Accuracy of ambulatory blood pressure determination: a comparative study.

This study was designed to discriminate, according to their accuracy, between three ambulatory pressurometers (Diasys 200R, Novacor; P IV, Del Mar Avionics; SpaceLab 90202, SpaceLab). The evaluation was performed against invasive arterial reference measurements. Accuracy was assessed by calculating the error on pressure (EOP) as the difference between invasive and non-invasive measurement of arterial blood pressure. For the systolic values, accuracy (mean of EOP differences) and uncertainty (SD of these differences) were -0.9 +/- 9.7, -4.3 +/- 10.1 and -16.7 +/- 10.1 mmHg for, respectively, Diasys, PIV and SpaceLab. For diastolic values, they were, respectively, 5.9 +/- 6.7, 6.8 +/- 8.5 and 9.1 +/- 6.6 mmHg. EOP was then separated in two different types of errors: (i) the error of dispersion appreciated by the index of homogeneity calculated by a Lehmann analysis and leading to a statistical classification (ii) the error due to the drift of EOP with the reference value, this last error being easier to correct. Two different behaviours were observed for the EOP: (i) the drift of EOP of systolic values was significantly larger for the oscillometric (SpaceLab) than for the auscultatory (Diasys and P IV) method, with no difference between Diasys and P IV (ii) the homogeneity index was not statistically different among these three devices. These data suggest that, in case the correction of the drift of EOP is carried out, there is no statistical significant difference in accuracy between these three pressurometers. However, in our experimental conditions, the two ambulatory pressurometers recording the Korotkoff sounds have a better accuracy than the one using the oscillometric approach.

[Unclear foot pain: initial symptom of a malignant synovioma].

Four cases of malignant synovialoma (synonym: synovial sarcoma) of the sole of the foot are reported. A striking feature of all four cases was the long delay before the final diagnosis was made. Although this condition is rare, our cases nevertheless show that chronic neuralgiform pain close to the joint should prompt consideration of this tumor in the differential diagnosis. However, even surgical revision or explorations do not always reveal the tumor.

Accuracy of pulse oximetry during moderate exercise: a comparative study.

This study was designed to discriminate, according to their accuracy, between eight pulse oximeters (PO) using a realistic evaluation performed by means of a moderate exercise to provide artefacts and thus represent a typical clinical situation. Accuracy was assessed by calculating error on saturation (EOS) as the difference between simultaneous measurement of non-invasive (SpO2) and invasive functional (SfO2) arterial blood oxygen saturation. An index of homogeneity of EOS (InH), calculated using a Lehmann analysis, was chosen as a representative of error dispersion, leading to a statistical classification; the influence on error introduced by the drift of EOS with SfO2 was thus eliminated. We obtained a clear-cut separation of the eight PO (Datex Satlite (DA), Kontron 7840 (KO), Nellcor 200 (NE), Novametrix 505 (NO), Ohmeda 3700 (OH), PPG-Hellige (PP), Physiocontrol 1600 (PH), and Radiometer Oxi (RA] into two statistically different subgroups by comparing their InH standard deviation (SD). There is a highly statistically significant difference between the accuracy measured for the high precision subgroup with an InH SD ranging from +/- 1.6 to +/- 1.9% SaO2 (DA, PH, and RA), and that measured for the low precision subgroup whose InH SD ranges from +/- 2.8 to +/- 5.0% SaO2 (NE, OH, NO, KO, PP). These results suggest that, in our clinical conditions, there is a wide range of inhomogeneity of error from one PO to another. The error introduced by the drift of EOS with SfO2 should be considered separately.

Isolation of Legionella pneumophila from hotels of Greece.

Twenty water samples collected from 6 hotels situated in various areas of Greece were examined for the presence of Legionella pneumophila and Legionella-like organisms. Five of the six hotels included in this investigation were associated with cases of legionellosis. Legionella pneumophila serogroups 1 and 8 were isolated from four of six hotels, mainly from the hot water supply system. This is the first isolation and identification of L. pneumophila in Greece.

[The place of bone scintigraphy in the diagnosis and follow-up of Ewing's sarcoma].

The radiological and scintigraphic findings of 26 patients with histologically proven Ewing's sarcoma were analysed. Three-phase bone scan should be done early in patients presenting with pain and normal radiographs. Perfusion and metabolism of a bone lesion can be assessed by skeletal scintigraphy. Bone metastases are first seen on bone scan. In the follow-up of the patient bone scans at regular intervals are essential to detect bone metastases and tumour recurrence. The scintigraphic findings have to be correlated with radiographs and if these are negative a short-term control is indicated. Three-phase bone scans can assess the tumours response to therapy.

Inhibitory effect of deoxypyridoxine on the action of certain mitogenic factors.

We studied the effect of pyridoxine deficiency state on the responses of human lymphocytes to certain mitogenic factors. Deoxypyridoxine (DB6), a potent pyridoxine antagonist, considerably inhibited Bromodeoxyuridine (BrdU) incorporation into the newly synthesized DNA of lymphocytes cultured with Phytohemagglutinin (PHA) and Concanavalin A (Con A) in separate sets of experiments. The addition of deoxypyridoxine simultaneously with or few hours prior to initiation of cell cultures did not significantly alter the inhibitory action of deoxypyridoxine. Titration studies of deoxypyridoxine showed the direct relation between the concentration of deoxypyridoxine and the degree of DNA synthesis' inhibition with subsequent lymphocyte proliferation. The results obtained with monoclonal antibody (anti-BrdU) studies were further confirmed with tritiated thymidine incorporation (3H Thymidine) studies. Addition of pyridoxine concurrently with the addition of DB- or 8 hours later than initiation of lymphocyte incubation period was capable of effectively reversing the inhibition caused by its antagonist, the level of DNA synthesis reaching that of the control cultures where only PHA or Con A were present. Our results confirm part of previously reported findings concerning the involvement of vitamin B- in immune processes.

Clinical and epidemiological aspects of hemorrhagic fever with renal syndrome (HFRS) in Greece.

Twenty-three cases of hemorrhagic fever with renal syndrome were serologically diagnosed in rural populations of Greece between 1982 and 1985. The clinical picture of the disease was severe, (3 deaths recorded among 23 cases) and more closely resembled Korean hemorrhagic fever than nephropathia epidemica. Examination of patients' sera by indirect immunofluorescence (IFA) assays and plaque reduction neutralization (PRNT) tests using Hantaan 76-118, Puumala, and Seoul (Urban rat-associated Hantaan-like) viruses, suggested that the virus existing in Greece is either prototype Hantaan virus or a distinct virus which is closely related to Hantaan. Small mammal survey results suggest that Apodemus flavicollis may be the host of this virus. Serosurvey results of 2,653 blood samples taken from apparently healthy residents (farmers, shepherds, and woodcutters) from 22 of 54 counties in Greece, showed the percentage of seropositives to be 4%, with a range from 0 to 14%. Fifteen of 22 counties were characterized as endemic areas of the virus due to the presence of seropositive individuals. In 6 of the 15 endemic areas, overt human disease was serologically diagnosed.

[Difficulties in the diagnosis of osteoid osteoma].

The difficulties arising in diagnosing osteoidosteoma are demonstrated by means of 30 surgically treated cases and the results are compared with those available from the literature. The value of bone scintigraphy and of tomography within the framework of a diagnostic concept is underlined. Diagnosis is confirmed and treatment is effected by the successful surgical removal of the nidus.

Two cases of hemorrhagic fever with renal syndrome in northern Greece.

Two cases of hemorrhagic fever with renal syndrome were diagnosed in woodcutters presumably exposed to wild rodents and their urine in a forest in northern Greece. The disease was characterized by acute renal insufficiency without hemorrhagic manifestations. One patient required hemodialysis, but both recovered without sequelae and developed a fourfold increase in titer of antibody to Hantaan virus, as determined by an immunofluorescence test. These are the first reported cases of hemorrhagic fever with renal syndrome in Greece.

Benzodiazepine receptor interactions may be involved in the neurotoxicity of various penicillin derivatives.

The interaction of seven penicillin derivatives with specific [3H]flunitrazepam binding to benzodiazepine receptors was investigated. The affinities of the penicillins for benzodiazepine receptor seemed to depend on the lipophilia of the derivatives. The concentrations of the penicillins which inhibit specific [3H]flunitrazepam binding are consistent with penicillin levels found in the central nervous system of patients developing penicillin induced convulsions. The results suggest that penicillins inhibit GABAergic transmission not only at the GABA receptor, but also at the benzodiazepine receptor, which is thought to be part of a neuronal system facilitating GABAergic transmission. Both mechanisms may account at least in part for the convulsive not neurotoxic properties of penicillins.

Inhibition of GABA and benzodiazepine receptor binding by penicillins.

Penicillins are thought to be GABA receptor antagonists. In order to determine the affinities of various penicillin derivatives for the GABA receptor, their potencies as inhibitors of specific [3H]GABA binding to rat brain membranes were investigated. All investigated penicillins inhibit specific [3H]GABA binding, with IC50 values ranging from 2 to 60 mM. The results are consistent with the assumption that penicillins are weak GABA receptor antagonists.
