[Evaluation of the hormonal status of patients with bronchial asthma by using various drug doses].

Drug loads were used to diagnose impairments of the hypothalamic-pituitary-adrenal system in 209 patients with bronchial asthma. A thyroid-releasing hormone test made in patients with moderate bronchial asthma who were taking no corticosteroids (CS) demonstrated a high level of thyroid-stimulating hormone at min 60 of the agent administration, which was indicative of decreased hypothalamic function. In 67.8% of patients, the vasopressin test was positive and suggested that the pituitary preserved its functional capacities. The decreased secretion of hydrocortisone and its active forms was observed in patients with severe bronchial asthma when CS was used in the daily dose adequate to 10-15 mg of prednisolone for 5 years. The 20-day therapy with dexamethasone demonstrated that the agent reduced the synthesis of hydrocortisone to a greater extent than that of corticosterone. The administration of depot-synacthen 24 hours later elevated the content of all hydrocortisone fractions and biologically active corticosterone. Depot-synacthen exerted an active stimulating action on the adrenal cortex.

[A comparative evaluation of methods of determining melanogen in the urine of patients with steroid-dependent bronchial asthma].

Fifty-four patients with steroid-dependent asthma of varying severity during exacerbation were examined to compare the efficacies of urinary melanogen measurement techniques. Jaksch's, Tormelen's methods, and the technique with 2M sulfuric acid were used. The tests were positive in but 3 (15%) patients with a medium severe course of the process and in 23 (68%) ones with a grave condition. Jaksch's and Tormelen's tests were similar in sensitivity, and the hydrochloric acid test was less demonstrative. Melanuria manifestation may be regarded as an early preclinical sign of adrenal insufficiency. Pigment metabolism studies in patients treated with steroids for a long time may be used as an extra method for the assessment of adrenocortical function and for monitoring the course of glucocorticosteroid therapy.

[The effect of injectable leukinferon on the immune reactivity of patients with steroid-dependent bronchial asthma].

The authors provide the results of the treatment with leukinferon for injections of 25 patients with steroid-dependent bronchial asthma. It is the authors' belief that leukinferon for injections may be recommended for introduction into multimodality schedules of the treatment of patients with the above-indicated pathology. The drug is tolerated well, without any phenomena of sensitization, when administered by inhalation in the form of aqueous aerosol in a dose of 10,000 IU/day. As a result of 6 to 8 injections, immunoreactivity showed a positive time-course of changes, the patients' clinical status improved, and the maintenance doses of glucocorticosteroids could be noticeably reduced.

[Analysis of the cause of death in patients with steroid-dependent bronchial asthma].

Description is given of 11 patients with a severe form of protracted corticosteroid-dependent asthma. They died in status asthmaticus. Most of these patients had protracted status asthmaticus and cardiopulmonary failure. Pneumonia with toxic liver and kidney affection developed in some of them in parallel with glucocorticoid therapy. Long-term glucocorticoid treatment may give rise to vasculitis and thromboembolism of the pulmonary trunk. The principal disease can be aggravated by infection, administration of histoglobin, decrease in the dose of glucocorticosteroids or their discontinuation.

[Effect of prodigiozan on immune reactivity in patients with steroid-dependent bronchial asthma].

An optimum scheme of prodigiosan therapy was developed for patients with steroid-dependent bronchial asthma taking account of the hormonal and immunological status. First the drug was administered at a dose of 0.1 ml of 0.005% solution, then in 3-4 days the dose was increased (in good tolerance) up to 0.2-0.4-0.8-1.0 ml. After the first administration of the drug at a dose exceeding 0.1 ml herpes virus could be activated in the body. Course therapy producing a stimulating effect on the immune system, inhibited the occurrence of autoimmune manifestations, prolonged bronchial asthma remission, permitted the reduction of retention of a maintenance dose of glucocorticoids.
