The authors evaluated the possibility that the time of the administration of erythropoietin (rHuEPO) may play any effect on the therapeutic outcome of the treatment. The study was carried out on 26 uremic subjects submitted to hemodialysis in the morning or in the evening. At the end of the dialytic treatment every patient was administered rHuEPO IV. The amount of erythropoietin needed to reach an Ht value of 32% was evaluated. The subjects whose rHuEPO had been administered in the first evening hours showed that they needed a less quantity of rHuEPO (885 +/- 50 vs 1550 +/- 75 IU/Kg; p < 0.01), and showed a more rapid improvement of the anemia. According to the lans of a physiologic circadian rhythm, the evening i.v. administration of rHuEPO seems to insure a quicker therapeutic success.
It's reported a case of progressive and rapid deterioration of the renal function, following to a wrong administration of high doses of recombinant erythropoietin. After the administration of 10,000 U of rHuEPO three times a week for at least five times instead of the 1000 U ordered, the patient showed a progressive increase of the plasmatic creatinine values from 2.2 to 3.3 mg/dl. In the same period we pointed out an increase of hematocrit (from 29% to 35%) and of the Hb (from 9.3 to 11.2 g/dl). The patient, suffering from a slightly high blood pressure, furthermore showed important pressure increases. The suspension of the treatment by erythropoietin permitted the restoration of the creatinine values to levels similar to the ones pointed out before the treatment and a reduction of the blood pressure values. The AA. think that a too rapid regression of the anaemia achieved by a rHuEPO treatment can condition a worsening of the renal function in the pre uraemic patient.
The serum concentrations of circulating ICAM-1 (cICAM-1) and soluble receptors for interleukin-2 (sIL-2R) were evaluated on 48 patients with B-cell chronic lymphocytic leukaemia (B-CLL) and on 15 healthy control subjects. The mean +/- SD concentration of cICAM-1 was significantly higher (p < 0.002) in B-CLL patients (407.7 +/- 164.3 ng/ml) than in healthy controls (245.4 +/- 76.7 ng/ml). Patients with progressive disease had higher cICAM-1 levels than patients with "indolent" disease (440.38 +/- 32.3 ng/ml versus 321.36 +/- 14.45 ng/ml; p < 0.0001). Serum levels of cICAM-1 were also significantly higher (p < 0.0002) in patients with advanced stage (III-IV) than in those with early stage (I-II). The increase of cICAM-1 levels was positively correlated to increases of soluble receptors for interleukin-2 (r = 0.9; p < 0.0001). These results seem to show that the measurement of serum levels of cICAM-1 may be an useful tool for monitoring disease activity and tumoral mass in patients with B-CLL. However, further studies are needed to define the functional role of high cICAM-1 levels in the immunological dysregulation of patients with malignancy.
The effect of administration of human recombinant erythropoietin on leukocytic cells has been evaluated. In humans the administration of 20,000 U i.v. of rHuEPO caused a rapid and transitory leukopenia that seemed to be caused by a preferential reduction of lymphocytes (2715 +/- 513 mm3 t 0; 1898 +/- 506 t 60', p < 0.002). Pretreatment with imidazole-2-hydroxybenzoate, a drug capable of inhibiting the synthesis of prostaglandins, caused a less marked and later reduction of lymphocytes (2242 +/- 477 t 0; 1560 +/- 318 t 180 minutes, p < 0.001). A less marked and later lymphocytopenia was also shown in subjects affected by chronic renal insufficiency (2907 +/- 726 t 0; 1944 +/- 512 t 180 minutes, p < 0.005).
