[Surgical method for correction of absolute valvular insufficiency in post-thrombophlebotic disease of the lower limbs].

A new technique has been worked out for intravasal autovenous valvuloplasty. It is based on the "ink-pot that does not spill" principle with creation of two cusps and is used in the treatment of patients with post-thrombophlebotic disease of the lower limbs induced by absolute valvular incompetence of the great veins. Criteria are offered for assessment of the efficacy of the technique for formation of venous valves. Altogether 54 operations were performed including those provided to 29 patients (the main group) operated on according to the technique proposed and those in 25 patients (control group) operated on the basis of the known technique reported in the literature. In 51 patients, these operations were accomplished in combination with different variants of procedures on the communicating and saphenous veins of the lower limbs. The time of some follow ups was as long as 7 years. The beneficial short-term results were obtained in 96.5% of cases whereas the long-term outcomes were positive in 92% of the basic group patients.

[A complex program for the treatment of patients with post-thrombophlebitic disease of the lower limbs].

The aim of the work was to improve the results of the treatment of patients suffering from post-thrombophlebitic disease (PTPD) of the lower limbs. An analysis was made of the results of examination and treatment of 800 patients, obtained over the recent 12 years. Wide spreading of operations carried out on the saphenous and communicating veins led to a decrease in the number of the long-term positive results in the presence of valvular insufficiency of the deep veins. At the same time combined operations performed on all three venous systems according to the indications cannot provide for stable results. This circumstance necessitates the carrying out of preventive and anti-recurrence measures. This formed the basis for revision of the treatment policy and development of a complex program for combined surgical and conservative treatment, provided there are strict indications for each of them, depending on the character and the degree of venous insufficiency in PTPD. We have worked out our own technology for conservative treatment; preoperative preparation and postoperative rehabilitation constituted a part of the program suggested. The program for permanent rehabilitation includes an individual training of patients in elementary practices of the treatment and prevention of disease progression. According to the program we undertook the treatment of 343 patients; the positive results could be attained in 87.5% of cases as shown by the 6-year follow up.

Surgery of arterial embolism in patients with infective endocarditis.

A total of 265 patients suffering from infective endocarditis (IE) were operated on at the Center of Cardiology and Cardiovascular Surgery. Of these, 20 patients presented with arterial embolism of the peripheral arteries of varying standing (from in-hospital episodes of embolism to 8 months). Embolism of the cerebral arteries was identified in 8 (3.0%) patients with IE. In 5 patients, an embolus was located in the orifice of the internal carotid artery. Three persons were diagnosed to have embolism of the intracranial arteries. Thromboembolism of the limb arteries was present in 12 (4.5%) cases. Active IE was diagnosed in 11 patients; in 9 cases, IE was in a stage of remission, Bivalvular mitralaortic disease was present in 4 patients; aortic disease was identified in 9 and mitral disease in 7 patients. In 3 patients with acute disorders of cerebral circulation arising from embolism of the carotid arteries and in 3 patients with progressive acute limb ischemia due to embolism of lower limb arteries, the operations were performed on an emergency basis. In these cases, two stage operations were undertaken. Five patients presenting with infective aneurysms of lower limb arteries and thromboembolism of the peripheral arteries with chronic lower limb artery ischemia underwent one stage operations. The first stage of operation consisted in resection of the infected vascular segment and an aneurysm, the second stage in correction of valvular heart disease. The outcomes of operations were regarded as satisfactory in all the cases. No complications or lethal outcomes were recorded in the postoperative period.

[Oxygen transport and immune status in patients with congenital valvular heart disease].

Oxygen transport, its metabolic maintenance and immune status were studied in 17 patients with congenital valvular heart disease (CVD) having compensated (n = 8, group 1) and decompensated (n = 9, group 2) defects of hemodynamics. CVD patients with decompensated central hemodynamics and progressing hypoxia had impaired compensatory rearrangement of oxygen transport system. Accumulation of intracellular lactate, low activity of basic energetic cycles of blood cells most evident in decompensated CVD was observed in both the groups. In conditions of severe energy-structural deficiency and impaired function of oxygen transport systems, CVD patients develop secondary immune deficiency presenting with depression of basic immunoregulatory subpopulations of T- and B- cellular immunity (CD2, CD3, CD4, CD5, CD8, CD16, CD22).

[Changes in lipid metabolism in patients with arteriosclerosis obliterans after charcoal hemoperfusion and hypolipidemic therapy in the preoperative period].

The paper analyzes the results of the treatment of 26 patients with atherosclerosis obliterans of the lower extremities, which has been performed to make an intensive hypolipidemic preparation for an operation. The comprehensive approach based on the combination of medication (nicotinic acid, and its derivatives, cholestyramine, and SKH -2K charcoal hemoperfusion ) and efferent therapy reduced the plasma levels of total cholesterol by 27%, triglycerides to 48%, low density lipoprotein cholesterol to 40%, and increased high density lipoprotein cholesterol up to 72%. The similar course therapy may be repeatedly used in a follow-up of this group of patients.

[Reconstructive vascular surgery in stenosis and kinking of the internal carotid artery].

Combination of stenosis and kinking of the internal carotid artery was revealed in 7.9% of 126 patients who underwent operation for stenosis of the bifurcation of the carotid artery. Among 9 patients with kinking of the internal carotid artery (ICA) 5 had a C-shaped kink, 2 - an S-shaped kink, and 2 patients looping of the artery. The combination led to a great measure to diminished volumetric cerebral blood flow (30-35 ml/min/g). The operations were performed under conduction anesthesia. Average time of ICA compression - 13 min. 26 sec. Retrograde pressure ranged from 42 to 88 mm Hg. In 2 cases the ICA was implanted into a new opening in the common carotid artery after eversion endarterectomy. The defect in the bifurcation was sutured. In 5 cases (4 with a C-shaped kink and 1 with an S-shaped kink) the artery was replanted into the former opening after eversion endarterectomy and resection of the kinked segment of the ICA. In 2 patients with looping of the artery the loop was resected with end to end anastomosis into the ICA and typical endarterectomy from the bifurcation of the carotid artery. There were no fatal outcomes. The results of operative treatment were good.

[Preoperative preparation of patients with arteriosclerosis obliterans].

The article analyses the immediate results of preoperative management and treatment of 13 patients with atherosclerosis obliterans of the lower limbs on the basis of drug hypolipidemic therapy and hemosorption on nonspecific sorbents of type CKH. Drug therapy comprised nicotinic acid, its derivatives in a daily dose of up to 2.5 mg, cholestiramine up to 20 g daily, vitamin therapy, and prodectin. Hemosorption was conducted on CKH-2K once in 4 days; 7 patients were given 5 perfusions each, 6 patients--7 perfusions each. Considerable clinical improvement occurred, hemodynamics improved, improvement of lipid metabolism was manifested by decrease of total cholesterol by 22%, triglycerides by 10%, and low density lipoproteins by 43% and increase of high density lipoproteins by 100%. The Cch atherogenicity coefficient decreased by 41%. The expediency of conducting no more than 4 sorptions was revealed and substantiated. Reconstructive vascular operations were performed on the patients. The postoperative period was uneventful. The late-term postoperative period was marked by progressive improvement of rheographic and biochemical values and stable normalization of lipid metabolism.

[Surgical treatment of pathologic twisting of the internal carotid arteries].

A total of 5182 carotid angiograms were reviewed. Abnormal bending of the internal carotid artery was detected in 310 (16.4%) cases. Forty-seven such cases were analysed. Twenty-five operations were performed, including the resection with end-to-end anastomosis (8), the resection with redressing and implantation into the former opening on the common carotid artery for loop formation (6) or S-bending (7). In one case of C-bending in the internal and external carotid arteries, the kinking was eliminated by means of a resection of a part of the common carotid artery. Indications for surgery are specified with reference to the kinking variant. Optimum types of reconstructions are proposed. There were no fatal outcomes. Seventeen patients were followed up for up to 5 years. The results of surgical treatment were fairly good.

[Posttraumatic aneurysm of the heart].

A case of the posttraumatic aneurysm of the left heart ventricle in a man of 30 developing 2 years after a knife wounding is described. Aneurysmorhaphy under conditions of the artificial circulation was performed. The postoperative period was complicated by polymicrobial sepsis. The long being of the patient under the artificial lung ventilation is noted.
