Quantitative analysis of myocardial ischaemia on end-diastolic thallium 201 perfusion images.

ECG gating of images with thallium 201 (201Tl) may improve the representation of myocardial perfusion by allowing isolation of the end-diastolic (ED) portion of the cardiac cycle. To characterize this phenomenon, circumferential distribution profiles for ED and conventional total-cycle images (TC) with 201Tl were analysed after summing ECG gated acquisitions; redistribution was defined as net washing of 201Tl for specific myocardial segments visualized on the left anterior oblique images. Scintigraphic data were compared with the presence of coronary stenoses in 150 patients. ED images had relatively less activity in the inferoseptal and inferoapical region than TC profiles. The ED mode of washout analysis provided more frequent confirmation of visually suggested redistribution than did the TC mode (84% incidence of net wash in for ED, 69% for TC). ED analysis showed a higher incidence of net 201Tl wash in in relation to critical circumflex and left anterior descending artery lesions, but yielded a similar overall performance to TC analysis in distinguishing patients with diseased coronary arteries from normal subjects. Thus, in selected cases, analysis of ED images could assist in confirming 201Tl redistribution.

Dipyridamole combined with symptom-limited exercise for myocardial perfusion scintigraphy: image characteristics and clinical role.

Although dipyridamole can be used with myocardial scintigraphy to demonstrate reversible perfusion defects, combining exercise with the pharmacologic tool could improve image quality and information yield. The incidence of perfusion defects and the quality of thallium 201 images were reviewed in a series of 820 patients who had been assigned to a specific stress-test mode. Supine bicycle exercise alone was used (group I) where no pharmacologic or physical factors (e.g., beta-blockers, arthritis) limited performance; otherwise, intravenous dipyridamole was followed by symptom-limited exercise (group II). Angiographic correlation was available in 57 patients in group I, and in 158 in group II; of these, 109 performed significant exercise (greater than or equal to 3 min at increasing workloads) following dipyridamole (group IIA), whereas in 49 (group IIB) the exercise phase following dipyridamole was truncated. All test-mode groups were similar with respect to the incidence of ST segment depression during testing, patient throughput, and the sensitivity of perfusion defects. Chest pain and reversible defects were induced more frequently in group II than in group I. In group IIA, splanchnic background activity was lower (P less than 0.001) than in group IIB, and the false-positive rate tended to be lower. Thus, combining exercise with dipyridamole in patients with non-cardiac limitations to exercise enabled the achievement of optimal results for perfusion scintigraphy.

Renal uptake of Tl-201 in hypertensive patients undergoing myocardial perfusion imaging.

The detection of renovascular disease (RVD) has particular relevance in hypertensive patients (HP) who have symptoms of target organ damage. To evaluate the possibility of RVD in HP undergoing myocardial perfusion scintigraphy for chest pain symptoms, posterior renal images were obtained at 1-3 hours after Tl-201 injection. Analog and computer images were obtained for 5 minutes in 45 HP; 12 patients with no history of hypertension or renal disease served as normal controls. For qualitative analysis, images were coded and read by three observers as to symmetry of renal uptake. Differential renal uptake of Tl-201 (DRU) was quantitated on computer images. In normal controls, uptake was agreed on as symmetric. In HP, 6 patients had marked asymmetry of DRU and 4 had possibly significant asymmetry; 2 had decreased uptake in both kidneys suggesting bilateral RVD or nephrosclerosis. Objective correlation with DRU was obtained in 10 HP who had contrast angiography, confirming 4 cases of unilateral RVD and 2 of bilateral RVD. Thirteen patients also had renography with Tc-99m DTPA; differential renal function by this modality correlated well with DRU of Tl-201 (r = 0.98). Thus, DRU of Tl-201 can be used as a supplement to myocardial scintigraphy to identify HP who require further evaluation and treatment of RVD.

Amino and iodotamoxifens: synthesis, estrogen receptor affinity and biodistribution.

Both geometrical isomers (E and Z) of an aminotamoxifen (2) have been prepared as precursors of the corresponding E and Z iodotamoxifens (1). The ability of E and Z-1 and 2 to compete with [3H]estradiol for estrogen receptors in rat uterine cytosol was measured relative to Z-tamoxifen and estradiol. The four tamoxifen derivatives showed affinities ranging from 50% to 1600% of that of tamoxifen. Under the same conditions, tamoxifen's relative binding affinity was 0.2% of that of estradiol. Preparative routes to the radioiodo-tamoxifens, [131I]-E and Z-1, were also developed and provided approximately 100 MBq of 'no carrier added' material in 40-60% radiochemical yield. Study of the biodistribution of these radioligands in tumor-bearing mice demonstrated significant radioactivity in the tumors and in the uterus. For [131I]-E-1, target to background ratios reached 28 for uterus/blood and 10 for tumor/blood; corresponding optimum ratios for [131I]-Z-1 were 10 and 5. A washout study using estradiol indicated selective uptake in the uterus of Swiss white mice. However, tumor uptake and image contrast in humans following intravenous administration of either [131I]-E or Z-1 were insufficient to allow diagnostic use of the radioiodotamoxifens.

Hepatic uptake of technetium-99m HM-PAO in a fetus.

The normal biodistribution of technetium-99m HM-PAO ([99mTc]HM-PAO) includes significant uptake in the brain, liver, and kidneys. A pregnant patient studied with [99mTc] HM-PAO to confirm brain death provided an opportunity to examine the transplacental distribution of this radio-pharmaceutical in the unborn fetus. Uptake in the fetus after transplacental delivery is almost exclusively hepatic with a small amount of biliary excretion.

Multimodality imaging of early heterotopic bone formation.

An atypical heterotopic bone formation that was difficult to diagnose presented in a young paraplegic patient as an acute deep vein thrombosis. A number of imaging methods, including contrast venography, ultrasonography, conventional radiography, bone scanning, leukocyte scanning, computed tomography, and magnetic resonance imaging, were used to arrive eventually at the final diagnosis. Early bone scanning remains a sensitive and effective method of diagnosis. Computed tomography can be useful in difficult cases, but the role of other imaging studies appears limited.

Ischemic left ventricular dysfunction assessed on ECG-gated thallium-201 myocardial perfusion images.

Ischemic dysfunction of the left ventricle can be suggested by ancillary data derived from thallium-201 myocardial perfusion images. In this study, qualitative and quantitative assessments of global and segmental contraction derived from ECG-gated left anterior oblique images were analyzed to define more precisely transient ischemic hypokinesis. Immediate (4 mins post stress) and delayed (2 to 4 h) images were compared in 200 patients; 165 had coronary angiography and 35 had a low probability of coronary artery disease based on pretest and test outcome variables. For both immediate and delayed images, a quantitative index of left ventricular contraction (derived from the time-activity curve of the left ventricular cavity and validated in a previous study), correlated well with contrast ventriculography scores. The index derived from the immediate image also was related to the severity/extent of coronary artery lesions and to thallium-201 lung uptake. The ratio of indices (immediate/delayed) was depressed (P less than 0.001) in patients with two or three critically diseased vessels, and reflected the qualitative assessment of stress-induced dysfunction on cinematic images. These data suggest that the quantitative index derived from ECG-gated perfusion scans may be a valuable indicator of stress-induced ventricular contractile dysfunction.

Screening for a renovascular etiology in hypertensive patients undergoing myocardial scintigraphy: differential renal thallium-201 uptake.

In hypertensive patients with target organ damage the search for possible etiologic factors may be particularly relevant. To evaluate renal symmetry, differential renal uptake of thallium-201 was quantified on renal images at 30 mins to 3 h after tracer injection in 112 hypertensive patients undergoing myocardial perfusion scintigraphy for chest pain. In some patients, renal angiographic status was known at the time of thallium-201 scintigraphy (n = 10), or disease was highly suspected (failed angioplasty, n = 6); the remainder (n = 96) were 'screened' for renovascular disease by renal thallium-201 imaging; 32 normotensive patients with no history of renal disease served as controls. Validation of thallium-201 renal imaging was obtained in a total of 17 hypertensive patients who had correlating contrast angiography, confirming eight cases of unilateral or asymmetric bilateral renovascular disease. Renography with technetium-99m DTPA was performed in 28 patients; differential renal function according to this modality correlated well with differential renal uptake of thallium-201 (r = 0.98). Of the screened hypertensives, eight had marked asymmetry of differential renal uptake and eight had possibly significant asymmetry. Renal thallium-201 imaging provided the first evidence of asymmetric renal perfusion in four cases subsequently confirmed by abdominal angiography. In addition, four post angioplasty cases had persistent or progressive asymmetry of renal perfusion disclosed by this technique. Thus, ancillary renal imaging with thallium-201 can be used to identify hypertensive patients who should be considered for renal angiographic evaluation.

Effect of cancer on the in vivo energy state of rat liver and skeletal muscle.

The effect of increasing tumor burden on host liver and skeletal muscle energy status was studied using P-31 nuclear magnetic resonance spectroscopy (NMR), in rats inoculated with a nonmetastasizing methylcholanthrene-induced sarcoma (TB), and compared to nontumor bearing (NTB) and pair-fed (PF) rats. During the 28-day study, serial measurements of body weight, food intake, and tumor volume were obtained. Using a 0.9-cm double-turn surface coil, weekly NMR measurements were obtained from liver and skeletal muscle. An increasing ratio of [Pi]/[ATP] was used as one measure of intracellular energy depletion. [Pi]/[ATP] in NTB rats remained constant over time at 0.78 +/- 0.10 in liver, and 0.30 +/- 0.10 in skeletal muscle. In TB rats, the [Pi]/[ATP] ratio increased significantly in liver (P = 0.00002) and skeletal muscle (P = 0.04) with increasing tumor burden. In PF rats, no significant change occurred in [Pi]/[ATP] in liver or skeletal muscle, indicating that declining food intake was not responsible for the change in [Pi]/[ATP] seen in TB rats. Surface-coil spectroscopy of liver and skeletal muscle permits serial measurement of visceral energy stores. Increasing tumor burden results in early, ongoing depletion of energy stores as reflected by increasing [Pi]/[ATP] in these organs.

Clinical evaluation of serum thyroglobulin measurement using a commercial kit in the diagnosis of recurrent thyroid cancer.

The value of serum thyroglobulin assay employing a kit manufactured by Diagnostic Products Corporation in the detection of recurrence of thyroid carcinoma in patients treated by thyroidectomy and ablative therapy was assessed by clinical follow-up and radioiodine scanning of 122 patients over a 2-year period. A total of 204 specimens were analysed. The assay was found to be sensitive and highly specific for the detection of recurrent thyroid carcinoma provided that lipemic sera are clarified by ultracentrifugation prior to measurement and that results from those patients who demonstrate positive serum antithyroglobulin antibodies are excluded. The predictive value of a serum thyroglobulin level above 20 pmol/L was 96% for recurrence of thyroid carcinoma. The predictive value for non-recurrence was 98% for serum thyroglobulin levels below 10 pmol/L. The specificity and sensitivity of the assay were poor for serum thyroglobulin levels between 10 and 20 pmol/L. In recurrence-free patients, the serum thyroglobulin levels were not changed by withdrawal of thyroid replacement therapy 4-7 weeks prior to sampling.

Cerebral perfusion imaging with technetium-99m HM-PAO in brain death and severe central nervous system injury.

We performed 38 cerebral perfusion studies in 33 patients with brain death or with severe central nervous system injury using technetium-99m hexamethyl-propyleneamine oxime [( 99mTc]HM-PAO). Uptake by the cerebrum and/or cerebellium was present in all patients who were not clinically brain dead (ten studies) although the study was often abnormal. In those patients who were brain dead, 16/17 studies demonstrated no uptake in either the cerebrum or cerebellum. In patients suspected of brain death, but who had conditions interfering with the diagnosis the test demonstrated no uptake in 9/11 studies, confirming brain death. A radionuclide angiogram (RNA) of the head was also performed in 33/38 studies and showed complete agreement with the [99mTc]HM-PAO uptake, except in one case. We conclude that cerebral perfusion imaging with [99mTc]HM-PAO is a simple, noninvasive and reliable test to confirm brain death. By comparison with conventional technetium agents, [99mTc]HM-PAO is not dependent on the quality of the bolus injection, is easier to interpret and allows evaluation of posterior fossa blood flow.

Estimation of left ventricular function on ECG-gated thallium-201 myocardial perfusion images.

ECG-gated thallium-201 perfusion images (GT) may provide information concerning left ventricular contraction. To assess the objectivity of this information, qualitative and quantitative evaluations of left anterior oblique GT images were performed in 600 patients and compared with contrast ventriculography (n = 180) and gated blood pool scintigraphy (n = 60). Cinematic playback of GT allowed qualitative rating of ventricular performance in 98% of studies. Abnormal GT (focal or global hypokinesis) was found in 41% of cases and could be related to myocardial infarction, nonischemic cardiomyopathy or left bundle branch block. Blinded readings showed reasonable interobserver agreement and correlation with blood pool scintigraphy regarding segmental and overall left ventricular function; decreased tracer uptake in an abnormally perfused segment presented only occasional difficulty. A quantitative index was derived by relating the increment in left ventricular cavity activity during contraction to myocardial count density; results were expressed as a planar ejection fraction (PEF). PEF correlated well with ejection fraction on blood pool scintigraphy (r = 0.83, P less than 0.001) and with contractility scores derived from ventriculography (P less than 0.001) and seemed suited for identification of hypokinetic ventricles. Thus, GT allows objective estimation of left ventricular function as an addendum to myocardial perfusion imaging.

Metabolic changes in human muscle denervation: topical 31P NMR spectroscopy studies.

Metabolic changes in human forearm flexor muscle following partial or complete denervation were studied using 31P nuclear magnetic resonance spectroscopy. In resting muscle, patients with electromyographically verified forearm flexor denervation had a lower ratio of PCr/Pi than healthy controls [4.76 +/- 2.50 (SD) versus 6.50 +/- 1.55 (SD); P less than 0.01] and a higher intracellular pH [7.09 +/- 0.06 versus 7.05 +/- 0.04 (SD); P less than 0.01]. These changes were more apparent when the diseased arm was compared to the contralateral healthy arm in patients with strictly unilateral denervation. Five patients with severe denervation had the lowest PCr/Pi ratio (1.94 +/- 0.70) and the highest pH (7.15 +/- 0.04). Four patients with intact innervation and disuse atrophy as a result of a forearm fracture did not exhibit these changes immediately following removal of the forearm cast. The findings show that denervation leads to a reduction in PCr/Pi within muscle and a rise in intracellular pH, and that these changes correlate with the degree of denervation and improve with recovery. 31P MRS may be useful to follow the course of denervation atrophy.

Vasoactive hormones in the renal response to systemic sepsis.

The pathophysiology of renal dysfunction in generalized sepsis remains unknown. In this study, 24 hours after surgical induction of peritonitis in 20 volume-loaded sheep, three patterns of renal function were seen. In group 1 (n = 8), glomerular filtration rate (GFR) decreased by 70%, urine volume by 85%, absolute sodium excretion by 95%, and fractional sodium excretion by 83%. Group 2 (n = 4) exhibited similar sodium retention but GFR did not fall. Group 3 (n = 8) showed no change in GFR or urine volume and only minimally reduced sodium excretion. Mean arterial pressure fell 17% in group 1 only; central venous pressure, pulmonary capillary wedge pressure, and plasma volume were maintained at or above presepsis values in all groups. Cardiac index was either increased or unchanged, and renal plasma flow was maintained in all groups; there was thus no hemodynamic evidence to suggest volume contraction. Histologic examination showed only minor changes with no consistent pattern. Renal functional changes correlated with other manifestations of severe sepsis--GFR and sodium retention correlated significantly with increased cardiac index, decreased systemic vascular resistance, pulmonary arterial hypertension, leukopenia, hypoproteinemia, and hypoglycemia. All of these changes were most marked in group 1. In groups 1 and 2, plasma renin activity (PRA) increased and urinary kallikrein excretion decreased. PRA correlated inversely with GFR, urine volume, and sodium excretion; urinary kallikrein excretion correlated positively with urine volume and sodium excretion. Urinary excretion of 6-keto-PGF1 alpha was increased in groups 1 and 2 and correlated inversely with mean arterial pressure in group 1 animals. During sepsis, urinary thromboxane B2 excretion continued at presepsis values in all groups. The results suggest that unusual reciprocal changes in activity of the renin-angiotensin and renal kallikrein-kinin systems may play a role in the renal response to sepsis. PGI2 synthesis is increased and may affect systemic hemodynamics and renal function; the role of thromboxane A2 in this context is unknown.

In vivo red blood cell compatibility testing using indium-113m tropolone-labeled red blood cells.

In vivo radionuclide crossmatch is a method for identifying compatible blood for transfusion when allo- or autoantibodies preclude the use of conventional crossmatching techniques. A technique for labeling small volumes of donor red blood cells with [113mIn]tropolone is reported. The use of 113mIn minimizes the accumulation of background radioactivity and the radiation dose especially so when multiple crossmatches are performed. Labeling red cells with [113mIn]tropolone is faster and easier to perform than with other radionuclides. Consistently high labeling efficiencies are obtained and minimal 113mIn activity elutes from the labeled red blood cells. A case study involving 22 crossmatches is presented to demonstrate the technique. The radiation dose equivalent from 113mIn is significantly less than with other radionuclides that may be used to label red cells.
