CT-guided extrapleural drainage of bronchogenic cyst.

A patient with a subcarinal bronchogenic cyst is described. The cyst was aspirated using an extrapleural percutaneous approach under CT guidance, employing repeated injections of 4-5 ml of saline to split the parietal pleura from the chest wall. Thus we describe a technique that allows the insertion of a catheter into a cyst and complete evacuation of its contents.

Purification and characterization of plasma T-kininogen from Wistar and brown Norway rats.

A rapid and convenient three-step purification scheme has been developed for the purification of T-kininogen (alpha 1-cysteine proteinase inhibitor) from rat plasma. The purification process includes chromatography on hydroxyapatite, immunoaffinity chromatography and gel filtration. This procedure is applied to plasma from the brown Norway rat which is known to be deficient in high and low molecular weight kininogens. The method furnished large amounts of T-kininogen from turpentine-treated Wistar rats as well as from untreated and turpentine-treated deficient brown Norway rats. The amino acid and hexose content of the three T-kininogens has been determined. While the composition of the molecules isolated from both injured rats was similar, the neutral sugar content of T-kininogen purified from untreated brown Norway rats was lower and its amino acid composition showed slight differences. The three molecules have identical behaviour and similar physicochemical and immunological properties when analysed by SDS electrophoresis, isoelectrofocusing and two-dimensional immunoelectrophoresis.

[Descriptive epidemiology of the vascular risk factors in Bañeres. Study group 'Bañeres Project'].

INTRODUCTION: Epidemiological studies of the risk factors of cerebrovascular disease are of great interest, particularly the identification of factors which may be modified. Previous studies carried out in the Alcoi region of Alicante province, showed a high prevalence of cerebrovascular disease. The town of Bañeres was therefore chosen for confirmation of this data and identification of the frequency of vascular risk factors. OBJECTIVE: To compare the group of patients with cerebrovascular disease with the remainder of the population interviewed. PATIENTS AND METHODS: In a door-to-door study in Bañeres 1,832 people were interviewed as part of the Bañeres Project. The population aged over 45 years was interviewed and filled in a questionnaire for diagnosis of transient ischemic accidents. RESULTS: Arterial hypertension: estimated prevalence 500/1,000 inhabitants, relative risk 3.24; diabetes mellitus: estimated prevalence 195/1,000, relative risk 2.18; coronary artery disease: estimated prevalence 58/1,000, relative risk 1.88; peptic ulcer: estimated prevalence 75/1,000, relative risk 1.23; smoking: estimated prevalence 110/1,000, relative risk 0.46; complete arrhythmia: prevalence 73/1,000, relative risk 5.23. Family histories of cerebrovascular accident, arterial hypertension, diabetes and coronary artery disease were not significant. CONCLUSIONS: Arterial hypertension, diabetes mellitus and arrhythmia were significantly more prevalent amongst patients with vascular disease in our setting. We found no association with the other risk factors analyzed.

Rod monochromatism -- an incomplete form.

An incomplete form of rod monochromatism is described in a young man with normal visual acuity and absence of nystagmus or photophobia. ERG showed normal threshold sensitivity in white and blue lights but virtual absence of the first portion of response in red light. The sensory threshold curve lacked the typical rod-cone discontinuity, whereas the curve after full dark adaptation was normal. It is suggested that the condition is due to atypical disfunction of the cones.

Nitric oxide--angiotensin II axis in renal and cardiovascular injury.

The knowledge and the information about Renin Angiotensin System and Arginine-Nitric Oxide pathway helped us in understanding more about cardiac, vascular and renal systems, their function, the physiology, and pathophysiology of different diseases that may affect them. The existence of a Nitric Oxide/Angiotensin II axis, brought modification to our ways of thinking with new and more pathophysiological approaches to the treatment of hypertension and end-organ disease. There appears to be a link between hypertension, salt sensitivity, endothelial function and NO-AnglI axis on one side and end organ damage on the other side. Anti-hypertensive agents that can restore endothelial function beside their antihypertensive effect, may be better in preventing or at least slowing end-organ damage.

[Emotional expressiveness and development of schizophrenia: a reply to the work of Vaughn].

The influence of family climate in the course of schizophrenia has been empirically established. However research has been so far limited to the Anglo-American culture and there are reasons to think that straight forward extrapolation to own our culture may not be totally justified. Consecqently a replication of Vaughn's (Vaughn et al, 1984) has been carried out with a sample of 60 schizophrenic patients in Valencia. Relapse rate was not predicted by EE status or by any EE scale-score according to British original criteria. As the cut-off point was changed from six or more critical remarks to four or more, then the relapse rate was significantly different between high and low EE groups. The study confirms the link between EE-status and relapse rates over nine months but is also underlines the need to take into account cultural differences when it comes to establish cut-off scores.
