Total aortic arch replacement by a cryopreserved aortic homograft.

The case is presented of a 60-year-old man with previous surgery on the transverse aorta and a pseudoaneurysm of the aortic arch and hemoptysis. The patient was operated upon with cardiopulmonary bypass and cerebral perfusion. The aortic arch was resected preserving a cuff of the aorta containing the three supra-aortic trunks. A bronchial fistula was closed with a pediculated flap of the native arch. A long segment of cryopreserved human aorta (ascending, arch and upper descending thoracic aorta) was inserted in the orthotopic position. The aortic cuff with the supra-aortic vessels was re-inserted onto the allograft. The patient is asymptomatic 2 months after the operation. Aortic homografts are a good alternative in cases with suspected infection or contamination, like this one. Other conditions, such as infected aneurysms, infected grafts, aorto-esophageal fistula and aorto-gastrointestinal fistula, are potentially good candidates for homografting. We believe that this case is the first report of a complete substitution of the aortic arch with a human cryopreserved homograft.

Role of dietary lipids and butylated hydroxy anisol (BHA) administration on albino rats. Part I. Serum unsaturated fatty acids.

Adult male albino rats were supplemented with edible plant oils, corn maize and cotton seed oils and their semihydrogenated forms (Heliopolis and Sultan cooking fats) for a period of 3 months. In general, the supplementation of tested oils produced increasing effect on the serum levels of total C18:1, C18:2 and C20:4 unsaturated fatty acids (UFA). The increases were significant in case of feeding corn maize oil and Sultan cooking fat for total and C18:1 and C18:2 UFA. However, the degree of significance was intensified by the feeding of cotton seed oil and Heliopolis cooking fat. Supplementation of Sultan cooking fat showed a significant increase in serum level of C20:4, while the other tested oils and Heliopolis cooking fat produced very highly significant increase. Butylated hydroxy anisol (BHA) produced no effect on the level of UFA when administered with dietary tested edible lipid but its role revealed by maintaining the higher level of UFA due to administration of oils and its semihydrogenated analogous.

MDL 27,531 selectively reverses strychnine-induced seizures in mice.

1. Strychnine-sensitive glycine receptors are primarily localized in the brainstem and spinal cord where they are the major mediators of postsynaptic inhibition. A compound which acts functionally like a glycine receptor agonist would be potentially useful as a pharmacological tool and as a therapeutic agent for treating disorders of glycinergic transmission. 2. MDL 27,531 (4-methyl-3-methylsulphonyl-5-phenyl-4H-1,2,4-triazole) blocked strychnine-induced tonic extensor seizures in mice following either intraperitoneal (ED50 = 12.8 mg kg-1; 30 min) or oral (ED50 = 7.3 mg kg-1; 30 min) administration. Time course studies revealed a maximal effect at 30-60 min, though significant activity was still seen after 24 h. 3. MDL 27,531 was selective in antagonizing strychnine seizures and little or no activity was seen against seizures produced by other chemical convulsants (bicuculline; quinolinic acid; mercaptopropionic acid); by electrical stimuli (maximal electroshock); or by sensory stimuli (audiogenic seizure susceptible mice). MDL 27,531 blocked pentylenetetrazol-induced seizures with an ED50 = 55 mg kg-1. This profile differed from those of the anticonvulsants diazepam, valproic acid, and diphenylhydantoin. 4. The antagonism of strychnine seizures by MDL 27,531 occurred at doses that did not produce signs of sedation (suppression of spontaneous motor activity), motor ataxia (disruption of rotarod performance), muscle relaxation (inhibition of morphine-induced Straub tail), or CNS depression (potentiation of hexobarbitone sleep time). MDL 27,531 had less side effect potential (as derived from ratios obtained from the above measures) relative to those of the known muscle relaxants diazepam and baclofen. 5. Although MDL 27,531 behaved functionally like a selective agonist at the strychnine-sensitive glycine receptor, the compound did not alter the in vitro binding of [3H]-strychnine to mice brainstem/spinal cord membranes at concentrations of up to 100 microM. In further in vitro binding assays, MDL 27,531 at concentrations of up to 100 microM, did not displace the binding of [3H]-muscimol, [3H]-flunitrazepam, or["S]-t-butylbicyclophosphorthionate to rat cortical membranes. These ligands bind to the 7y-aminobutyric acid (GABA), benzodiazepine, and picrotoxin-convulsant binding sites, respectively.6. MDL 27,531 (10-100mgkg-', i.p.) enhanced binding of the benzodiazepine antagonist [3H]-Ro15-1788 to mouse cerebral cortex in vivo without directly affecting GABA levels.7. Ro 15-1788 (16, 32 mg kg-') significantly blocked the MDL 27,531 antagonism of strychnineinduced seizures, though this antagonism was not competitive. The same doses of Ro 15-1788 produced parallel rightward shifts in the dose-response curves for diazepam inhibition of pentylenetetrazol-induced seizures, consistent with a competitive antagonism.8. Thus, MDL 27,531 acts functionally like an agonist at the strychnine-sensitive glycine receptor in its capacity to reverse selectively strychnine-induced seizures. Though the precise mechanism of action of MDL 27,531 is unknown, MDL 27,531 may act at an allosteric site on the strychnine-sensitive receptor which produces agonist-like activity.

Pharmacodynamics and biophasic drug levels of methionine enkephalin.

Methionine enkephalin (Met-E) is a naturally occurring pentapeptide. It appears to mediate pain perception by blocking CNS pathways. Using rabbits, a log dose relationship was obtained between Met-E-induced dilation of the pupil (mydriasis) and constant intravenous infusion dose rates (14, 20, 28, 37, 56, 73, and 129 micrograms/min/kg). Steady-state dilation is reached within 9 min. Dose-effect curves (DEC) were fitted by a linear regression analysis of the log dose versus percentage dilation plots. Fitted DEC were used to determine temporal profiles for the relative biophasic drug level.

[Late intramedullary metastases of a medulloblastoma with primary extramedullary seeded metastases--magnetic resonance tomographic detection using gadolinium-DTPA].

Dissemination of tumour cells via CSF occurs if there is close proximity of a malignant space-occupying growth to the CSF circulation system. According to the anatomic localisation of the metastases a distinction is made between intra- and extra medullary metastases. Magnetic resonance tomography is a highly sensitive method that can point not only to the pathological process, but also to its localisation and extent, especially if paramagnetic substances are used, to a much more accurate degree than had been possible so far with invasive myelography or computed tomography.

Inhibition of tumor growth in a mouse fibrosarcoma after interleukin 2 application.

Highly purified natural IL-2, obtained from induced human PBL's, was used to treat tumor-bearing mice. 3 X 10(6) vital cells of chemically induced fibrosarcoma BALB/c female Meth A were transplanted subcutaneously to female mice. Repeated injections of 10,000 U IL-2 into established tumors of 7-8 mm diameter led to complete rejections in 75% of the treated animals. Single intratumoral injections and other routes of the application of IL-2 did not significantly influence the tumor growth rate.

Congenital hypothyroidism: diagnosis, treatment, and prognosis.

Congenital hypothyroidism is a common but preventable cause of mental retardation. The incidence of congenital hypothyroidism in the newborn population is about 1:3500 to 1:4000. Infants with Down's syndrome are at a special risk. Fort et al. reported an incidence of persistent primary congenital hypothyroidism in infants with Down's syndrome of 1:141 or 28 times higher than the general newborn population. Premature infants have varying degrees of immaturity of hypothalamic-pituitary-thyroid system and are at a special risk for a variety of thyroid disorders. These patients need adequate understanding and interpretation of their laboratory values before institution of replacement hormonal therapy. Screening programs are available in 48 states and have led to prompt diagnosis and recognition of most cases that would otherwise be missed. Prevention of mental retardation depends on early and adequate treatment. This requires close cooperation between the program and the physicians involved in the care of the infant. Many state screening programs maintain a list of experts who are available for consultation, if necessary.
