[Arterial and venous fistulas of duodenal-biliary zone].

Causes of gastrointestinal bleedings in patients treated from 1995 to 2001 were analyzed. Rare causes of bleeding were studied particularly. Four cases of patients hospitalized urgently for gastrointestinal bleeding are described in detail. In autopsy of these patients different variants of vascular fistulas of duodenobiliary zone were revealed: fistula of aneurysm of abdominal aorta with blood bursting into horizontal part of the duodenum (2 cases), fistula between diverticulum of proximal part of jejunum and abdominal aorta (1), choledochoportal fistula (1). Clinical and anatomical correlations differential-diagnostic difficulties were analyzed.

Hemostasis in children with dysbacteriosis in chronic constipation.

The purpose of the study was to investigate hemostasis in children with dysbacteriosis disturbance in chronic constipation. The disturbance of factors in the inner mechanism of blood coagulation (VII, IX, XI, XII) in compensated chronic constipation was defined based on the reduction in colon bacillus levels. We observed hypocoagulation caused by the reduced activity of the prothrombin complex factors, disaggregate thrombocytopathy, and endotheliosis with fibrinolysis inhibition in subcompensated chronic colostasis with continuous reduction of colon bacillus levels and pathogenic microflora appearance. In decompensated colostasis there was an increase in pathogenic microorganisms and a continuous reduction of colon bacillus levels. In hemostasis there was a factor deficiency in inner (XII, XI, IX, VIII) and outer (II, V, VII, X) blood coagulation mechanisms. Fibrinolysis inhibition, endotheliosis development with thrombocyte aggregation, and microthrombosis formation were determined. Thus, in children with chronic constipation, there was a marked reduction in the amount of colon bacillus, which led to the reproduction of pathogenic bacteria. We also observed chronometric hypocoagulation with the inner (XII, XI, IX, VIII) and outer (II, V, VII, X) mechanisms of blood coagulation, at the base of which there is the deficiency of vitamin K-dependent factors (II, VII, IX, X) and a slightly marked disturbance in the final stage of coagulation. In thrombocyte vascular hemostasis, thrombocytopathy was observed with increased adenosine-5-diphosphate aggregation and the inhibition of the inner mechanism with fibrinolysis and endotheliosis.

[Hyposelenoses].

Literature on the various manifestations of the selenium deficiency in humans and animals is presented. Mechanisms of the biological effects of selenium brought about by glutathione peroxidase are considered. Biogeochemical anomalies, parenteral nutrition, malabsorption syndromes as conditions for the development of hyposelenosis are characterized as well as their manifestations (white-muscular disease, myopathies, cardiomyopathies, Keshan disease, Kashin-Beck disease). Selenium deficiency facilitates the development of the ischemic heart disease, myocardium infarction, certain types of malignancy and so on. The attention is drawn to the link of the selenium deficiency with imbalance of other trace elements.

[Genetic characteristics of the Yakut aboriginal cattle and its hybrids].

Materials are presented concerning the analysis of blood groups in the Yakutsk aboriginal cattle (n = 960) and its hybrids (n = 145) bred in Verkhoyansk and Yakutsk regions of Yakutskaya ASSR with respect to the problems of its origin. The genetic similarity of the Yakutsk cattle and its hybrids was evaluated; the cattle populations were studied in various years. The experiment also involved some hybrids bred in the northern part of the Soviet Union. The strongest similarity was found for the Yakutsk aboriginal breed and the cattle bred in Kalmytskaya and Buryatskaya Regions (0.841-0.839), i.e. those which are ascribed to the same group, according to their origin. Similarity with the Ashire and hybrids of Zebu and Black and White cattle was found to be less pronounced. The frequencies of antigens ranged from 0.2 (the antigen H") to 100% (the antigen F). The antigen frequencies were found to be higher in purebred animals, as compared to the hybrids almost within the whole antigen spectrum.

[Morphological changes in the cerebral cortex after a stroke].

29 brains of right-handed persons who died within the period of 4 hours to 6 years after the stroke were studies. The patients had various disorders of speech and movements, partial or complete speech recovery was observed in 10 cases. Aphasias resulted from the lesions of associative fibers connecting the speech zones of the brain cortex, direct destruction of speech regions, in cases of hematoma pressure on cortex speech areas. Changes in neurons, nerve fibres and neuroglia in the cortex of speech areas were determined by the time passed after the stroke. They were also dependent on the site and size of the focus and, to a certain extent, correlated with a clinical picture. The changes also depended on localization and the size of lesion. Sometimes the changes correlated with clinical data. Clinical and morphological comparisons indicate that a correlation exists between speech recovery degree and the intensity of repairing processes in neurons and nerve fibres in cortical speech areas.

[Rupture of an atherosclerotic aneurysm of the common iliac artery].

A complex symptom complex developed in a 76-year-old man with atherosclerotic right artery ililaca communis aneurysm and was caused by retroperitoneal rupture. It was characterized by incomplete small intestinal ileus, progressive hydronephrosis, ischemia and chronic venous insufficiency of the right leg.

[An immunohistochemical study of the primordium and primary differentiation of the parafollicular cells in the human thyroid].

Formation and differentiation of calcitonin-secreting cells has been studied in human embryos. The cells form from the entodermal part of the rudiment of the last pharyngeal cavity. They do not secrete calcitonin before incorporation into the thyroid gland. The first calcitonin-secreting cells appear in the thyroid gland as a diffuse net. The properties and origin of this net have been described.

[The use of a fluctuating current in the combined therapy of periodontitis].

Multiple-modality treatment of 63 patients with periodontitis included removal of dental deposit, curettage, if indicated, followed by fluctuating current treatment. The control group was made up of 25 patients in whom dental deposit was removed and oral hygiene was followed up. The periodontal status was assessed by clinical, functional, and morphologic methods. Liquidation of gingival inflammation, resultant from multiple-modality treatment, was associated with normalization of the regional hemodynamics in periodontal tissues.

[New data on the epidemiology and morphology of Viliuisk encephalomyelitis].

Viliuisk encephalomyelitis (VEM) appears to be endemic disease, affecting native population in Yakutia (Yakut, Even, Evenk). The infectious nature of VEM is very likely, but not yet established with certainty. The mortality rate caused by VEM from 1950 to 1990 was 17.4 per 10000 of native population. The maximum morbidity can be seen in Viliui and Central zones of Yakutia. Women were affected one and a half times more than men. VEM is characterized by multiple micronecrotic foci with marked inflammatory reactions and gliosis in the brain tissue. The vascular component in VEM is manifested by vasculitis, plasmarhagies, obliteration of the capillary lumen with development of capillarofibrosis, progressive reduction of the microcirculatory bed, the deformation of walls in major vessels with atrophy of their muscular layer, in some cases--by hyaline thrombi. The data presented give no grounds to consider the vascular component as secondary, relating only to the brain atrophy.

[The use of perfusion for brain fixation].

Two modifications of the apparatus for the fixation of the whole human brain are described. This apparatus can also be used for the fixation of other organs and for dead body embalming.

[Rupture of atherosclerotic aneurysms of abdominal aorta into the duodenum].

A woman, 77 years old, and two men aged 65 and 70 years died 28-96 hours after sudden beginning of recurrent massive gastrointestinal hemorrhages of clinically unknown origin. Ruptured atherosclerotic abdominal aortic aneurysms with aortoduodenal fistulas were revealed at autopsies. Anatomical predispositions for these rare complications and the causes of the diagnostic mistakes are discussed.
