higher than those of Wiron 99. The number of recastings was found to have negligible effect on surface texture and on the amount of corrosion products released.

Effect of dental base metal alloys on IgE levels and some blood parameters.

Despite the widespread use of nickel-based alloys, claims for safety of these alloys have not yet been accepted universally. The allergenic effects of nickel on dental patients and the potential toxic effects of nickel and beryllium on laboratory technicians continue to cause concern within the dental profession. The purpose of this study was to investigate immunoglobulin type E (IgE) values and some blood parameters of dental laboratory technicians who use dental base metal alloys. The following two groups were studied: 19 students who had been working with dental base metal alloys for two years; and 21 pre-clinical students who had never worked with dental base metal alloys. The latter group were used as a control. Blood specimens were taken from both groups and analysed using The Blood Counter. Total erythrocyte, thrombocyte, leukocyte, lymphocyte, granulocyte and monocyte counts were determined. Measurement of IgE was made with Coat-A-Count Total IgE IRMA. Blood and IgE measurements were repeated after 8 months. In the experimental group both erythrocyte and thrombocyte values were found to be statistically significantly decreased compared with the control group. No significant differences were found in lymphocyte and monocyte numbers between the initial and later measurements. There were no significance changes in IgE values for both groups. These results provide no evidence that dental base metal alloys (Ni, Cr, Be, Co) caused an increase in sensitization, during the period of the study.

Existence of Candida albicans and microorganisms in denture stomatitis patients.

The aetiology of denture stomatitis is not clear from the literature. Some studies show its aetiology as Candida albicans, while other reports point out the significance of microorganisms. In this study the existence of C. albicans and microorganisms was investigated in subjects with and without denture stomatitis. The results showed that a combination of C. albicans and microorganisms is more likely to be responsible for denture stomatitis.

Scanning electron microscopic examination of different cleaners: surface contaminant removal from dentures.

Dentures were examined by scanning electron microscopy to evaluate removal of surface contaminants. Five complete dentures were obtained during patient appointments. The palatal surface of each denture was divided into eight pieces (1 cm2) then each sample cleaned with Corega, Dentipur, Fittydent, sodium hypochloride, Savlon, Ipanol, brushing methods and one sample was also kept as a control. They were prepared for SEM examination and photographed at x500. One photograph of each sample was evaluated in random order by three judges for a total of 120 observations. Photographs were compared with one of a clean denture sample. Statistical analysis of the results showed that soaking dentures in sodium hypochloride and Savlon removed significantly more contaminants than any of the other methods used in this study.

Thyroid hormone alterations during and after cardiopulmonary bypass.

The haemodynamic effects of thyroid hormones are well known and include those on heart rate, contractility and myocardial oxygen consumption. Cardiopulmonary bypass produces various alterations in endocrine homoeostasis and may exert important haemodynamic effects postoperatively. The purpose of this prospective study was to determine the relation of cardiopulmonary bypass to changes in thyroid function. Blood samples were obtained from 20 patients preoperatively, at specific times before, during and after cardiopulmonary bypass. Total thyroxin (TT4), total triiodothyronine (TT3), free thyroxin (fT4), free triiodothyronine (fT3), thyroid-stimulating hormone (TSH), thyroid-binding globulin (TBG) and albumin were measured by radioimmunoassay and competitive immunoassay. Values of TT4, TT3 and fT3 were significantly depressed up to 24 h after cardiopulmonary bypass (P < 0.05). TSH and fT4 levels remained within normal ranges at all sampling times. It is increasingly evident that cardiopulmonary bypass affects thyroid hormone metabolism, leading to a transient depression characterized by low levels of circulating TT4, TT3 and fT3.

Gastroesophageal reflux: a determinant in the outcome of caustic esophageal burns.

Deep circumferencial burns of the esophagus always result in stricture formation and obstruction of the lumen. The usual treatment of caustic esophageal strictures is long-term esophageal dilatations. A new method of treatment, long-term stenting of the strictured esophagus gave superior results when compared with the classic dilatation therapy (healing rates, 68% v 33%; P < .01). Although success in the stent group was very satisfactory, the 32% failure rate requires explanation. In the years between 1991 and 1993, 53 stent-treated patients were screened for gastroesophageal reflux (GER). All patients were investigated with 24-hour ambulatory distal esophageal pHmetry. In 18 patients reflux index (RI) was found to be below 4. In 14 patients RI was between 4.1 and 19. In the final group of 21 patients RI was over 20 (minimum, 21.8; maximum, 72.8). When these data were compared with the healing rates of the patients, it was found that none of the 21 patients with RI over 20 responded to the described therapy. We conclude that the esophagus, after a serious caustic insult, not only narrows but also shortens thus altering the lower esophageal sphincter function leading to serious GER. Therefore all caustic esophageal burn patients should be screened for GER periodically during the dilatation or stent therapy programs, and GER should be controlled before RI approaches 20.

Unilateral inguinal hernia in girls: is routine contralateral exploration justified?

To determine the incidence of contralateral hernia development after unilateral inguinal hernia repair in girls, collected case series from two large hospitals were analyzed retrospectively. Among the 294 girls who had analyzed repair of a unilateral inguinal hernia (during a 15-year period), 245 could be traced; the mean follow-up period was 8.4 years. In 25 (10.2%) of the patients, contralateral hernia developed, mostly within one year (4 months to 6.5 years). The incidence of contralateral hernia development with respect to the original side of the inguinal hernia was significantly higher (19%) for the originally left-sided hernias than for the right-sided ones (6%) (P < .01). Although the incidence of contralateral hernia development for girls with a left inguinal hernia decreased as age increased, it was still 14.9% for the girls age 3 and up. Contralateral exploration should not be routine for girls who have a right-sided hernia, at any age. For left-sided hernias, it may be performed routinely for girls up to 2 years of age, and selectively for older patients.

Comparison of different treatment methods for localized and generalized simple denture stomatitis.

The purpose of this study was to compare the treatment effects of fluconazole, fluconazole plus chlorhexidine and the fitting of new dentures upon the treatment of denture stomatitis and 15 subjects with clinical evidence of localized simple denture stomatitis were investigated clinically and mycologically. Patients with clinical evidence of generalized simple denture stomatitis were divided into three treatment groups. The first group was given 50 mg of fluconazole tablets daily for 2 weeks. The second group were instructed to apply chlorhexidine solution to the inner surface of the denture twice a day in addition to fluconazole treatment for 2 weeks. New dentures were constructed for the third group. Patients with clinical evidence of localized simple denture stomatitis were fitted new dentures without antifungal therapy. Palatal swabs were taken from each subject before, immediately after, and 2 weeks after the end of treatment, and the samples were examined mycologically in order to identify yeast colonies. It was found that for generalized simple denture stomatitis, treatment with fluconazole plus chlorhexidine resulted in better improvement of palatal inflammation than that by either fluconazole or the fitting of new dentures without medication. Whereas fitting new dentures did not reduce inflammation in patients with generalized simple denture stomatitis, the beneficial effects of prosthetic treatment were seen in patients with localized simple denture stomatitis.
