Clinical trial of diclofenac sodium (Naclof) eye drops on Nigerians.

Twenty-eight eyes of 26 age-matched patients who had planned extracapsular cataract extraction with or without intraocular lens implantation were enrolled into a double blind randomised actively controlled study of 2 groups. Each group of 14 eyes was assigned to receive 0.1% diclofenac sodium (Naclof) eye drops or 1% prednisolone acetate eye suspension. The patients received either 0.1% Diclofenac sodium eye drops or 1.0% prednisolone acetate eye suspension four times a day as their post operative anti-inflammatory medication for a period of four weeks. No significant difference was noticed in the subjective assessment of pain and conjunctival injection in the 28 days follow-up period except day 1 in the diclofenac sodium group (0.05> p >0.01). Other measured objective variables of inflammation such as anterior chamber cells and flare showed no significant difference from the 3rd-28th postoperative days (0.05< p > 0.20). The result demonstrated that 0.1% diclofenac sodium eye drops was as effective as 1% prednisolone acetate eye suspension in the control of postoperative inflammation after cataract surgery, and could serve as a viable alternative to topical steroids in Nigerians who are steroid responders.

Ecology of Mooren's ulcer in Nigeria.

Five cases of Mooren's ulcer in young Nigerians seen over a 2-year period, in Lagos, healed after limbal conjunctivectomy. The maximum follow up in 2 cases was 2 years. Previous trauma or corneal foreign bodies as seen in four of our cases seem to suggest the precipitation of Mooren's ulcer by various ocular insults. We believe that collagenolytic activity of the conjunctiva plays a prominent role in the pathogenesis of the ulcer and is supported by this report in which our five cases healed after excision of the limbal conjunctiva. Helminthosis, found in four of our five patients with Mooren's ulcer in an endemic area, such as Nigeria may be an associated finding. The presence of lymphocytes and plasma cells in the histopathology of the excised tissue in one of our cases is suggestive of an autoimmune phenomenon in this disease.

Changes in albumoid protein during cataract formation.

The water-insoluble albumoid protein of the human cataractous lenses was extracted in Tris/glycine buffer (pH 8.2). Subsequent homogenization in 8 M urea containing 1% beta-mercaptoethanol gave three major fractions: urea soluble fraction (USF), urea beta-mercaptoethanol-soluble fraction (U-beta-merc-IF). and urea beta-mercaptoethanol-soluble fraction (U-beta-merc-SF). The urea beta-mercaptoethanol-soluble fraction (U-beta-merc-SF) constitutes the pigmented protein of the human cataractous lenses. The amino-acid composition indicates the presence of tryptophan, high concentration of cysteine and low concentration of lysine and methionine. The brown protein is an aggregate of protein subunits which may be a protein chromophore, that directs all possible adaptive phenomenon during vision.

Bilateral congenital anophthalmos. Report of two cases.

Two cases of bilateral congenital anophthalmos in Nigerian children are reported. Both cases are sporadic, and belong to the primary type of anophthalmos. The first case has associated polydactyly of both hands. There is no history of ingestion of any teratogenic drugs during pregnancy in the two cases.
