[Plasmapheresis in combined treatment of patients with acute intestinal obstruction].

Comparative analysis of the results of treatment of patients with acute intestinal obstruction was carried out. Plasmapheresis was used in postoperative period in 38 patients. 32 operated patients were treated with conventional methods of detoxication. It was established that after the earliest performance of plasmapheresis in postoperative period, manifestations of toxemia revived more rapidly and significantly and intestinal motility restored more rapidly.

[Functional activity of placental, splenic and hepatic tissues in pyo-septic diseases under conditions of extracorporeal detoxication].

Immunocorrective and detoxication treatment by plasma perfusion through fragments of alloplacental tissue (group 1), fragments of xenosplenic tissue (group 2) and isolated xenohepatocytes (group 3) was carried out as an adjuvant therapy in 61 patients with pyoseptic diseases. Control group consisted of 21 patients. Comparative analysis of effectiveness of the treatment showed, that in patients of group 2 after 5 days substantial increase of lymphocytes count was detected due to increased pool of T- and B-lymphocytes. In patients of group 1 increase in lymphocytes content occurred even in greater degree, but it occurred mainly owing to the augmentation of B-lymphocytes pool. In patients of groups 1 and 2 already to the 3rd day an increase of FAL as well IgA and IgM occurred. There was no immunocorrective effect in patients of group 3, as well as control group. However, detoxication activity, having been evaluated by bilirubin and cytolytic enzymes dynamics, by urea and kreatinine level increased in all patients of groups 1, 2, 3 in comparison with the control. It is suggested, that allogenic placental tissue could be recommended for the treatment of pyoseptic conditions, especially in patients with transplanted organs, because this tissue electively activates only humoral immunity.

[Temporary perfusion substitution of liver and spleen functions in patients with peritonitis].

Thirty-one patients with generalized purulent peritonitis of various etiology were examined; a new perfusion method with the use of isolated live hepatocytes and xenospleen fragments was applied in the intensive therapy complex in the early period after a cavitary operation in 16 patients. It is shown that the suggested method has significant advantages over the traditional methods in that it causes effective detoxification, improves microcirculation, and reduces the indicators of the cytologic syndrome. Application of the hybrid system of detoxification in patients with peritonitis made it possible to reduce the lethality among the most serious patients with infectious-toxic shock and polyorganic insufficiency.

[Experience in using preserved xenogeneic hepatocytes in the combined treatment of jaundice patients].

The authors give brief characteristics of the apparatus "subsidiary liver" and describe the mechanism of its work. The method of taking the liver of piglets and isolation of hepatocytes are described. The characteristics of patients are given in who the apparatus "subsidiary liver" was used. Certain parameters of blood were shown to be reduced after using the apparatus "subsidiary liver".

Assisted circulation in cardiac and respiratory insufficiency.

Intra-aortic counterpulsation with a balloon pump (IABP) was used in 63 patients with cardiogenic shock, heart failure after surgery with artificial circulation, internal lacerations of the myocardium, or unstable angina. It was found that the IABP was most efficient in conditions related to myocardial ischemia. Use of arteriovenous perfusion (in 11 patients) or artificial ventricles is indicated in severe perturbations of the pumping function of the heart. As experience has shown, cardiologic and heart surgery patients usually have multiorgan insufficiency. For this reason, it is necessary to use other artificial organs in complex treatment: artificial lungs, kidneys, livers, etc.

[Sterilization of the artificial blood circulation apparatus with ozone].

Presented in this paper is the sterilization method of the assisted circulation unit. The method has been approved by stand testing and then applied for sterilizing the unit Ae, yeK-5 m when experimental implanting artificial heart. According to the method, ozone in concentration of 5 x 10(-3) moles/1 and with flow rate of 0,5 1/min is passed through an internal circuit of the unit during I h and 15 min. Control wash-out samples from lines have demonstrated complete sterility of the internal circuit. The method makes the sterilization process of such units much easier and may be recommended for clinical application.

[Comparative evaluation of membrane, bubble and foam-film oxygenators in artificial circulation during heart surgery].

Single-use membraneous oxygenators "Teflo" (TMO) and bubble oxygenator "Variflow" (VF-1) issued by "Travenol" firm were used on 30 patients in open-heart operations with extracorporal circulation. A comparative assessment of working, technical and functional performances of these devices against the foam-membraneous oxygenator (FMO) of "AIK-5m" apparatus proved the reliability, safety, feasibility and high effectiveness of the single-use oxygenating systems, membraneous oxygenator in particular, which is the most promising one for the use in extracorporal circulation.

[Elaboration of the structure and principles of construction of a unified system for the apparatus for artificial circulation].

A pertinet analysis and assessment of medico-technical and ergonomic characteristics, both of individual extracorporeal circulation units (ECU) and major functional components (pumps, oxygenators, etc) served as a background to suggesting a principle for establishing a unitized ECU system based on the functional (performance) and spatial (packaged) module.

[Surgical cryo-ultrasonic instrument].

With the YPCK-7H unit, serving as a base, a cryo-ultrasonic instrument for a combined use of cryogenic and ultrasonic methods has been constructed. A pipe for the passage of liquid nitrogen is mounted on an ultrasonic waveguide, this securing the cooling of the soft tissue subjected to the operation down to--135--80degreesC and separation of the tissues with the frequency of ultrasonic oscillations within a range of 20 to 45 kHz. The surgical cryoultrasonic instrument is apt to significantly increase the efficacy of hemostasis and helps achieve a still better neatness of the cut, this obviously being due to the cryogenic factor which adds advantage to the ultrasonic operative intervention.
