The authors, by means of a recently introduced method, evaluated the intraplatelet concentrations of magnesium in 45 normotensive patients with type II diabetes mellitus, in 45 hypertensive diabetics and in 15 healthy controls. They also evaluated plasma and erythrocyte concentrations of the cation through direct current plasma spectrometer. Both normotensive and hypertensive diabetics showed a reduction in plasma, erythrocyte, and platelet concentrations of magnesium compared to controls. On the contrary, no significant difference was found between hypertensive and normotensive diabetics with regard to plasma and erythrocyte magnesium, whereas intraplatelet assay of the ion pointed out significantly lower concentrations of magnesium in hypertensive compared to normotensive patients (56.4 +/- 9.0 vs 60.7 +/- 10.2 micrograms/10(8) cells--p < 0.05). The authors believe that intraplatelet assay of magnesium may be the most reliable method for the evaluation of the cation in hypertensive diabetics, probably because platelets share common features with smooth muscle cells, including the alpha-2-adrenoceptor cyclase system and a coupling mechanism concerning the calcium-dependent contraction.

The study about the adhesion molecules pointed out their pathogenetic role in a lot of diseases within cardiological, nephrological, oncological precincts and in a lot of infectious diseases. Their role was in fact pointed out in the ischemic heart disease and in the reperfusion damage, in the glomerulonephritis, in the tumoural metastatic processes, in the infectious and inflammatory chronic processes. The Authors considered the recent literature and emphasized the therapeutic possibilities that such an experimental approach seems to suggest.
By means of high sensitivity phonometers, noise levels in different sites of an intensive care unit (ICU) for nephrology and dialysis were evaluated before and after acoustical renovation of the rooms. The noise levels registered in the diurnal hours of the working day were always higher than those recommended for sites devoted to patient care. The authors analysed the negative effects of noise pollution on nephrology patients and the medical personnel of the unit.
Trigger points in chronic pain and myofascial syndromes are often localized at a distance from the painful area. Infiltration of trigger points with Bupivacaine 0.5% greatly reduces pain and can eliminate it with no need of any other analgesic therapy.
We evaluated the effects of oral magnesium supplementation on plasma lipid concentrations in patients with non-insulin-dependent diabetes mellitus. Twenty-six persons with non-insulin-dependent diabetes mellitus received 4.5 g magnesium pidolate/d for one month, and 17 persons received placebo. Before and at the end of the treatment period cholesterol concentration, triglycerides, LDL cholesterol, HDL cholesterol, and plasma and erythrocyte magnesium were evaluated. Chronic magnesium supplementation produced a significant reduction of plasma cholesterol and LDL cholesterol, and an increase of HDL cholesterol. These results suggest that oral supplementation of magnesium may be useful in the treatment of hyperlipidaemia in patients with non-insulin-dependent diabetes mellitus.
UNLABELLED: The authors evaluated the ability of rHuEPO (recombinant human erythropoietin) in performing a vasoactive calcium-mediated action. Forty healthy volunteers were evaluated for the muscle blood flow at rest (RBF) and after reactive hyperemia by Xenon 133 clearance. Pre-treatments with several calcium antagonists were used. RESULTS: The i.v. infusion of rHuEPO brought about a reduction of the RBF and MBF (maximal blood flow). This reduction was maintained with nifedipine or verapamil. Gallopamil and ketanserine prevented the reduction of blood flow induced by rHuEPO. CONCLUSIONS. The rHuEPO has a vasoconstrictive action in vivo since gallopamil and ketanserine, drugs able to prevent intracellular mobilisation of the Ca++, can block the rHuEPO induced vasoconstriction. The action exerted by rHuEPO could be due not an increased flow of the Ca++ from the extracellular department but to a mobilisation from Ca++ deposits.
We assayed 2',5 oligoadenylate synthetase (2'5-AS) activity in the serum of eight patients with B-cell chronic lymphocytic leukemia (B-CLL) who were undergoing therapy with alpha interferon (IFN). Mean pretreatment levels of 2'5-AS were normal, but upon treatment the levels rose to higher than normal values. Moreover, the degree of enzyme induction in serum was significantly higher in CLL responders (P < 0.003) whereas no difference was found in non-responders. The lack of serum 2'5-AS induction in non-responder patients might be related to unresponsiveness to treatment with IFN. These results seem to show that the measurement of serum levels of 2'5-AS activity may be a useful tool in monitoring IFN treatment.
OBJECTIVE: There have been several studies that have already explored the potential beneficial role of cyclo-oxygenase (CO) inhibitors on oleic acid (OA)-induced lung injury in different species. These studies report no significant effect of CO inhibition, though thromboxane B2 (TxB2) was effectively blocked. However, recent studies indicate that pre-treatment with aspirin (ASA) preserve gas exchange in OA lung injury in dogs. Aim of our study has been to evaluate the potential beneficial effects of the pre-treatment with low doses of ASA on gas exchange, hemodynamics, respiratory mechanics, prostanoids and lung histology in OA-induced lung injury in sheep. DESIGN: 0.09 ml/kg of OA was administered into the right atrium of 14 anaesthetized sheep. Six received a bolus of ASA (10 mg/kg i.v.) 30 min before OA, the others saline as placebo. MEASUREMENTS AND RESULTS: Pulmonary and tissue gas exchange, pulmonary and systematic hemodynamics, respiratory system mechanics, TxB2 and 6-keto-PGF1 alpha, leukocytes and platelets concentrations were measured throughout the subsequent 3 h and lung histology was effected at end-experiment. The principal findings of our study are: 1) ASA reduces OA-induced early pulmonary vasoconstriction and bronchoconstriction, parallelled by a suppression of TxB2 generation; 2) the late increase in pulmonary artery pressure and airway resistance due to OA is not inhibited by ASA; 3) the early disturbance in pulmonary gas exchange is reduced by ASA, whereas the late severe deterioration is exaggerated by ASA; 4) the stability of tissue exchange ratio (R) at approximately 1 in ASA-group compared to its fall to approximately 0.7 in controls. CONCLUSION: Our findings suggest that ASA: 1) is only effective to treat the very transient TxB2-induced pulmonary vasoconstriction resulting in hydrostatic edema, and it is ineffective, even accentuates, the subsequent major pulmonary endothelial cell injury leading to alveolar flooding that is unrelated to TxB2; 2) has a transient protective effect on the TxB2-induced early bronchospasm; 3) has a biphasic behaviour on gas exchange, with a benefit which lasts only one hour and then results in a worse gas exchange; 4) has an immediate, stabilizing, persisting effect on R, contrasting with its transient effect on pulmonary hemodynamics and PaO2.

The effects of recombinant human erythropoietin (rHuEPO) on survival and blood pressure in hypovolemic hemorrhagic shock in rats have been studied by intravenous administration. The hormone caused an increase in the time of survival compared to the group of rats without treatment (102 +/- 4.5 vs. 25 +/- 4.5 min, p < 0.01, vehicle; 102 +/- 4.5 vs. 25 +/- 2 min, p < 0.01, saline solution) and an increase in the percentage of the surviving animals (5/7 after 120 min vs. 0/7, p < 0.05). The animals treated showed an increase in mean arterial pressure that was not present in the control group. Through their data, the authors suggest the vasopressor effect of erythropoietin. Furthermore, from other studies done before, they hypothesize that the vasoactive ability of the hormone is endothelin mediated.
The authors examined the literature on the close relationship of magnesium to carbohydrate metabolism. It was found that a diabetic condition is apt to bring about marked changes in body magnesium content and its compartmental distribution, and that the administration of dietary magnesium supplements can have a favorable influence on metabolic control, thus reducing insulin requirement and counteracting the progression of late diabetic complications.
