Human fascioliasis in Cajamarca/Peru. I. Diagnostic methods and treatment with praziquantel.

In a prospective study 34 Peruvian fascioliasis patients were followed up for three months after treatment with praziquantel. The rapid sedimentation of faeces was demonstrated to be the most appropriate parasitological technique for the diagnosis of Fasciola hepatica infections in man compared to the MIF concentration or the examination of duodenal fluid by Enterotest. After therapy with different doses of praziquantel only 21% of the patients stopped to excrete worm eggs although 17 patients had been treated repeatedly. During the period of observation all patients presented a decreasing number of symptoms. It is concluded that praziquantel is not very active against F. hepatica in man.

Taurine- and penicillin-induced epileptic activity.

The present study has been performed to investigate the effect of i.v. administration of taurine on the electrical activity of the epileptogenic focus induced by penicillin applied to the right sensory motor cortex of adult rats. Taurine (100 mg/kg body weight) was administered 15, 30, 60, and 120 min before the application of penicillin. The EEG was unipolarly recorded by means of electrodes applied to the pia. Taurine caused a decrease of the frequency as well as the spike amplitude of epileptic discharge. The spread of epileptogenic foci to the opposite hemisphere was retarded when compared to that of control animals. The maximal antiepileptic effect of taurine was observed when the amino acid was administered 30-60 min previous to penicillin. It is suggested that high concentrations of taurine in the brain might be necessary to inhibit the epileptic activity.

Right ventricular infarction: relationships between ST segment elevation in V4R and hemodynamic, scintigraphic, and echocardiographic findings in patients with acute inferior myocardial infarction.

The relationship between ST segment elevation on the right precordial lead V4R and the hemodynamic, echocardiographic, and myocardial scintigrapic signs suggestive of right ventricular (RV) infarction was studied in 42 patients with acute inferior myocardial infarction. Twenty-two patients had ST segment elevation in V4R. Among these patients, a significant correlation was demonstrated between V4R ST segment elevation and the hemodynamic (p less than 0.001), scintigraphic (p less than 0.001), and echographic (p less than 0.02) criteria for acute RV infarction. These results support the validity of this new electrocardiographic sign as a practical means in aiding the clinical detection of RV involvement with acute transmural inferior myocardial infarction.

Transcatheter embolization of brain arteriovenous malformations and meningiomas. Evoked responses correlations.

Transcatheter embolization of selective arteries of the carotid and vertebral systems were performed in several patients with brain arteriovenous malformations, angiomas and chemodectomas. All patients were submitted to angiography and electrophysiological examination consisting of a conventional EEG and of visual, auditory and somatosensory evoked responses before and after embolization. Patients with tumors showed a marked decrease of blood flow and tumoral necrosis after embolization, facilitating subsequent surgical procedures. Most patients showed an electrophysiological improvement after embolization, in those regions in which angiography revealed brain decompression resulting from the size reduction of the abnormal tissue. Transcatheter embolization can be very useful in the treatment of brain arteriovenous malformations and intracranial tumors, often facilitating surgical procedures.

Evoked responses after head trauma.

A neurological examination, routine EEG and a battery of evoked responses (i. e. to a flash, to a checkerboard pattern, to three different tones and to the electrical stimulation of the median nerves) were performed in 14 patients within 24 hours, after one month, six months and a year after head trauma. The results have shown a significant correlation between the number of low R values in the first study and both severeness of the trauma and the clinical evolution a year later. ERs appeared to be - unlike the native EEG - a powerful measure for recommending prophylactic treatment in those traumas which are generally considered to produce slight brain damage, but may provoke a post-traumatic epilepsy and/or great memory deficits. This correlation also shows that bad prognosis may be anticipated in cases with widespread ERs' abnormalities.

A simplified cardiac cannulation technique.

A technique for venous cannulation, left heart venting, and aortic root cannulation is described. It is simple and tidy and permits full mobility of the cannula with minimal manipulation and no blood loss.

Visual, auditory and somatosensory evoked responses in patients with cerebrovascular disorders.

A diagnostic evoked response battery, consisting of visual evoked responses to flashes and to a checkerboard pattern, auditory evoked responses to three different tones and somatosensory evoked responses to left, right and bilateral median nerve stimulation, was administered to 20 patients with strokes, and compared with 20 normal control subjects. Recordings were obtained at C3, C4, O1, O2, T3 and T4. Evoked responses were found to be of lower amplitudes and lower amplitudes and lower waveform symmetry (expressed by low correlation coefficients between left and right homologous areas) in the patients than in normal subjects. A discriminant equation with 17 variates was computed to separate the two groups. One patient (5%) and 2 (10%) normal subjects were misclassified. Comparison between patients with left or right lesions showed that somatosensory and auditory evoked responses were very useful for identification of the affected side. Quantitative study of evoked responses to different types of stimuli and sensory modalities provides a sensitive method for the evaluation of brain function in such patients. This technique gives higher accuracy than the routine EEG examination in terms of both the percentage of dectection and the accuracy of localization of the affected side.

Treatment of Kienböck's disease using a silicone rubber implant.

Ten patients with Kienböck's disease treated by resection and replacement with a silicone rubber implant through a volar approach were reviewed after follow-up ranging from twenty-four to thirty months. The results were good in seven and unsatisfactory in three: two patients had volar dislocation of the implant and one, median-nerve paresthesias of unexplained origin.

Fruiting organs of Cladosporium werneckii.

Submerged mycelia of a strain of Cladosporium werneckii isolated from tinea nigra palmaris, when cultured on enriched corn-meal agar media, developed fruiting bodies resembling perithecia.
