Do objective estimates of chances for survival influence decisions to withhold or withdraw treatment? The French Multicentric Group of ICU Research.

The authors studied the impact on clinical decision making of providing feedback of objective prognostic information describing the probability of survival for ICU patients with multiple organ system failure (OSF). The prognostic estimates, derived from a control period (1), were to be provided on a daily basis to physicians providing treatment in 25 French ICUs during a subsequent experimental period (2). The types of, frequencies of, and reasons for decisions to limit or stop treatment in the two periods were compared. In the experimental period 2, 17 ICUs participated in the feedback study. Within these 17 units, there was a small but significant (p less than 0.05) increase in decisions to stop active treatment and provide comfort care that was limited to patients with three or more OSFs. There was no change in decision making in the eight units that did not participate in the feedback study. Although these results suggest a direct causal relationship between the provision of objective prognostic data and changes in physician decision making, the small increase in comfort care decisions (n = 14) between period 1 and period 2 and the fact that only 17 of the 25 original units participated in the feedback study make it difficult to eliminate other influences. There was no indication in this study, however, that explicit provision of prognostic data led to a sense of therapeutic futility.

[The value of mammary thermography. A report on 655 cases (author's transl)].

A total of 655 patients were investigated by both mammography and contact thermography, and 63 cancers of the breast were detected. Thermography was slightly more sensitive (0.82) than mammography (0.75). They were of equal specificity (0.96 and 0.97). A critical analysis of the experimental conditions, however, leads the authors to propose thermography as the essential investigation completing clinical examination and mammography. It may also have a certain prognostic value, but this remains to be demonstrated.

[Assessment of diagnostic procedures. Some comments about classical indices (author's transl)].

The assessment of diagnostic technologies, concerns today both clinicians and health care administrators. This assessment is based upon calculation of various indices measuring the informative value of diagnostic test: sensitivity, specificity, accuracy, predictive value. Despite their extreme semplicity, these indices can be misinterpreted and misused; in addition they are often calculated from very particular patients sample, especially when retrospective data are used. The main points which influence the value confered to a diagnostic test, the major bias to avoid designing such studies are pointed out this paper. The likeness between controlled therapeutic trial and comparative assessment of diagnostic technologies is particularly emphasized.

[Study methods of drugs in Alzheimer disease].

The availability of new drugs for Alzheimer's disease, with different pharmacological profiles, leads to a redefinition the relevant methodology for developing drugs in this indication, including the inclusion/exclusion criteria, the duration of the studies, and therefore, the relevant guidelines. This was the purpose of the Giens Round-table devoted to the new methodology for drug development in Alzheimer disease.
