[Peripheral autonomic insufficiency in the Guillain-Barré syndrome].

Overall 11 patients with acute inflammatory demyelinating polyradiculoneuropathy or Guillain-Barre syndrome were examined for the status of the peripheral part of the autonomic nervous system. Methods used in the study permit estimating the status of vegetative fibers in different systems: in the cardiovascular system, in the pupil, and in sweat glands. Part of the patients were examined over time for all the vegetative and electroneuromyographic characteristics. Based on the data obtained the conclusion is made about a slow recovery of the function of vegetative fibers in the cardiovascular system. It is emphasized that the degree of vegetative fibers injury and, therefore, the gravity of the clinical manifestations and the course of vegetative polyneuropathy are determined in many respects by the degree of axon impairment.

[The place of laparoscopy in the diagnosis and treatment of ovarian cysts].

Laparoscopy as a diagnostic and therapeutic tool was tried in 56 patients. Laparoscopy was indicated in ovarian cysts detected by ultrasound, sterility, suspected external endometriosis. The diagnosis at laparoscopy coincided with histological pattern of the removed neoplasms in 100% in relation to malignancy. Endoscopic interventions had complications in 1.8% of cases. Recurrences were not recorded 18 months.

[Relaparotomies following the creation of intestinal anastomoses in newborn infants].

An analysis of the immediate postoperative period in 295 newborns after putting intestinal anastomoses was made. Postoperative surgical complications were found in 38 children who required 47 relaparotomies. Surgical tactics was determined for the complications of the postoperative period as well as the indications for relaparotomy.

[The use of adreno- and gangliolytics for preventing hemodynamic disorders in patients in the immediate preoperative period].

The indices of central and peripheral hemodynamics in 40 patients with the generally accepted scheme of premedication and in 40 patients who were given adrenogangliolytics for medicamentous preparing before narcosis have been analyzed. The inclusion of these drugs in the premedication preparing was established to ensure a sufficiently complete prevention of an excessive preoperative stress reaction.

[Diagnostic possibilities of laparoscopy in ovarian cysts].

Laparoscopy was used for the diagnosis of ovarian cysts in 90 patients. Ovarian cysts detected by ultrasonic examination, sterility, suspected external genital endometriosis were the indications for laparoscopy. In 32 cases the cysts were found in laparoscopy. In 23 patients a diagnostic puncture of the cyst and a subsequent cytologic examination were carried out. All the patients in whom ovarian cysts were diagnosed were operated on (endoscopically or via laparoscopy). Laparoscopic diagnoses coincided with the histologic findings in examinations of the removed formations in 100% of cases.

[Antibacterial and therapeutic effectiveness of a pectin from sea grass Zostera].

The kinetics of the antibacterial activity of zosterin, a polysaccharide preparation of a sea grass belonging to Zoster, was studied. By its chemical structure zosterin is a low ++methoxylated pectin. In vitro the preparation markedly inhibited the growth of ++Gram-negative and ++Gram-positive organisms: S. aureus, E. coli, Y. pseudotuberculosis, S. typhimurium and Ps. aeruginosa. On a model of experimental pseudotuberculosis++ infection caused by oral contamination of mice F1 (CBA X C57B1) with a suspension of Y. pseudotuberculosis zosterin was shown to have a therapeutic effect. It protected 30 to 40 per cent of the animals when administered per os simultaneously with or 24 hours after the contamination. The results are in favour of the zosterin further investigation as a preparation useful in prevention of intestinal infections in persons being in contact with the patients.

[Adrenoganglioplegia as a method of correction of hemodynamic disorders in surgical patients with initial hypotension].

The possibility to use adreno- and ganglionic blockaders in the operative period in surgical patients with initial hypotension is considered. The examinations performed in 37 patients have shown that addition of adreno- and ganglionic blockaders to routine correcting therapy results in earlier (after premedication) and more economical (at the expense of elevation of the initial low peripheral vascular resistance) arrest of the initial hypotonia.

[The determination of the optimal inoculation dose of an oral cholera chemical bivalent vaccine in a controlled experiment].

276 volunteers aged 19 years and over were placed under observation in the course of the trial of oral cholera vaccine in tablets, containing choleragen toxoid, O-antigens of serovars Inaba and Ogawa and a number of Vibrio cholerae exoenzymes, for safety, reactogenic properties and immunological effectiveness. The vaccine was found to produce no reactions in a dose of 1-4 tablets; the administration of 3 tablets (300,000 binding units of the toxoid and 10,000 units of O-antigens, serovars Inaba and Ogawa) was shown to induce the most intensive synthesis of both antitoxins and vibriocidal antibodies in the blood sera of volunteers, as well as IgA coproantibodies. The oral vaccine was found to have an advantage over parenteral vaccines due to the absence of reactogenic properties and the formation of local immunity: coproantibodies appeared in 80% and 9% of the vaccinees respectively.

Ultrastructural changes in the size of mast cell granules and in the granular matrix during an experimental anaphylactic shock.

A staining method for the identification and the functional characteristics of mast cells in semithin section is described. Transmission electron microscopy in the conditions of an experimental anaphylactic shock (EASh) was used to measure microplanimetrically the longest diameters of the non-secreting (NG) and the secreting (SG) granules. Determined was also the percent ratio between these 2 types of granules of the mast cells. Under normal conditions (control animals) 96.2% of the granules were NG, and nearly half of them (48.7%) had a diameter (mast cell granule diameter--McGD) shorter than 1.0 micron (1,000 nm). In the conditions of a lethal EASh 87.7% of the granules were SG, and most of them (54.4%) had McGD longer than 1.5 micron (1,500 nm). In EASh a considerable part of NG (6.2%) also had McGD longer than 1.5 micron (1,500 nm). In mast cell degranulation the electron dense or fine lamellar structure of the granular matrix was often disturbed. Tiny, oval, and randomly dispersed particles appeared. It was admitted that if the areas among the particles of a size lower than 0.2 micron (200 nm) were prevailing the granular matrix disorganization (GMD) was of type (SG1); if most of the areas were larger than 0.2 micron (200 nm) GMD was of type II (SG2) and preceded the complete fusion of the granules.

[Intestinal obstruction in children with cystoid formations in the mesentery and omentum].

The work presents clinical characteristics of 14 children with cystoid formations of the mesenterium and omentum accompanied by the most frequent complication--intestinal obstruction which was observed in 14 children. The main cause of the mesenterial cysts was a torsion of the intestine with the cyst, that of the omentum damage was a compression of the intestinal loops. They should be treated surgically. Lethal outcomes did not follow.

[Results of treatment of newborn infants with diaphragmatic hernias].

The article generalizes an experience with the treatment of 91 newborns with diaphragmatic hernias for the period from 1955 to 1985. The diagnostics has improved and greater amount of newborns were admitted to the clinic within the first day after birth. The frequency and character of postoperative complications were analyzed as well as causes of their appearance and effective measures for their prevention and structure of lethality. The tactics of resuscitation both before and after operation has been changed: the artificial ventilation of the lungs was performed without a positive pressure at the end of the expiration with the use of Tubarin and infusion of Dopamine. Lethality was 2 times reduced.

[Use of hypothermic protection of the body during surgical correction of congenital heart defects].

The article presents an experience with using the method of general moderate hypothermia (28-30 degrees C) in 260 patients operated for congenital heart failures aged from 3 to 45. The duration of the isolation of the heart from blood circulation was from 3 till 25 minutes. The significance of preoperative topical diagnosis, perfect surgical technique and adequate narcosis is emphasized.
