Serum bilirubin subfractions in patients with alcohol abuse during detoxication.

Sera were obtained from 41 alcohol abusers consecutively admitted for detoxication. Blood samples were withdrawn on the second, fourth and seventh days of abstention. Initial bilirubin values were moderately elevated in 10 patients. Determination of the bilirubin subfractions by high performance liquid chromatography showed elevated values of unconjugated (alpha), monoconjugated (beta), diconjugated (gamma) and albumin-bound (delta) bilirubin, in 8, 15, 4 and 15 patients, respectively. During abstention, the total bilirubin value normalized rapidly, only three patients still had values above the upper reference limit after 7 days. In patients with initially elevated values of bilirubin subfractions, only a few had elevated beta and gamma levels on the seventh day, whereas delta levels decreased at a slower rate and remained virtually unchanged. On admission, 27 patients exhibited elevated levels of serum bile acids; these values decreased during abstention and after 7 days only six patients had slightly elevated values. Only five patients had initially elevated levels of alkaline phosphatase (ALP). These became normalized in all but two patients during abstention. The results suggest that mild cholestasis is common among alcohol-abusers without clinically evident liver disease and that these changes are reversible on abstention.

Relation of structure to function for the U. S. reference standard endotoxin after exposure to 60Co radiation.

The structure and function of the highly purified U.S. reference standard endotoxin (RSE) were studied after exposure to ionizing radiation from a 60Co source. With increasing doses of radiation, the trilaminar ribbon-like structure of untreated endotoxin exhibited focal swelling, after which only spherical particles were seen by electron microscopy. These morphological changes were paralleled by the respective loss of O-side chain repeating units and pieces of the R-core from the lipopolysaccharide molecules, as demonstrated by electrophoresis. The biologic function of the irradiated endotoxin was assessed with a variety of tests. At higher doses of radiation, a direct relation was observed between the degradation of the molecular and supramolecular structure and the loss of biologic function. At lower doses of radiation, however, there was variability among the functional assays in their rate of change with progressive irradiation of the RSE. The results suggest that the carbohydrate moiety plays an important role both in determining the supramolecular structure and in modulating certain biologic activities of bacterial endotoxins.

The female orgasm: pelvic contractions.

Eleven nulliparous women manually self-stimulated to orgasm, each on three separate occasions. Pelvic contraction pressure was measured by an anal probe and a vaginal probe simultaneously. Near the perceived start of orgasm, a series of regular contractions began in nine of the women. Anal and vaginal contraction waveforms were synchronized with each other, and the same number of orgasmic contractions occurred in each lumen. Anal pressure had a higher resting baseline and greater amplitude contractions than vaginal pressure. The perceived start of orgasm did not correspond precisely with the onset of regular contractions. Mean intercontraction intervals increased linearly at an increment of about 0.1 second through the series of regular contractions. Amplitudes of contraction pressure waveforms, representing pelvic muscular force, were initially low, increased through the first half of the regular series, and then decreased. Area and net area of the pressure waveforms, reflecting pelvic muscular exertion (force x time), increased during the regular orgasmic contractions. Three of the women's orgasms consistently included only a series of regular contractions (orgasm type I). For six other women, orgasms consistently continued beyond the regular series with additional irregular contractions (orgasm type II). Types I and II had been identified previously in men. Two women had no regular contractions during reported orgasms. This pattern, type IV, had not been recorded in men. Women of different types showed marked differences in orgasm duration and number of contractions. Identification of these types in subjects is important for meaningful comparison of contraction parameters in different studies.

Self-esteem patterns distinctive of groups of drug abusing and other dysfunctional adolescents.

Self-report measures on seven scales representing four broad processes of self-esteem were used to contrast three groups of drug abusers (pretreatment, in-treatment, and posttreatment), two other groups of institutionalized dysfunctional youth, and a "normal" public school sample, all from a population of White middle-class suburban adolescents. Multivariate analysis of covariance with adjustment for age and sex was used to compare the dysfunctional groups with the normal group. Discrimination analysis portrayed the dimensions of self-esteem separating all the groups. Pretreatment and in-treatment drug abusers had significantly lower scores on all seven measures of self-esteem; posttreatment abusers did not show these decrements, but reported greater ease in sharing feelings than normals did. The learning disabled adoles reported a lower sense of well being and greater ease in sharing feelings. These outcomes suggest that low self-esteem is not a general consequence of personal dysfunction per se, and may play a causal role in drug abuse.

Mesor-hypertension: hints by chronobiologists.

Circadian systems are intermodulated by networks of specialized neural, hormonal and cellular functions, with time structures that are interdependent. In cardiovascular pathophysiology, circadian and ultradian rhythms of clinical interest have been demonstrated. Cardiac output, heart rate, arterial pressure and blood volume are the best known. Systolic and diastolic blood pressure and heart rate have circadian patterns in health and therefore arterial pressure cannot be evaluated by a single measurement during a 24-h span. With correct monitoring for at least 48-h it is possible to detect the mesor-hypertension and the possible amplitude-hypertension that precedes the mesor-hypertension. Prolonged elevation of blood pressure can cause irreparable harm to sensitive tissues. To quantify the damage, the concept of hyperbaric impact has been introduced. This is a measure of the excess load exerted upon the arterial walls. Studies of the beta-blocker penbutolol with correct automatic monitoring have shown the persistence of the physiological circadian variation in the cardiovascular parameters during penbutolol administration. The so-called elimination of the circadian rhythm in blood pressure, which would not really be desirable, was not seen in any of our patients, whose cardiovascular parameters were monitored continuously, day and night, while taking penbutolol. The amplitudes of the rhythms were always prominent. A phase shift, a delay of about 100 degrees, was demonstrated in the heart rate of one 63-year-old mesor-normotensive woman.

Circadian systolic and diastolic hyperbaric indices of high school and college students.

Apparently healthy North American (mostly Minnesotan) students, 117 boys and 147 girls, 14-21 years of age, self-measured blood pressure at 1 h or longer intervals for spans ranging from 24 h over several weeks to much longer spans. Each series was analyzed for a circadian rhythm by the single cosinor fit of a 24-h cosine curve. The mesor, amplitude and acrophase thus obtained were used for the determination of a mean acrometron, i.e. the mean of the sum of mesor + amplitude for each individual investigated. A circadian hyperbaric impact (HBI) was then computed as the tension-time product represented by the area under the fitted cosine curve and above the acrometron as a critical value. A highly skewed distribution of these HBIs was found: the HBI for systolic and diastolic blood pressure equalled 0 in 64 and 49 of the series on boys and 78 and 72 of the series on girls, respectively. Individuals singled out as outliers by the HBI are recommended for traking, some with and some without eventual intervention. The overall HBI is a step toward assessing the values that may lie above some group reference value, assuming a fixed rather than a likely-changing threshold for the damaging effect and further assuming the linearity of any impact, if not damage, along the scales of pressure and time. The overall HBI, based on such unvalidated approximations, will have to be complemented by an index related to the changes that lie outside a chronodesm with focus both upon the pressures that are too low as well as upon those that are too high.
