[Spread of multidrug resistance of Mycobacterium and its impact on the efficiency of treatment in patients with tuberculosis in the penitentiaries of the Arkhangelsk Region].

The paper deals with drug resistance of Mycobacterium tuberculosis in the prisons of the Arkhangelsk Region. The present study was undertaken to examine the spread of multidrug resistance of Mycobacterium tuberculosis and its impact on the outcomes of treatment of new cases of tuberculosis in the penitentiaries of the Arkhangelsk Region.

[The significance of investigation of the left ventricle diastolic function in cardiosurgery].

An examination of 79 patients (main group) with congenital, acquired heart diseases and ischemic heart disease (IHD) without activity of the rheumatoid process and infectious endocarditis and 64 subjects of the same age has shown that in order to improve the assessment of the results of operative treatment of patients with the acquired heart disease and IHD it is necessary to test the diastolic function of the left ventricle (LV) in addition to the systole parameters. Cardiosurgical correction of pathological processes either causing or aggravating LV diastolic dysfunction (overloading LV with the volume or pressure, myocardium ischemia) can either eliminate or reduce its degree. A pathological monophasic filling of LV is a sign of a pronounced systole-diastolic dysfunction of LV and lung hypertension. A radical operative intervention reestablishes the biphasic filling of LV and reduces pressure in the pulmonary artery.

[The influence of hereditary thrombophilic mechanisms on the degree of permanent intravascular coagulation in patients with artificial heart valves].

The genotyping of 40 patients with artificial heart valves (AHV) was performed after prosthesis of the mitral and aotic valves with bicuspid AHV (Medinzh-2 and CarboMedics). The patients took phenylin and varfarin. The patients' genotype was estimated by the thrombophylic genes: factor V Leiden (FVL), prothrombin G20210A, methylene tetrahydrofolate reductase C677T, G/A--455FGB, 4G/5G PAI-1, PI A1/A2 GPIIIa. The genes determining the thrombocytic activity or the vascular wall state substantially influence the third degree of the intensity of the permanent intravascular coagulation (PIC-3) independent of the degree of correction of hemostasis of oral anticoagulants. The addition of anti-aggregants to therapy is the only that can normalize functional activity of thrombocytes in patients with AHV having such defects. The laboratory detection of the genetic defects is of great practical importance for the determination of risk groups of formation of PIC-3 and the strategy of antithrombotic protection of patients with AHV.

[Relationship between various clinical parameters and duration and variability of Q-T and R-R intervals in ECG (an epidemiological population study)].

AIM: To study the relationship between clinical values of a standard epidemiological study of a male population and the length and variability of Q-T and R-R intervals. MATERIAL AND METHODS: A representative sampling from a male population (2184 subjects aged 25-64 years) in Novosibirsk was examined using standard epidemiological methods. Later 251 men were excluded from the study because of impossibility of correct analysis of ECG. The length and dispersion of Q-T interval and time parameters of variability of R-R interval were evaluated. RESULTS: The size and variability of Q-T and R-R intervals are associated with age and AP irrespective of other clinical parameters of a simultaneous epidemiological study. The length of R-R interval is independently related to body weight index, tobacco smoking, and plasma triglyceride level. R-R variability parameters are associated with coronary disease irrespective of the risk factors. CONCLUSION: Scanning studies with complex analysis of Q-T and R-R interval duration and variability effectively detect subjects at a high risk of arrhythmias and fatal events.

[Comparative analysis of parameters of corrected and uncorrected QT-interval dispersion of magneto-, electrocardiography and isomagnetic maps in patients with ischemic heart disease].

AIM: To compare possibilities of magnetocardiography (MCG) and electrocardiography for assessment of regional dispersion of ventricular recovery time using parameters of corrected and uncorrected QT-interval dispersion (DQTs and DQT). MATERIAL: Twenty three patients with class II angina pectoris including 11 patients with history of myocardial infarction (MI) and 13 practically healthy subjects. RESULTS: Mean DQT and DQTc were significantly higher (p<0.005) in patients than in healthy subjects according to both techniques. Values of DQT and DQTc obtained by MCG were higher in patients with history of MI compared with those without MI (p=0.006 and 0.02, respectively). There was a significant positive correlation between age and DQT and DQTc determined by electrocardiography. Mean number of T-wave dipoles was significantly higher in patients than in healthy subjects. Substantial positive correlation was found between number of T-wave dipoles on isomagnetic maps and age in both patients and healthy people. CONCLUSION: The method of MCG gave supplementary information on the state of ventricular depolarization in patients with ischemic heart disease.

[Prognostic value of duration and variability of QT and RR intervals in general population of Novosibirsk].

Increased duration and dispersion of QT interval and lowered variability of RR interval are considered to be probable precursors of dangerous ventricular arrhythmias and sudden death. Standard epidemiological methods were used for the study of a representative sample of nonorganized male population (age 25-64 years) in Novosibirsk in a framework of the MONICA project. Database of the total mortality register was used for verification of deaths. Resting ECGs were analyzed using the Minnesota code criteria blindly relative to other data of the study. Duration and dispersion of QT and QTc intervals as well as temporal parameters of RR variability were determined manually. Assessment of significance of these factors for prognosis was made with the use of methods of survival analysis. Among all characteristics of QTc interval maximal duration and dispersion of QTc were the strongest predictors of death from any cause and cardiovascular death being independent of age, mean blood pressure, ischemic heart disease, body mass index and smoking. Predictive power of these QTc interval parameters was similar in total male population and in subpopulation of patients with cardiovascular diseases. In men without cardiovascular diseases parameters of RR variability on ECG at rest were predictors of cardiovascular death independent of age, mean blood pressure, body mass index, total cholesterol and smoking.

Influence of nonuniform magnetic fields on orientation of plant seedlings in microgravity conditions.

Experiments on the spatial behavior of the flax (Linum usitatissimum, L.) seedlings in a nonuniform magnetic field were conducted on the orbital space stations "Salut" and "Mir". This field can displace sensory organelles (statoliths) inside receptor cells and such displacement should cause a physiological reaction of the plant-tropistic curvature. Experiments were conducted in the custom-built "Magnetogravistat" facility, where seeds were germinated and grown for 3-4 days in a magnetic field with the dynamic factor grad (H2/2) approximately equal to 10(7) Oe2/cm, then fixed on orbit and returned to Earth for analysis. It was found, that 93% of the seedlings were oriented in the field consistently with curvature in response to displacement of statoliths along the field gradient by ponderomotive magnetic forces, while control seedlings grew in the direction of the initial orientation of the seed. This suggests, that gravity receptors of plants recognized magnetic forces on statoliths as gravity, and that gravity stimulus can be substituted for plants by a force of a different physical nature.

[Effects of the dual chamber electrocardiostimulation on the systolic and diastolic function of the myocardium].

An assessment of the indicators of the systolic and diastolic functions of the left ventricle was made in 52 patients against the background of electrostimulation with a dual chamber electro-cardiostimulator with different atrioventricular delays (185, 125 and 79 ms). Different variants of the reactions to electrostimulation with a shorter atrioventricular delay were detected: the positive dynamics of the diastolic indicators was registered in the hypertrophic type of the diastolic dysfunction, aggravation of the diastolic disorders--in the restrictive type.

[Intracardiac hemodynamics after closure of atrial septal defects with the endovascular device, Amplatzer septal occluder].

Changed size and forms of the right and left ventricles of the heart and intraventricular hemodynamics were studied in 5 patients after closing the defects of the interventricular septum with an intravascular device. It was found that after occlusion of the defects by the safe and atraumatic endovascular device the right ventricular myocardium of the heart relaxed, the stroke index became less, the ejection fraction became greater, the diastolic diameter of the right ventricle decreased, and of the left ventricle increased. The indices of the regional contraction function of the tri-cuspid and output sectors of the input part and external and pulmonary output sectors of the right ventricle were found to normalize.

[Lactogenous fistula].

In the patients with an acute purulent lactational mastitis were studied the rate and aetiology of lactogenous fistula (LF) and its influence on the disease course. Classification of LF was elaborated, were substantiated suboperational prophylaxis and early treatment of LF using the "governed" lactation, cryotherapy. For the lengthy functioning LF the tactics of treatment was determined.

[Classification and formulation of clinical diagnosis in peptic ulcer].

An ulcer disease classification, basing on the disease clinical course criteria and the local changes characteristics, is proposed. An adequate estimation of the patient state and optimal tactics of treatment are guaranteed by the diagnosis formulation using the classification.

[Surgical treatment of perforated gastroduodenal ulcer].

The surgical treatment tactics of patients with perforative gastroduodenal ulcer was substantiated, basing on the complex analysis done. Radical intervention, mainly organ preserving one, constitutes the operation.

[Social characteristics of patients in penitentiaries++].

Three hundred and forty five convicted patients with tuberculosis underwent sociohygienic examinations. Among the examinees, the disease was first detected in 74.2% in a reformatory, in 25.8% in an investigating cell. The persons who fell ill with tuberculosis in places of confinement were mainly young, their ages ranged from 20 to 39 years. Only every 4 patients with tuberculosis are condemned for the first time, more than half the patients have over 3 convictions and every 5 patients have over 5 convictions. The most socially disadapted individuals fall ill with tuberculosis in the reformatory; among the examinees, 7.6% were homeless before conviction. The overwhelming majority of the convicts had no social contacts: 52.2% were never married. The study ascertained that in addition to the negative social environment, some criminogenic, personal, and behavioral factors of this contingent predispose to tuberculosis.

[Effectiveness of chemotherapy in combination with electrophoresis and ultraviolet irradiation of blood in newly diagnosed patients with destructive pulmonary tuberculosis].

Efficacy of inpatient treatment was compared for 222 new-onset cases of destructive tuberculosis of the lungs. 86 patients received chemotherapy plus electrophoresis and UV blood irradiation (group 1), 136 patients received chemotherapy alone (group 2). Group 1 patients benefitted more; bacterial discharge ceased in 100%, destruction in 89% of patients within 3 months against 59% and 38%, respectively, in controls. Combined therapy prevents toxic allergic reactions and shortens hospital stay by 48 days.

Left ventricular function immediately after exercise in elite cyclists.

To determine the response to exercise of the left ventricle of endurance-trained athletes, 6 elite (world class) cyclists were compared to 6 untrained healthy control subjects. In athletes the stroke volume increased with exercise. In untrained volunteers the stroke volume did not change with exercise. This difference of the response of the stroke volume to exercise reflected a difference of the left ventricular end-diastolic volume. In athletes the left ventricular end-diastolic volume tended to increase. In control subjects the end-diastolic volume decreased. In conclusion, athletes increased cardiac output by increasing stroke volume and heart rate, whereas control subjects increased their cardiac output only by increasing their heart rate.

[The use of highly dispersed iron particles for increasing the sensitivity of the luminescent microscopic method of determining Mycobacterium tuberculosis].

A highly dispersed ferromagnetic powder obtained by a plasmochemical method (particle size was 100-500 A) was treated by means of an ultrasonic disperser: suspension was added to the trisodium phosphate homogenized and neutralized sputum (0.3 mg of the initial powder per 1 ml of sputum) of patients with various forms of pulmonary tuberculosis. The sputum was then incubated at slight stirring for 40 min and centrifuged; the precipitate was used to prepare smears which were stained with auramine; mycobacteria were detected by luminescence microscopy. The ferromagnetic suspension was found to increase luminescence microscopy sensitivity to 85.4%. The efficacy of the method was 30.8% more than that in the cultivation of infectious material in solid nutrient media.
