Mortality and incidence of cancer of workers in the man made vitreous fibres producing industry: an international investigation at 13 European plants.

A total of 25 146 workers at 13 plants producing man made mineral fibres (MMMF) in seven European countries (Denmark, Finland, Federal Republic of Germany, Italy, Norway, Sweden, and United Kingdom) were studied in a historical cohort investigation. At 12 of the 13 plants an environmental survey was carried out to determine present day concentrations of airborne man made mineral fibres that showed levels of respirable fibres usually below 1 f/ml and most commonly in the range 0.01 to 0.1 f/ml. Workers were entered into the cohort at the moment of their first employment at one of the 13 factories (which started to operate between 1900 and 1955), and were followed up to at least 31 December 1977. Three per cent of the workers were lost to follow up. National death rates and national cancer incidence rates, where applicable, were used for each of the seven countries for comparison with the workers' cohort. A single death from mesothelioma was reported out of a total of 309 353 person-years of observation. No consistent differences (within and between plants) were noted between observed and expected numbers concerning individual causes of death or individual cancer sites, apart from lung cancer. For this cause a tendency was observed for the standardised mortality ratio (SMR) to increase with time from first employment. When the data from all the plants were pooled a statistically significant raised SMR of 192 (17 observed, 8.9 expected; 95% confidence interval 117-307) appeared in the group with 30 years or more since first employment. The relevance of this finding, to which different factors including uncontrolled confounders such as smoking habits may have contributed, cannot be established at present. The result is suggestive, however, of an increased risk associated with the man made mineral fibres working environment of 30 or more years ago.

Lateral and anterior instability in acute ankle distortion. A radiologic investigation.

In a prospective consecutive investigation radiologic tests for lateral and anterior instability in acute ankle distortion were found to be reliable indications of lateral ligament ruptures. Arthrography of the ankle joint was used as reference. Absence of instability, however, gives little information as to the ligamental status. The size of this group constitutes a clinical problem. Better results are achieved by combining lateral and anterior instability tests. If only one test is used, the anterior instability test should be preferred. The results of the two tests bare no relation to the degree of ligament ruptures found at operation.

Arthrography of the ankle. Value in diagnosis of rupture of the calcaneofibular ligament.

As part of a clinical prospective investigations 102 patients with fresh ankle injuries underwent ankle arthrography and surgery for rupture of the lateral ligaments of the ankle. Three hypotheses concerning the improvement of the arthrographic diagnosis of rupture of the calcaneofibular ligament were tested. Two were rejected. Improvement in the diagnostic specificity was possible by combining arthrography with a stress inversion test, but the sensitivity of this combination was low. It was demonstrated that absence of peroneus sheath filling was a better diagnostic sign with good rather than with poor recess filling.

Cancer incidence in the rubber industry in Norway.

Cancer incidence was studied among 2,448 male rubber workers employed for 18 months or more with at least eight years of observation time. A suggested increased risk of bladder cancer, lymphoma, and leukemia was found in the footwear department; the number of observed to expected cases was 4/2.81, 5/2.26, and 4/1.76, respectively. An unexpected increased risk of pancreatic cancer was found for the maintenance-workshop department, the observed to expected numbers being 6/0.90.

Cancer of respiratory organs among workers at a nickel refinery in Norway.

In a previous report (Pedersen et al., 1973) an excess risk of respiratory cancer among Norwegian nickel workers was demonstrated. The number of cancer cases was relatively small for the study of temporal changes in exposure. In this report the number of cancer cases increased by 70% through an extended follow-up to 1979, and the results published previously were confirmed. Data on smoking habits of the employees at the nickel refinery have now become available. The interaction between smoking and occupational nickel exposure is assessed by comparing the excess risk of lung cancer caused by smoking in the study group and in a sample of the general population. It is shown that the interaction is closer to being additive than multiplicative. This observation has methodological implications for studying temporal changes in occupational exposure. These implications are discussed. No substantial reduction in occupational exposure at the nickel refinery can be observed as far as lung cancer is concerned. For nasal cancer, however, the risk is much smaller among those employed around 1960 than among those employed around 1930. The difference in trend of occupational risk for the two types of cancer cannot be explained by the data presented.

Risk of cancer in the Norwegian aluminium industry.

The study comprises 7,410 male employees from the Norwegian primary aluminium industry, and presents overall mortality as well as cancer incidence. During the period 1953-79, 428 new cases of cancer were observed compared to the expected number of 412.2. The only statistically significant difference for the four investigated plants combined was cancer of the lung with 57 observed against 35.9 expected cases. The increased risk was almost solely confined to two subgroups in processing departments; workers with a short duration of employment and workers with a very long duration of employment in the older plants. These results were difficult to interpret because of the problems in selecting the appropriate reference population for estimation of expected number of cases of lung cancer. Furthermore, the individual occupational histories were incomplete and there was no information on smoking habits. In order to determine whether there is an excess risk for workers with long periods of employment in the newer plants, it is necessary to wait for another 10-20 years, when more conclusive data will become available.

One year's treatment with propranolol after myocardial infarction: preliminary report of Norwegian multicentre trial.

A prospective, randomised, double-blind study was performed to compare the effects of propranolol and placebo on sudden cardiac death in a high-risk group of patients who survived acute myocardial infarction. Altogether 4929 patients with definite acute myocardial infarction were screened for inclusion: 574 (11.6%) died before randomisation, and 3795 (77%) were excluded. Five hundred and sixty patients aged 35 to 70 years were stratified into two risk groups and randomly assigned treatment with propranolol 40 mg four times a day or placebo. Treatment started four to six days after the infarction. By one year there had been 11 sudden deaths in the propranolol group and 23 in the placebo group (p less than 0.038, two-tailed test analysed according to the "intention-to-treat" principle). Altogether there were 25 deaths in the propranolol group and 37 in the placebo group (P less than 0.12), with 16 and 21 non-fatal reinfarctions respectively. A quarter of the patients were withdrawn from each group. Withdrawal because of heart failure during the first two weeks of treatment was significantly more common among propranolol-treated patients than among the controls, but thereafter the withdrawal rate was the same. The significant reduction in sudden death was comparable with that after alprenolol, practolol, and timolol, which suggests that the mechanism of prevention is beta-blockade rather than any other pharmacological property of the individual drugs.

Ruptures of the lateral ligaments of the ankle. Clinical diagnosis.

During the 11-month period 1 October 1977 to 31 August 1978 a total of 44 patients with acute supination trauma of the ankle were examined. The clinical findings were compared with the results of arthrography. This revealed that direct and indirect tenderness of the anterior talofibular ligament and calcaneofibular ligament respectively, combined with a greater than or equal to 4 cm swelling anteriorly and over the lateral malleolus, indicated a ligament injury with great likelihood. If some of the diagnostic signs are absent, most emphasis should be laid on the swelling over the lateral malleolus and on the direct and indirect tenderness of the calcaneofibular ligament. The talar-tilt test and examination for the drawer sign were of limited diagnostic value.

Cadmium and zinc in kidneys from Danish cattle.

This paper describes an investigation on the content of cadmium and zinc in Danish cattles kidneys in relation to animal age and place of rearing. The investigation included kidneys from 176 animals sampled in 1979-81 from 26 slaughterhouses (fig. 1). The investigation shows a statistically significant relationship between kidney cadmium content and age (table I and fig. 2). An age difference of 7 years leads to a 9 fold increase in cadmium concentration. The effect of place of rearing is less marked. The content is highest in kidneys from animals bred in the South-Eastern parts of Denmark and is gradually decreasing when going in a North-Western direction (fig. 3). This geographic pattern parallels earlier Danish studies on atmospheric deposition and of cadmium content in lichens. The zinc concentration shows no relationship with neither age nor place of rearing, and no correlation could be established between renal cadmium and zinc content (fig. 4). Based on the PTWI of 400-500 micrograms it is suggested, that only kidneys from Danish cattles less than two years of age should be used for human consumption. In West Germany a proposal has been made for a maximum tolerable cadmium content of 1 mg/kg kidney for human consumption.

Rupture of the lateral ligaments of the ankle: operation or plaster cast? A propective study.

The material comprises 444 patients, aged 12-50 years, with acute sprain of the ankle, seen over an 11-month period (1.10. 1977 to 31.8. 1978). Clinical examination showed no rupture of the ligaments in 53. Arthrography in the remaining patients revealed rupture of the lateral ligaments in 209 (in two-thirds of the anterior talofibular ligament and in one-third of the anterior talofibular as well as calcaneofibular ligament). Conservative treatment with a below-knee plaster cast for 5 weeks was employed in 107 patients, while 102 were treated surgically by suture of the ruptured ligament(s) and subsequently wore a below-knee plaster cast for 5 weeks. The follow-up 1 year after the accident was attended by 63 per cent of the patients. Good results were found in 76 per cent of those treated by plaster cast only and in 81 per cent of those treated by surgery. The difference is not statistically significant.

Clinical testing of Conray meglumine and Amipaque in cerebral angiography.

Amipaque 280 mg I/ml and Conray meglumine 282 mg I/ml have been compared in a double-bind trial in cerebral angiography. Altogether 55 cerebral artery branches have been examined in 21 patients, who served as their own controls. The effect of the contrast injections on the intra-arterial blood pressure, pulse rate, ECG, circulatory rate and the patients' reactions were recorded under standardized conditions. The quality of the films was also evaluated. No significant difference in the cardiovascular effects of the two contrast media was found. There was a significant difference in favour of Amipaque in the discomfort of the patients--less pain and sensation of warmth. No serious side effects were observed.
