Experimental microvascular anastomoses with a CO2 laser. A series of 240 anastomoses.

In order to compare conventional microvascular suture anastomoses with CO2 laser-assisted anastomoses, 240 anastomoses were performed in Wistar albino rats. Average time required for anastomosis was 8.30 minutes in the laser group and 14.30 minutes using the conventional technique. Patency rate was the same in both groups for arterial anastomosis but in femoral veinous anastomoses, the patency rate was of 89% using laser and 75% when done by hand. Secondary vein thromboses amounted respectively to 2% with laser and 18% with conventional sutures. Scanning electron microscope examination revealed a better endothelialization in the laser group. T.E.M. examination showed an important inflammatory reaction around the threads in the conventional suture group, even at the end of the first month. In the laser group, a partial and transient necrosis of the media appeared between the second and fourth day. Laser assisted microvascular anastomosis seems to be a promising technique with potential clinical applications.

A ten years follow-up of the results of surgery for Dupuytren's disease. A study of fifty-eight cases.

Fifty-eight patients (52 males and 6 females) operated on for Dupuytren contracture were examined by the same author with a more than ten year follow-up. At time of surgery the average was 55 years old. 69 hands (169 fingers) rated 4.33 according to the simplified Tubiana's score were treated by the same operative procedure: Mac Indoe's incision, digital Z plasty (if needed), subtotal fasciectomy and physiotherapy beginning 8 days postoperatively. At long term, recurrence appears for 49 hands (71%) one every two in the two first postoperative years, one out of five after five years. 24 of them were graded stage I. The recurrence appeared 14 times associated with an extension of the disease and the earlier, the higher was the initial stage. Some factors seem to be of a bad prognosis regarding recurrence: age (93% of recurrence under 50 years old) Ledderhose or Lapeyronie (100%) other associated diseases (Alcoholism, diabetes mellitus, epilepsy) and severe preoperative stage. Subjective results are good: 45 patients are satisfied and only 3 underwent a second operation.

[The peroperative isotopic location of osteoid osteomas and other lesions exhibiting increased uptake on scintigraphy].

The technique, advantages and pitfalls of the isotopic localization and control during operation in orthopaedic surgery were evaluated throughout 28 interventions on lesions, which exhibited an increased uptake of a radioactive bone-seeker: the dimethyl-aminodiphosphonate, provisionally designated SF44 (Laboratoires Solabco, Coutras, France). For bone scanning, following the injection of this radiopharmaceutical that increased by 25% the lesion to normal bone ratio compared to the data for the diphosphonates in current use, the localization of lesions at surgical sites was carried out with the use of a sterilisable small radiation probe of a circular cross section 2 mm in diameter (Quartz et Silice, Paris, France). The probe was connected to a portable electronic device that converted the rates of radioactive disintegration into an acoustic signal, which increased with increasing radioactivity (Novelec, Meylan, France). The method was atraumatic and has proved to be useful at various times of the operation. More often, it was used to locate the lesion on the exposed bone. It enabled monitoring the progress of excisions and allowed to shorten their dimensions. The method was of great value at the end of the intervention for ensuring a complete excision as indicated by the lack of any residual focus of increased uptake detectable on the periphery of the operative site. Isotopic control during an operation is fully efficient to locate osteoid osteomas and other circumscribed lesions of the skeleton that give rise to an increased uptake of radioactivity.

Preliminary note on biometric data relating to the human coraco-acromial arch.

A biometric study based on 20 human scapulae made it possible to specify the variations in the gap of the coraco-acromial arch in relation to its depth and height. A graphic representation in rectangular coordinates, then in spatial representation in relation to the three planes of reference, leads to the following findings: the bony variations in the arch occur essentially: at the coracoid apophysis, and two types of arch can be distinguished depending on the predominance of bony or of ligamentous components.

Histological study of a polyamide arterial allograft (inside diameter: 1.5 mm) in rats.

The histological results of the implantation of twenty aortal prosthesis (6-6 polyamide, internal diameter 1.5 mm) in rats are presented. The success rate is 77%. Biological tolerance is remarkably good. After the first month, the formation of a continuous endothelial layer is noted. At three months, the structure of the neo-membrane resembles that of the aortal.
