Controlled clinical trial of three short-course regimens of chemotherapy for pulmonary tuberculosis in Nigeria--a preliminary report.

A controlled clinical trial of daily short course (6-month) chemotherapy in newly diagnosed cases of pulmonary tuberculosis in Nigerians was carried out. The three regimens used contained streptomycin, isoniazid, rifampicin and pyrazinamide in the initial phase; and isoniazid plus rifampicin or isoniazid plus rifampicin and/or pyrazinamide in the continuation phase. Sputum culture conversion was satisfactory after 2 and 6 months of treatment and no positive cultures were found one year after treatment had been completed. Side effects were few and consisted mainly of arthralgia, possibly associated with pyrazinamide.

Calcinosis universalis associated with tuberculosis.

A 12-year-old girl with clinical evidence of abdominal tuberculosis also had a widespread soft tissue calcification termed calcinosis universalis. Despite antituberculous therapy the patient died and autopsy confirmed tuberculous peritonitis. No cause could be found for the soft tissue calcification. The co-existence of the two diseases is probably incidental.

Tar, nicotine, and carbon monoxide yields of some Nigerian cigarettes.

Fourteen cigarette brands manufactured in Nigeria in 1981 were analysed to determine the tar, nicotine, and carbon monoxide yields. Five of the brands belonged to the high and middle to high tar category (greater than 22 mg/cigarette) and nine to the middle tar (17-22 mg/cigarette) category. None of the cigarettes was in the low to middle and low tar (less than 17 mg/cigarette) category. The nicotine and carbon monoxide yields were similar to those of European cigarettes. Tobacco companies need to manufacture low tar cigarettes in the Third World as is the practice in the economically developed parts of the world.

Serum thyroxine, triiodothyronine, reverse triiodothyronine, thyroid stimulating hormone, thyroxine binding globulin and thyroxine binding pre-albumin concentrations in healthy African adults.

A study of the serum concentrations of the thyroid hormones thyroxine (T4), triiodothyronine (T3) and reverse triiodothyronine (rT3), thyrotropin (TSH) and the thyroid hormone binding proteins (thyroxine binding globulin (TBG), and thyroxine binding pre-albumin (TBPA)) in non-hospitalized adult Nigerian African subjects was conducted. The subjects were divided into three subgroups, male subjects, non-pregnant female subjects and pregnant female subjects to highlight peculiarities of each sub-group. All three subgroups showed elevation of TBG concentration as compared to British subjects. A statistically significant difference was observed in the concentrations of T4, T3, and TBG in the pregnant female subjects compared to the male and the non-pregnant subjects. A statistically significant difference was also observed between the concentrations rT3 in the male and pregnant female subjects compared to the non-pregnant female subjects. No statistically significant difference was observed between the concentrations of TSH, and TBPA, in all three subgroups, although the mean values for TSH were highest in the pregnant female subjects and the mean values for TBPA and albumin were lowest in this same group. The significance of these findings are discussed in relation to the evaluation of thyroid function in the adult African subject.

Clinical trial of sulbactam/ampicillin in the treatment of typhoid fever, and in-vitro sensitivity of isolates to sulbactam/ampicillin and chloramphenicol.

A trial of Sulbactam/Ampicillin in treatment of typhoid fever was carried out while in-vitro sensitivity of the drug was compared with chloramphenicol. Sulbactam/Ampicillin was found to be effective in 11 out of 12 patients who received the drug. The 1 patient who failed to respond had intestinal perforation with peritonitis. No side effects were encountered. The in-vitro study showed that 91.7% of the isolates were sensitive to sulbactam/ampicillin compared to 75% sensitivity to chloramphenicol. The difference was however not statistically significant (P < 0.05). It is concluded that sulbactam/ampicillin is effective and safe in the treatment of typhoid infection.

Catamenial pneumothorax: a case report.

A case of catamenial pneumothorax in a 17-year-old girl is presented. It is the first reported case in a Nigerian. Pleural blebs were found at thoracotomy and good response to dry sponge pleurodesis has been confirmed. The possibility of missed diagnosis of this entity in an environment where spontaneous pneumothorax is not uncommon is discussed.

Severe hypertension and haemospermia--a case report.

Haemospermia is an uncommon disorder of diverse aetiology, which includes diseases of the urethra, prostate and hypertension. There is no report of haemospermia in this environment and this communication describes a case associated with severe hypertension.
