Dental treatment in a patient with malignant pheochromocytoma and severe uncontrolled high blood pressure.

A young female with severely uncontrolled hypertension suspected to result from malignant pheochromocytoma underwent combined medical and dental care for the treatment of acute dental pain. The dental treatment was performed with the patient under intravenous sedation and careful perioperative management with the use of high doses of alpha and beta receptor blocking agents. The procedure was completed without intraoperative problems and an uneventful posttreatment course.

Dental treatment in very high risk patients with active ischemic heart disease.

A new method of predental treatment and management for patients with active ischemic heart disease is reported. Patients with unstable angina pectoris or who have just had a myocardial infarction underwent full treatment for pain-induced dental problems, with the use of an incremental three-level antiangina and antianxiety medication: before dental treatment; in the waiting room, and c) during dental treatment. The setting was a tertiary referral oral medical service and hospital dental clinic. Twenty-six patients were treated, 16 men, 10 women, ranging in age from 45 to 68 years. Ten patients with unstable angina pectoris and 16 patients less than 3 months after a myocardial infarction. All patients underwent comprehensive dental treatment. Good cardiovascular control was achieved, and all the procedures were uneventfully completed. This study indicates the need to reconsider the absolute versus relative contraindications for dental treatment in patients with active ischemic heart disease and recommends the use of a gradual three-level therapy.

Comparison between the psychopathological profile of dental anxiety patients and an average dental population.

The psychological characteristics of 38 patients with extreme dental fear were compared with those of an age and sex matched control group chosen from the average population visiting the dental clinic. The results were assessed by means of the Kleinknecht Dental Anxiety test, the Brief Symptom Inventory and Spielberger Anxiety State questionnaires. The majority of the dental fear patients were women, the male/female ration being 0.29. In the sample group, the dental anxiety manifested as a state and not as a trait: the anxiety state mean score of the dental phobic group was significantly higher than that of the controls (p less than 0.05), while the mean score for anxiety trait did not differ. Profound psychopathological distress other than anxiety was present only among the phobic males.

Alveolar bone changes in experimentally induced prediabetes and diabetes.

Alveolar bone resorption could be the result of local and systemic nocive factors. Diabetes mellitus is considered to be one of the conditions capable of inducing this resorption. Using rat models diagnosed as diabetics and prediabetics, alveolar bone resorption was measured directly on the jaws. Significant differences were found between the diabetic and prediabetic rats when compared with the controls. Our results confirmed that significant alveolar bone resorption occurs in animals with only a genetic predisposition to develop diabetes. Bone loss in prediabetic and diabetic rats is a noninflammatory atrophic degeneration that requires further investigation.

Kindler syndrome in two related Kurdish families.

We describe a 19-year-old girl with Kindler syndrome. She has suffered from bullae on pressure areas of the skin since birth. These healed with atrophic scars and caused marked atrophy of the skin of the palms and soles and wrinkled and parchment-like skin of the dorsum of the hands and feet. Since infancy the patient has also suffered from severe photosensitivity on exposed areas and developed poikiloderma on the skin of her face, neck, chest, and upper back. Since age 17 years the patient has been free of bullae but moderate photosensitivity exists. Oral examination showed limitation of mouth opening, ankyloglossia, overjet malocclusions, and atrophy of buccal mucosa with widespread white macules. Results of laboratory tests were within normal limits. The proposita's parents (family 1) are Jews of Kurdish origin and first cousins. She is the only one affected among five siblings. Family 1 is related to a second family (family 2) through a common great-grandfather. The parents of family 2 are first cousins and they have three affected children. An autosomal recessive mode of inheritance of Kindler syndrome is suggested in these two related families.

Intraligamentary anesthesia--a histological study.

Intraligamentary dental anesthesia is considered to be an efficient and clinically safe anesthetic technique, and as a result, is widely used. The present study evaluates histologically any possible damage to the periodontal apparatus, in baboon monkeys. The damage induced by the injection needle and/or the anesthetic solution, which was injected under high pressure, was localized, minor and reversible. In most of the specimens, all signs of damage disappeared within 8 days following injection. In others, some granulation tissue was evident in the lamina propria 15 days following injection. However, no bone or cemental damage was observed.

Intraligamentary--intraosseous anesthesia. A radiographic demonstration.

Intraligamentary dental anesthesia has become a widely accepted technique. The periodontal ligament seems to provide easy access to the tooth apex. In the present study, radiopaque material was injected into baboon monkeys. Serial radiographs during incremental injections showed clouding of the crestal bone. The material was seen gradually advancing through the alveolar bone crest, apically. The spread was noticed through the marrow spaces, unexpectedly avoiding the PDL route.
